The impact of birth weight and gestational age on acute mastoiditis in children.

OBJECTIVE: Previous data correlate preterm and low birth weight (LBW) with acute otitis media, but there is a gap concerning the relations with acute mastoiditis (AM). This study investigates the effect of LBW and preterm birth on AM disease severity, neuro-otological complications, and recurrence. STUDY DESIGN: Retrospective cohort. SETTING: Tertiary medical center. METHODS: The cohort is retrospective in nature consisting of 294 children with AM admitted between 1999 and 2020. Data collection included: patient gestational age and birth weight, signs and symptoms, physical examination, lab…

Pediatric Acute Mastoiditis: Our Experience in a Tertiary Care Center.

Introduction Acute mastoiditis (AM) is a common intra-temporal complication of acute otitis media (AOM) and is more commonly seen in children. Occasionally, it presents as the first sign of ear disease. This study aimed to evaluate the clinical course of AM and determine therapeutic options for pediatric patients presenting with AM. Methods This was a prospective, observational study conducted on patients with AM presenting at a tertiary center during one year period. Convenience sampling was employed and 79 pediatric patients (18 years or below) were recruited for the study. Data on the demog…

Acute mastoiditis: 20 years of experience with a uniform management protocol.

OBJECTIVES: To characterize the clinical presentation of pediatric patients who, upon AM diagnosis, also had imaging-diagnosed ICCs (ID-ICCs); to define the group of AM patients at risk of developing ID-ICCs; and to update knowledge about organisms causing AM. STUDY DESIGN: Analysis of all AM patients admitted between 1997 and 2018 and treated according to an obligatory protocol including both brain imaging and sampling for bacterial culture upon clinical diagnosis of AM. RESULTS: Of 166 admitted patients (0.5-19 years old) 22 (13%) already had ID-ICCs. In patients who, on admission, had alr

Differentiating Acute Otitis Media and Acute Mastoiditis in Hospitalized Children.

Acute otitis media is a common infection in children. Most acute otitis media episodes can be treated at an outpatient setting with antimicrobials, or only expectant observation. Hospital treatment with parenteral medication, and myringotomy or tympanostomy, may be needed to treat those with severe, prolonged symptoms, or with complications. The most common intratemporal complication of acute otitis media is acute mastoiditis. If a child with acute mastoiditis does not respond to this treatment, or if complications develop, further examinations and other surgical procedures, including mastoide…

A case of hypertrophic cranial pachymeningitis associated with invasive Aspergillus mastoiditis.

We report a rare case of hypertrophic cranial pachymeningitis (HCP) associated with invasive Aspergillus mastoiditis. A 63-year-old man with diabetes mellitus underwent mastoidectomy because of chronic discharge from his left ear. The mastoidectomy was unsuccessful in resolving purulent otorrhea; moreover, 7 months later, the patient developed left abducens nerve palsy. Magnetic resonance imaging revealed HCP at the left middle cranial fossa. Although the pathogen could not be identified, an Aspergillus infection was considered based on elevated serum β-d-glucan and a positive Aspergillus anti…

Complicated cerebral venous sinus thrombosis with intracranial hemorrhage and mastoiditis.

Cerebral venous sinus thrombosis (CVST) is a rare cause of stroke, occurring when a blood clot forms in any of the brain venous sinuses. Sym … CVST with concurrent intracranial hemorrhage (ICH). We present a complex case of CVST with ICH and mastoiditis as well as provide a literature review about CVST.

[Fusobacterium necrophorum--cause of a mastoiditis with skull- and mandibular joint osteomyelitis].

BACKGROUND: The typical clinical manifestation of an infection with the obligate anaerobic, gram-negative, rod-shaped bacteria Fusobacterium necrophorum is the Lemierre syndrome. As the cause of osteomyelitis and mastoiditis factors of the normal bacteria flora are more likely to be found than Fusobacterium necrophorum. Nevertheless, Necrobacillosis is an important differential diagnosis of complicated courses of mastoiditis. MATERIAL AND METHODS: Because the clinical courses of mastoiditis with osteomyelitis may differ a lot, making the appropriate diagnosis more difficult, consistently and …

The effects of mastoid aeration on autophony in patients with patulous eustachian tube.

The effect of the mastoid pneumatization on sound transmission from the nasopharyngeal cavity to the inner ear, which is one of the important factors to indicate the severity of the patulous eustachian tube (ET), are investigated in patients with patulous ET. Twenty ears of 13 subjects with patulous ET were retrospectively analyzed. Sound transmission from the nasopharyngeal cavity to the inner ear was evaluated by observing the masking effects of noise presented in the nasopharyngeal cavity on the auditory threshold (nasal noise masking audiometry). The relationship between this masking effec…

Lessons learned: no increase despite clinical suspicion of acute mastoiditis.

Based on clinical information from one Norwegian hospital in 2005, an increase in childhood acute mastoiditis was postulated, but later nationally disproved. Our purpose was to explore the discrepancy between the clinically raised suspicion and Norwegian treatment data. Complete Norwegian data on children aged 0-16 hospitalized for acute mastoiditis in the period 1999-2005 was analyzed to study national variation in incidence by hospital, treatment region and age of hospitalization. In children below age 16 hospitalization rates for acute mastoiditis ranged from 4.9 to 6.3 from 1999 to 2005 an…

Hypertrophic chronic pachymeningitis associated with chronic otitis media and mastoiditis.

We describe the case history of a 70-year-old female patient presenting with bilateral hearing disturbance, facial paralysis, and vertigo. Radiological tests of temporal bone revealed soft tissue in the mastoid and tympanic cavities, and T1 weighted MRI revealed prominent Gd enhancement of the middle skull basal meninges. Middle ear inflammation appeared to induce pachymeningitis and to exacerbate associated symptoms, leading to a decline in the patient's overall condition. Bilateral mastoidectomies were effective in improving her general condition. Her hearing improved only on the right side …

T-cell lymphoma presenting as acute mastoiditis with a facial palsy.

Isolated T-cell lymphomas affecting only the mastoid are extremely rare. Presentation with oto-neurological signs prior to systemic involvement of a lymphoproliferative disease is also unusual. This is the youngest reported patient with a peripheral T-cell lymphoma with disease isolated only in the mastoid who presented with acute mastoiditis and a complete seventh cranial nerve palsy.

Tuberculous otitis media with mastoiditis and central nervous system involvement.

Tuberculosis of the middle ear and mastoid is currently a rare disease in developed countries, but this disease still occurs and may cause serious consequences. We report a case of disseminated tuberculosis involving the middle ear, mastoid, lung and central nervous system. Tuberculosis should be considered in the differential diagnosis of chronic ear drainage, especially in young children.

[Effects of pneumatization of the temporal bone on the onset and course of acute mastoiditis].

Roentgenogram planimetry according to Schuller was made in 75 patients with acute mastoiditis (AM) to study correlations between the degree of pneumatization of the temporal bone and AM onset. It was found that complication of acute otitis media purulenta with AM takes place only in patients with well developed pneumatic system. Development of intracranial complications occurs in cases with wider area of the cell system. Mastoiditis arises because of inadequate treatment of acute inflammation of the middle ear and aditoanthral obstruction.

The structure of the trabeculae of cancellous bone. 2. Long bones and mastoid.

The trabeculae of the mastoid, the upper end of the femur, and the tibia were examined to ascertain whether they contain vascular channels according to a pattern similar to that observed in the calcaneus. The trabeculae were serially sectioned in transverse planes. Each section was microradiographed and photographed under ordinary and polarized light. On the photos of the individual sections (1) the number of the vascular channels, (2) the thickness of the trabecular segments with or without osteons, and (3) the maximum distance of the osteocytic lacunae from filtering surfaces (i.e., haversia…

[Benign intracranial hypertension in childhood following mastoiditis (author's transl)].

Case report concerning a three years old boy with benign intracranial hypertension. The typical symptoms are results of increased intracranial pressure, associated with headache, vomiting, choked disk, occasionally dehiscence of cranial sutures and sixth nerve paresis. In computed tomography there were no tumor signs. In childhood a possible cause in thrombosis of the lateral sinus following an occult mastoiditis after antibiotic therapy. Mastoidectomy, intensive antibiotic therapy as well as serial lumbar punctures led to complete recovery in most cases.
