Nasal aesthetic changes after cleft orthognathic surgery remain understudied. Previous scarring associated with prior cleft surgery may affect the predictability of outcomes after jaw surgery. This study evaluates changes in nasal aesthetics using three-dimensional photography after Le Fort I advancement in patients with nonsyndromic cleft-related maxillary hypoplasia. Cephalometric parameters were recorded pre- and postoperatively. Three-dimensional photogrammetric imaging analyzed changes in interalar width (IAW), internostril width (INW), nasal tip projection (NTP), collumelar length (CL),

Distraction osteogenesis (DO) is a highly effective technique for correction of severe maxillary hypoplasia, especially in patients with orofacial clefts and craniofacial syndromes. The purpose of this retrospective, longitudinal study was to assess long-term airway alterations after maxillary advancement using a rigid external distraction system (RED) in non growing cleft patients. Fifteen cleft patients (8 males and 7 females) aged from 14 to 25 years were included in this study. All of them were treated with a rigid external distraction system for maxillary advancement after a high Le Fort

BACKGROUND: Lateral approaches have traditionally been used to gain access to lesions of the infratemporal fossa (ITF). However, dysfunction of the facial nerve secondary to its translocation, conductive hearing loss, and dental malocclusion because of mandibular head resection or dislocation are significant limitations associated with some of these approaches. Although facial nerve translocation and extended maxillotomy approaches avoid some of these drawbacks, they are invasive and require extensive osteotomies and facial incisions. To avoid these potential complications and maintain an extr

This article reports the case of a 19-year-old young man with Class III malocclusion and posterior crossbite with concerns about temporomandibular disorder (TMD), esthetics and functional problems. Surgical-orthodontic treatment was carried out by decompensation of the mandibular incisors and segmentation of the maxilla in 4 pieces, which allowed expansion and advancement. Remission of the signs and symptoms occurred after surgical-orthodontic intervention. The maxillary dental arch presented normal transverse dimension. Satisfactory static and functional occlusion and esthetic results were ac

The aim of this paper was to propose a new method of bimaxillary orthognathic surgery planning and model surgery based on the concept of 6 degrees of freedom (DOF). A 22-year-old man with Class III malocclusion was referred to our clinic with complaints of facial deformity and chewing difficulty. To correct a prognathic mandible, facial asymmetry, flat occlusal plane angle, labioversion of the maxillary central incisors, and concavity of the facial profile, bimaxillary orthognathic surgery was planned. After preoperative orthodontic treatment, surgical planning based on the concept of 6 DOF wa

Patients with cleft lip and palate frequently develop dento-facial deformity requiring orthognatic surgery. The origin of this deformity is therapeutic and surgeons are currently trying to prevent this iatrogenicity. The maxillary dento-facial deformity in these patients is a retrognathia with infragnathia, associated with endognathia, obliquity of the occlusal plane, with deviation of the superior incisive midline in case of unilateral clefts. The difficulties in the treatment of these skeletal deformities are due to the palatal, labial, and pterygomaxillary scar tissue. Orthognathic surgery

Changes in external nasal morphology after Le Fort I osteotomy can lead to reduced patient satisfaction and diminished quality of life postoperatively. Despite various modifications such as alar cinch suture and subspinal osteotomy to overcome such changes, the external nasal morphology may vary unexpectedly on an individual basis. This report investigated the factors influencing the changes in external nasal morphology, by considering the patient's preoperative external nasal morphology as well as the surgical technique, and the direction of repositioning of maxilla. Multiple regression analy

Surgical ciliated cysts occur primarily in the maxilla after radical maxillary sinus surgery. We report the first case of a surgical ciliated cyst that developed in the infratemporal fossa 25 years after the patient sustained severe facial trauma. The patient complained of mandibular pain and limited mouth opening. The patient's condition was completely resolved 5 months after marsupialization via Le Fort I osteotomy. Surgical morbidities can be minimized by proper diagnosis and less invasive surgery.

Traditional approaches to sinus floor elevation for dental implant placement have limitations. This technical report describes a modified transalveolar method of elevating the sinus membrane when 2 or more implants are placed simultaneously in a maxillary posterior area where the residual bone height is insufficient. With a narrow osteotomy connecting drilled holes, the Schneiderian membrane can be directly elevated with curettes as with the lateral approach, resulting in a reduced risk of perforation.

This study presents a technique to gain soft tissue volume in the paranasal region, using a xenogenous bonegraft wrap with a porcine pericardium collagen membrane in a patient with severe paranasal deficiency and midfacial hypoplasia. The technique consisted of using particulate bonegraft that is wrapped on collagen membrane and placed vertically and parallel to the lateral wall of the nasal cavity, over maxillary osteotomies. In a tomographic analysis of linear and volumetric measurements with 14 days and 6 months after the procedure, it was observed good stability of the vertical bonegraft w

PURPOSE OF REVIEW: To give an update on recent publications and tendencies concerning complications in cleft orthognathic surgery.
RECENT FINDINGS: Cleft-specific changes after orthognathic surgery and their impact on surgical outcomes are discussed. Focus lays on the causes and mechanisms of cleft-related surgical complications and strategies to prevent or minimize these complications. Bimaxillary surgery is seen as a safe procedure for cleft patients. Maxillary distraction, total or segmental, is pointed out as an alternative method to improve outcomes. Different techniques for osteotomies

BACKGROUND: Improving facial aesthetics has been shown to be a strong motivating factor in patients who decide to undergo orthognathic surgery. The nasolabial region is a keystone of facial aesthetics and thus is of central importance in planning and execution of orthognathic surgery. This article was performed to study modifications of nasolabial area after maxillary advancement.
METHODS: Forty-two patients undergoing orthognathic surgery were considered. In those patients, after Le Fort I osteotomy, only maxillary advancement was performed.
RESULTS: For each patient, several points in the

Osteogenic distraction is a bone regeneration and reconstruction technique. Razdolsky et al. (2001) [1] have defined it as "the process of creating new bone by stretching". Disjunction entails separating two anatomical structures at their junction system and, therefore, at a suture. Usually, it involves separating two semi-maxillae in the transverse dimension by means of an osteotomy. Transverse maxillary distraction appears to offer an alternative of choice to orthognathic surgery alone, which is frequently prone to relapse (Betts et al., 2004) [2]. The greatest benefit of osteogenic distract
