Status and prospect of novel treatment options toward alveolar and cystic echinococcosis.

Cystic echinococcosis (CE) and alveolar echinococcosis (AE) are the two most important global parasitic infectious diseases caused by species of Echinococcus granulosus and E. multilocularis, respectively. Although numerous trials have been performed in search of novel therapeutic options to curb the neglected zoonosis, no other nonsurgical options are currently available to replace the licensed anti echinococcal drugs albendazole (ABZ) and mebendazole (MBZ). A safer and more effective treatment plan for echinococcosis is therefore urgently needed to compensate for this therapeutic shortfall. …

Uncertainties in diagnosis, treatment and prevention of trichinellosis.

This review summarizes expert opinion and evidence on the diagnosis, treatment and prevention of trichinellosis. Laboratory test results are … tomatic disease, there are theoretical reasons to recommend treatment with albendazole (rather than mebendazole) taken with fatty meals and prednisone. Education of the public is probably not a reliable way to prevent trichinellosis and when feasible should be augmented with mandatory testing of all potentially infected meat.

Treatment options for hepatic cystic echinococcosis.

A number of surgical and non-surgical options exist to treat cystic echinococcosis of the liver. For decades, surgical excision via a conservative or radical approach was the recommended approach for hepatic hydatid cysts. The availability of chemotherapeutic agents with significant activity against Echinococcus granulosis has made it possible to undertake ultrasound- or computed tomography-guided transhepatic percutaneous drainage (termed puncture, aspiration, injection, and re-aspiration (PAIR)) of hydatid cysts. Pre- and post-intervention chemotherapy with albendazole or mebendazole offers …

Induction of protective immunity to Brugia pahangi in jirds by drug-abbreviated infection.

Protective immunity of homologous challenge infection was examined in jirds after drug-abbreviated infection with Brugia pahangi. Mebendazole (MBZ) treatment at the early prepatent (5-7 weeks of post infection) or the late prepatent (7-9 weeks of post infection) period was highly effective in causing almost complete eradication of the primary infection. After challenge infection, the worm burden was significantly reduced 19% (31.1 in average) and 77% (9.5) to that of the controls (38.8 and 41.7), respectively. The magnitude of eosinophil response paralleled the degree of protection. No or only…

Bone hydatid disease.

Bone hydatid disease lacks a typical clinical appearance and image characteristics on x ray or CT are similar to those of tuberculosis, metastases and giant cell tumour or bone cysts. However, MRI does show distinctive diagnostic features of bone hydatid disease, especially in the spine. Until recently, treatment of osseous hydatid disease has been entirely surgical. Effective chemotherapy using benzimidazoles, particularly mebendazole, albendazole and combination treatments, has now been achieved in experimental studies and clinical practice. However, most of these drugs are still in the expe…

A 20-year-old man with eosinophilia and easy bruisability.

Acquired platelet dysfunction with eosinophilia is a rare, benign self-limiting disorder characterised by platelet dysfunction and an association with parasitic infections. A 20-year-old national serviceman who presented with easy bruising and spontaneous epistaxis is reported. Investigations revealed eosinophilia and an abnormal platelet function test similar to that seen in patients with Glanzmann's thrombasthenia infections. He also had evidence of hookworm infestation. Following a course of mebendazole, his symptoms and platelet dysfunction resolved. Hence, this disorder of acquired platel…

The Relationship of Drug Therapy to Aplastic Anemia in Pakistan: A Hospital-Based Case Control Study.

INTRODUCTION: Drug-induced aplastic anemia has long been a menacing outcome of modern pharmacotherapy. The incidence of idiosyncratic, drug- … ncreased risk of aplastic anemia was noted with exposure to thiazide (OR=3.1, 95% C.I, 1.3-7.4) and mebendazole (OR=3.7, 95% C.I, 1.5-9.2). However, risks were not increased with chloramphenicol, trimethoprim/sulfamethoxazole, benzodiazepines, antihistamines, oral contraceptives, and herbal medicine. CONCLUSION: This large-scale case-control study provide as …

School-based deworming program yields small improvement in growth of Zanzibari school children after one year.

Efficacy trials of antihelminthic therapies conducted in Africa have reported improvements in children's growth, but nutritional evaluations of large-scale deworming programs are lacking. We evaluated the first-year effect on growth of a school-based deworming program in Zanzibar, where growth retardation occurs in school children. Children in four primary schools were given thrice-yearly mebendazole (500 mg) and compared with children in four schools that received twice-yearly mebendazole and children in four non-program schools. Evaluation schools were randomly selected and allocated to cont…

Chest pain and eosinophilia in a middle-aged female caused by hookworm infection: a case report.

Hookworm infection, while often asymptomatic, may rarely cause Löffler syndrome. We report the case of a 45-year-old female farmer who presented with a 10-year history of recurrent chest pain. Laboratory tests revealed marked eosinophilia (33.4%) and microcytic anemia. Cardiac and abdominal imaging findings were normal. Stool microscopy confirmed hookworm infection. Treatment of oral mebendazole (400 mg twice daily) for three days resulted in complete resolution of symptoms, normalization of eosinophil count, and clearance of parasites on follow-up. This case highlights hookworm infection as a…

Enterobius vermicularis infestation leading to Meckel's diverticulitis in an adolescent boy: An extremely rare presentation.

Enterobius vermicularis is an intestinal nematode commonly affecting children worldwide. Its transmission is by feco-oral route. Meckel's diverticulitis due to E. vermicularis infestation is an extremely rare presentation. An 11-year-old boy presented with acute abdomen. During surgery inflamed Meckel's diverticulum (M.D) was seen. Histopathology examination of specimen revealed E. vermicularis. Till date, only one case of E. vermicularis infestation of M.D is reported around five decades ago. This histopathological confirmation is extremely important as the required treatment (Mebendazole) of…

Eosinophilic pleocytosis and myelitis related to Toxocara canis infection.

Toxocara canis causes the visceral larva migrans syndrome in which central nervous involvement is rare. We report the case of a 40-year-old woman presenting with a subacute weakness of the right leg and dysaesthesiae in the right Th8-Th10 dermatomas. Spinal magnetic resonance imaging examination showed abnormal hyperintensity within the spinal cord. Cerebrospinal fluid analysis revealed eosinophilic pleocytosis. Antibody titres to Toxocara canis were higher in the cerebrospinal fluid than in the serum. Treatment using mebendazole led to a complete clinical recovery, normalization of cerebrospi…

Intestinal capillariasis: a cause of chronic diarrhea and hypoalbuminemia.

Seventeen cases of intestinal capillariasis in Srinagarind Hospital, Khon Kaen University, Thailand were reviewed. The mean age was 40.41 years with a range from 21 to 69 years. Most cases had intermittent or continuous voluminous watery diarrhea for one month to three years with hypoalbuminemia. Borborygmi, vague abdominal pain, weight loss and pedal edema were significant associated symptoms. Fecal examination was the most useful for diagnosis by finding the Capillaria philippinensis ova in all cases. Mebendazole 400 mg per day for 20 to 28 days is the treatment of choice. Usually, relapse a…

Unintended percutaneous aspiration of pulmonary echinococcal cysts.

Three patients had viable, noninfected, echinococcal cysts of the lung unintentionally diagnosed by percutaneous transthoracic aspiration without deleterious effects. Although aspiration generally should be avoided in hydatid disease because of the possibility of allergic reaction or spread of disease, neither of these potentially adverse effects was seen. Cystobronchial communication was produced iatrogenically in all three cases. The patients were followed for up to 3 years 2 months. Two patients were treated with mebendazole. The pathologist should be alerted when clear colorless fluid is w…

[Clinical evaluation of ciclobendazole (C-C 2481) in the treatment of colonic helminth diseases].

A total of 105 patients with mild helminthiases have been treated with ciclobendazole, a benzimidazole derivative related to mebendazole. The treatment lasted 3 days. A group of 74 patients received 200 mg/day ciclobendazole while another 31 patients were given double this dose. The excretion rates achieved were very high at 84.2 and 83.3% respectively in trichuriasis and 93.3 and 100% respectively in ascariasis. In a relatively small number of cases of ankylostomiasis, the values were, however, considerable lower (38.5 and 20.0% respectively). Since ciclobendazole is well tolerated, there is …

The Hymenolepis diminuta-golden hamster (Mesocricetus auratus) model for the evaluation of gastrointestinal anticestode activity.

A novel laboratory anticestode assay was developed using Hymenolepis diminuta in the hamster. The commercial anticestode compounds, praziquantel, bunamidine, and niclosamide were active against patent infections of Hymenolepis diminuta in golden hamsters (Mesocricetus auratus) when given orally at 3.125, 100, and 200 mg/kg, respectively. The gastrointestinal nematode anthelmintics, cambendazole and mebendazole, were active at 50 mg/kg. Rafoxanide (fasciolicide) was active at 25 mg/kg, the lowest level tested. The coccidiostat, nicarbazin, was active at experimental levels (800 mg/kg and up). T…

Impact of preventive chemotherapy on transmission of soil-transmitted helminth infections in Pemba Island, United Republic of Tanzania, 1994-2021.

Soil-transmitted helminth (STH) infections cause significant morbidity in children and women of reproductive age. The World Health Organizat … hes to better control STH in Pemba would be (i) to assure high coverage in all schools, (ii) to use mebendazole instead of albendazole given its better activity on T. trichiura and (iii) to use a combination of ivermectin and mebendazole to further increase anthelminthic efficacy on T. trichiura.
