Radiological Analysis of the Facial Recess: Impact on Posterior Tympanotomy Difficulty During Pediatric Cochlear Implantation.

OBJECTIVES: We analyzed several radiological features of the facial recess to correlate them with the intraoperative findings to highlight the most reliable predictors of posterior tympanotomy difficulty. STUDY DESIGN: Retrospective observational cohort study. SETTING: Multicenter study at tertiary referral institutions. METHODS: We included 184 pediatric patients who underwent cochlear implantation through the posterior tympanotomy approach. The correlation was attempted between 8 radiological features in the preoperative high-resolution computed tomography scan and intraoperative surgical

Clinical practice guidelines for the diagnosis and management of otitis media with effusion (OME) in children in Japan - 2022 update.

UNLABELLED: This is an update of the 2015 Guidelines developed by the Japan Otological Society and Oto-Rhino-Laryngeal Society of Japan defining otitis media with effusion (OME) in children (younger than 12 years old) and describing the disease rate, diagnosis, and method of examination. Recommended therapies that received consensus from the guideline committee were updated in consideration of current therapies used in Japan and based on available evidence. METHOD: Regarding the treatment of OME in children, we developed Clinical Questions (CQs) and retrieved documents on each theme, includin

[Effect of the diameter of tympanostomy with CO₂ laser on secretory otitis media after radiotherapy].

Objective:To investigate the postoperative effect of different diameters of CO₂ laser tympanostomy in the treatment of secretory otitis media caused by radiotherapy in patients with head and neck tumor, and to find out the best diameter range of tympanostomy according to the curative effect. Methods:The 40 ears after radiotherapy in the otorhinolaryngology department. The average 40 ears were divided into two groups: the diameter of perforation ≤3 mm and the diameter of perforation > 3 mm. Each group was 20 ears. Both groups were compared with tympanum and pure tone threshold after perforation

Procedural Sedation in Minor Procedure Rooms for Pediatric Myringotomy and Tympanostomy: A Quality Improvement Initiative.

OBJECTIVE: Lengthy wait times for elective surgery is a widespread health care system conundrum that may increase patient distress and jeopardize health outcomes. The primary aim of this quality improvement project was to reduce the surgical wait time in patients undergoing tympanostomy tube insertion. METHODS: As of January 2018, our tertiary care institution implemented a novel protocol whereby healthy children may undergo tympanostomy tube insertion in a minor procedure room under ketamine sedation administered by pediatric emergency physicians to address lack of both physical and anesthes

Adenoidectomy for middle ear disease in cleft palate children: a systematic review.

PURPOSE: Cleft palate children have a higher incidence of otitis media with effusion, more frequent recurrent acute otitis media episodes, and worse conductive hearing losses than non-cleft children. Nevertheless, data on adenoidectomy for middle ear disease in this patient group are scarce, since many feared worsening of velopharyngeal insufficiency after the procedure. This review aims at collecting the available evidence on this subject, to frame possible further areas of research and interventions. METHODS: A PRISMA-compliant systematic review was performed. Multiple databases were search

A Systematic Review and Meta-Analysis: Timing of Elective Removal of Tympanostomy Tubes.

OBJECTIVE(S): Tympanostomy tube (TT) placement is the most common surgical procedure in children. Less than 10% of TT do not self-extrude. This study is a systematic review (SR) on elective TT removal in the pediatric population: timing, perforation rates, and role of simultaneous repair. A PICOTS (population, intervention, comparison, outcome, timing, setting) question was formulated: In pediatric patients who have retained TT, what is the preferred time to elective removal of such tubes, and what are the outcomes in terms of perforation rates? Does an intervention at the time of TT removal i

[The short-term and long-term efficacy of balloon dilatation Eustachian tuboplasty and tympanostomy tubes in children patients with recurrent otitis media with effusion].

Objective:To evaluate the short-term and long-term efficacy of balloon dilatation Eustachian tuboplasty(BET) and insertion of tympanostomy tubes in the treatment of recurrent otitis media with effusion(OME) in children, and to provide new ideas for the treatment of OME. Method:We made a retrospective analysis of 29 OME patients who had accepted BET and insertion of tympanostomy tubes in Beijing Children's Hospital. Their case history, the past history of insertion of tympanostomy tubes or adenoidectomy, and the audiometry and Eustachian tube function before surgery and 1 month, 6 months, 9 mon

"On-command" dissolvable tympanostomy tube in the chinchilla model: A proof of concept.

OBJECTIVES: To prove the concept that a dissolvable "on-command" tympanostomy tube placed into the tympanic membrane of a chinchilla can dissolve when a benign solution is applied and result in a well healed tympanic membrane without histologic evidence of injury. STUDY DESIGN: Prospective Randomized Single-Subject Controlled Trial. METHODS: Prototype tympanostomy tubes were fabricated from poly(butyl methacrylate-co-(2-dimethylaminoethyl) methacrylate-co- methyl methacrylate) (PBM). "In vitro" dissolution studies were performed with applications of the benign chemical, hydrogen peroxide (HP

Grommets in HBOT patients: GA vs LA, unanswered questions.

We read with interest the article on grommet procedures for patients undergoing hyperbaric oxygen therapy (HBOT), and have a number of comments. It appears the authors may have missed a number of cases. In a previous paper from The Townsville Hospital Hyperbaric Unit (TTH HMU), Commons et al presented 14 of 106 patients (13%) who required grommets over the period between June 2009 and May 2010. These patients are included in the Lamprell et al data set. Figure 1 shows an apparent spike in their cases in 2010 (n = 13, part of the period covered in the previous paper) when compared to the remain

Surgery for Ménière's disease.

BACKGROUND: This is an update of a Cochrane review first published in The Cochrane Library in Issue 1, 2010.Ménière's disease is characterised by three major symptoms: vertigo, deafness, and tinnitus or aural fullness, all of which are discontinuous and variable in intensity. A number of surgical modalities, of varying levels of invasiveness, have been developed to reduce the symptoms of Ménière's disease, but it is not clear whether or not these are effective. OBJECTIVES: To assess the effectiveness of surgical options for the treatment of Ménière's disease. All surgical interventions used i

Early postoperative complications in children with secretory otitis media after tympanostomy tube insertion in the Military Medical Academy during 2000-2009.

BACKGROUND/AIM: Secretory otitis media (SOM) is a chronic, nonpurulent inflammation of the middle ear, characterized by a long-term presence of liquids of different density in the middle ear for at least three consecutive months, different degrees of hearing loss and the absence of perforation of the eardrum. The aim of this study was to estimate the early postoperative complications after insertion of tympanostomy tube (TT) in children with secretory otitis media (SOM) in an 18-month period after TT insertion. METHODS: This retrospective study included children with SOM (n = 478), aged from

The clinical management of otorrhoea following grommet insertion: a survey of otolaryngologists and general practitioners.

A postal questionnaire of all Consultant Otolaryngologists in the British Isles, and of General Practitioners within the local family practitioner committee area, was designed to compare the current practice in the management of discharge from the ear following grommet insertion. 76% of the Otolaryngologists and 52% of the General Practitioners completed the questionnaire. Between the two groups, statistically significant differences were apparent in the management of this common complication of grommet surgery. A higher proportion of Consultants took a bacteriology swab of the ear as part of
