The present document offers an update on the recommendations for managing patients with cow's milk allergy - a disorder that manifests in the first year of life, with an estimated prevalence of 1.6-3% in this paediatric age group. The main causal allergens are the caseins and proteins in lactoserum (beta-lactoglobulin, alpha-lactoalbumin), and the clinical manifestations are highly variable in terms of their presentation and severity. Most allergic reactions affect the skin, followed by the gastrointestinal and respiratory systems, and severe anaphylaxis may occur. The diagnosis of cow's milk …

This literature review and case study answers the question: 'Do the late effects of childhood cranial radiation therapy include impacts on breastfeeding?' PubMed was searched for papers using the terms lactation and cranial radiotherapy or childhood cranial radiotherapy. The case study was written from one author's experience of helping a mother with a history of childhood cranial radiation therapy. The few available studies report a high rate of lactation failure in women who were treated with cranial radiation therapy for childhood cancer, but the exceptions indicate that lactation failure i

Cow's milk protein allergy is an adverse immune reaction to one or more of the constituent proteins of milk obtained from any animal, most commonly alpha s 1-casein cow's milk. In many cases, the allergy is genetic in origin. The infants may experience symptoms within minutes after feeding (rapid onset) or commonly after 7-10 days of consuming the cow's milk (slower onset). Many children with cow's milk protein allergy develop symptoms in at least two of the following organ systems: gastrointestinal, skin and respiratory tract. Acrodermatitis Enteropathica (primary or secondary zinc deficiency…

INTRODUCTION: We aimed to describe current nursing practice and clarify the safest and most effective dose of milk and molasses enemas used to relieve constipation in pediatric patients presenting to a suburban pediatric emergency department.
METHODS: We surveyed emergency nurses about current practice in administration of milk and molasses enemas. In addition, we identified consecutive patients aged 2 to 17 years with a discharge diagnosis of constipation or abdominal pain between 2009 and 2012. Stable patients were included from the emergency department, in the absence of chronic medical co…

Breast milk can occasionally transmit serious viral and bacterial infections to preterm infants. We present three cases of late-onset neonatal sepsis, including one that resulted in death, occurring in preterm infants. The likely source of the microorganisms in all three cases was expressed breast milk.

Breast milk beta-glucuronidase was thought to be one of the etiological factors in the pathogenesis of late-onset breast-milk jaundice, but results of these studies are conflicting. In this study breast milk beta-glucuronidase levels were determined in groups with physiologic jaundice, early breast-feeding jaundice and late breast-milk jaundice. No difference in beta-glucuronidase levels of these three groups was found in samples taken on the 4th and 15th days of life. beta-glucuronidase activity in breast milk declined from the 4th to 15th day in all groups. These results imply that factors o…

The relation between hypercalcemia and pancreatitis, first described in patients with hyperparathyroidism, is controversial. Other causes of hypercalcemia also have been associated with pancreatitis. In this report, the authors describe a patient with pancreatitis and the milk-alkali syndrome who had the classic triad of hypercalcemia, alkalosis, and renal insufficiency. The authors also review the literature for all the reported cases of pancreatitis associated with hypercalcemia.

We describe the case of a young man with IGE-mediated hypersensitivity to milk, casein, and lactoglobulin, who went into respiratory crisis every time he milked his sheep.

Demonstration of Staphylococcal Thermonuclease (TNase) from Powder Milk. Some authors have reported that the number of Staphylococcus aureus needed to produce a food-poisoning is 10(6) CFU per gram of food, however, other authors have reported foods without microorganisms but these have produced food-poisoning. Because the methods for staphylococcal enterotoxins demonstration in foods are laborious and expensive procedures, in this paper we tried to demonstrate the thermonuclease (TNase) presence in foods directly, as a helper test for screening foods suspected to be contaminated with this mic…

The concentration of EGF immunoreactivity in rat whey increases from 0.3 pmol/ml at lactation day 1 to 2.0 pmol/ml at lactation day 19. The concentration of EGF is not influenced when the rats undergo sialoadenectomy prior to mating. On S-200 gel chromatography, almost all EGF-reactivity in rat whey elutes as a broad peak corresponding to a Stokes radius of 4.0 nm (an approximate molecular weight of 80 kDa). Almost no 6 kDa EGF is present. Judged by gel filtration of whey pre-incubated with 125I-EGF (6 kDa), no binding protein for EGF is present in rat whey. When rat milk is incubated overnigh…

Reactions induced by neurotoxic effects with lindane (gamma-hexachlorcyclohexane) after treatment of infants have been reported by different authors. This has led to controversial discussions as to the indication for therapy with lindane in early infancy. To our knowledge there are to date no published reports on the concentration of lindane in mother's milk after local scabies therapy. In a mother treated with lindane for scabies we found elevated levels of lindane in the mother's milk. Compared to average levels of lindane in mother's milk (0.03 ppm, milk-fat) there was a 30-fold increase af…

Cheese made from sheep milk was implicated in food-poisoning incidents in December 1984 and January 1985. Bacteriological examination of batches of cheese failed to reveal a viable pathogen but enterotoxin A produced by Staphylococcus aureus was present. This was the first time that enterotoxin was detected in a food produced in the UK which was associated with poisoning and from which viable Staph. aureus could not be isolated. Subsequent detailed examination of milk, yoghurt and cheese from the same producer revealed that contamination with Staph. aureus was associated with post-infection ca…

The medical literature contains numerous references to the use of oral cromolyn sodium in gastrointestinal disturbances (abdominal pain, diarrhea, ulcerative colitis) and dermatologic illnesses (urticaria, infantile eczema, purpura). But there are few, if any, reports of anaphylactic reactions to foods which have been successfully controlled using oral cromolyn sodium. This paper reports the results of treatment with oral cromolyn in a 7-year-old child who had acute, severe anaphylaxis after ingestion of milk or foods containing milk. After 3 months of oral cromolyn therapy, the patient was ab…

The histocompatibility antigens of one hundred patients with a severe form of cow's milk allergy were studied. HLA-A and B locus antigens were identified in all patients, C locus antigens in 62 patients and DR locus antigens in 41 patients. The A, B and C locus antigen frequencies were compared with those of healthy blood donors, and DR locus antigen frequencies with those of healthy unrelated volunteers and cadaver kidney donors. The series included six patients with concomitant coeliac disease, who were treated as a separate group. No statistically significant differences between the patient…
