Psoriatic arthritis (PsA) is a heterogeneous, immune-mediated inflammatory disease characterised by multidomain clinical involvement, including peripheral arthritis, enthesitis, axial disease, dactylitis, and skin and nail manifestations, alongside a substantial burden of comorbidity. Over the recent decades, therapeutic options for PsA have expanded, with the introduction of multiple biologic and targeted compounds targeting tumour necrosis factor, the IL-23/IL-17 axis, and Janus kinase signalling. These advances have improved outcomes for many patients; however, incomplete responses, domain-

Meningioma is the most common primary brain tumor. Surgical resection and, in some cases, adjuvant radiotherapy remain the cornerstone of treatment, offering an exceptionally high cure rate. However, treatment can be challenging, particularly in cases of high-grade tumors, symptomatic tumors located in eloquent brain regions, and in patients who are medically inoperable. Recent molecular profiling has redefined meningioma classification, identifying distinct consensus groups-Immunogenic (MG1), NF2-wildtype (MG2), Hypermetabolic (MG3), and Proliferative (MG4)-with significant prognostic and the

INTRODUCTION: Biological and targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) have transformed rheumatoid arthritis (RA) management; however, individualizing treatment, balancing efficacy and safety remains challenging.
AREAS COVERED: This narrative review summarized evidence guiding b/tsDMARD selection based on a literature search conducted in PubMed. First, we outlined efficacy-driven factors, including concomitant csDMARD use, serological markers, and C-reactive protein/interleukin-6-related pathways, that may support selecting particular mechanisms of action. Second,

C-C motif chemokine ligand 2 (CCL2) interacts with cytokines, adipokines, miRNAs, and multiple synovial cell populations. Experimental studies indicate that these interactions can form a CCL2-associated inflammatory amplification network across cell types. In cellular and animal models, increased CCL2 is associated with monocyte recruitment, synovial fibroblast activation, osteoclast-related bone remodelling, and vascular responses. Therapeutic strategies targeting the CCL2-centered inflammatory network include antagonists of the CCL2/CCR2 axis, natural products, synthetic compounds, conventio

The interleukin-36 (IL-36) cytokine family is an important regulator of inflammatory responses at epithelial barrier surfaces, with particularly prominent roles in skin immunity and inflammatory disease. As members of the IL-1 superfamily, IL-36 cytokines include three agonists (IL-36α, IL-36β, and IL-36γ) and two antagonists (IL-36Ra and IL-38), which signal through a shared heterodimeric receptor complex to activate MyD88-dependent pathways, including NF-κB and MAPK, and induce pro-inflammatory cytokines, chemokines, and antimicrobial peptides. A defining feature of IL-36 biology is its depe

With multiple molecular targeted therapies available for patients with cancer that correspond to a specific genetic alteration, the selection of the best treatment is essential to ensure therapeutic efficacy. Molecular tumor boards (MTBs) play a key role in this process to deliver personalized medicine to patients with cancer in a multidisciplinary manner. Historically, personalized medicine has been offered to patients with advanced cancer, but the incorporation of molecular targeted therapies and immunotherapy into the perioperative setting requires clinicians to understand the role of the M

The role of immunotherapy in the care of surgical oncology patients promises to expand as investigators and clinicians evaluate new targets and approaches. Currently active clinical trials evaluate new immune checkpoints, including lymphocyte activation gene 3, T cell immunoreceptor with Ig and ITIM domains, and killer Ig-like receptor 2DL1/2L3. Vaccines delivered through mRNA have demonstrated exciting results in early clinical trials and hold promise for expanded application. Investigational approaches include dendritic cell vaccines, peptide vaccines, cytokines therapies, and cellular thera

Gastric cancer (GC) remains a deadly disease because of late detection and limited treatment options at advanced stages. Treatment of patients with metastatic disease is based on chemotherapy, complemented by antibodies targeting HER2, VEGFR2, and more recently PD-1 or claudin 18.2. Further targets, such as FGFR2b, as well as novel drug classes including antibody-drug conjugates (ADCs) and bispecific antibodies, are promising developments in GC treatment. Despite the failure of several targeted agents, the landscape of GC therapy is evolving rapidly, facilitated by umbrella or platform precisi

BACKGROUND: Gastric cancer (GC) is among the most common malignancies worldwide and represents a major cause of cancer-related mortality. The incidence of GC varies significantly across regions due to genetic predisposition, environmental exposure, dietary habits, and infectious agents, particularly Helicobacter pylori. Despite its high prevalence, GC is often diagnosed at an advanced stage because of nonspecific or absent early clinical symptoms.
OBJECTIVE: The objective of this review is to provide a comprehensive overview of the epidemiology, etiology, risk factors, molecular mechanisms, d

Uterine mesenchymal tumors (UMTs) are the second most common type of tumors within the uterus corpus after endometrial carcinomas. Among the UMTs, smooth muscle neoplasms are the most common subtype, followed by endometrial stromal sarcoma (ESS). ESSs are uncommon malignancies characterized by molecular heterogeneity and an aggressive behavior. Their management poses significant challenges, particularly for high-grade subtypes. Surgery is the primary intervention for localized disease, while the role of adjuvant therapies, including radiation and chemotherapy, must be better investigated. Horm

UNLABELLED: Biliary tract cancers (BTC) are aggressive malignancies encompassing intrahepatic and extrahepatic cholangiocarcinoma, gallbladder carcinoma, and ampullary carcinoma. Here, we report integrative analysis of 63 BTC cell lines via multi-omics and genome-scale CRISPR screens. We identify widespread EGFR dependency in BTC, alongside dependencies selective to anatomic subtypes. Additionally, we delineate strategies to overcome therapeutic resistance, with combined EGFR inhibition potentiating targeting of KRAS-mutant and FGFR2 fusion-driven models and SHP2 inhibition effective in the la

Two-thirds of patients with advanced thyroid cancer are refractory to radioactive iodine (RAI). Inhibition of genetic drivers of thyroid cancer in the intracellular mitogen-activated protein kinase pathway, like BRAF, RAS, RET, and NTRK, results in increased expression of thyroid differentiation genes and the sodium-iodine symporter. Targeted therapies in combination with RAI result in structural decrease of metastatic lesions in 33% to 63% of patients that successfully take up RAI after redifferentiation. The true efficacy of targeted therapies with RAI remains to be proven in clinical trials

The transient receptor potential canonical (TRPC) channel family includes TRPC6, a nonselective receptor-activated cation channel. Its activation result in Ca2+, Na+ along with other cationic ion influx and the phosphorylation of tyrosine, serine and phosphoinositides regulates TRPC6. The channel is widely distributed and plays physiological role in different body parts such as kidney, lungs, blood vessels, heart, brain, intrinsic cardiac ganglia and eye. It has been determined that TRPC6 is a crucial part of the kidney podocytes. Mutation in TRPC6 gene results in focal segmental glomeruloscle

Pediatric adrenocortical carcinoma (pACC) is an exceptionally rare and aggressive malignancy, accounting for only 0.2-0.3% of childhood cancers. Characterized by significant endocrine activity and often associated with genetic syndromes such as Li-Fraumeni syndrome, pACC exhibits distinct clinical and molecular profiles compared to adult adrenocortical carcinoma (ACC). Current treatment approaches, largely adapted from adult protocols, center on surgery and chemotherapy, including mitotane. However, the lack of pediatric-specific data and major clinical trials underscores a pressing need for t

Current therapies for Graves' disease (GD) primarily aim to manage hyperthyroidism through synthetic antithyroid drugs, radioiodine, or surgery. However, these approaches are often limited by their incomplete efficacy and the risk of inducing hypothyroidism. The latest advances in understanding the autoimmune mechanisms driving GD have paved the way for novel therapies targeting the thyrotropin receptor (TSH-R) or immune pathways. Overall, key targets include cluster of differentiation 20 (CD20), cluster of differentiation 40 (CD40), protein tyrosine phosphatase non-receptor type 22 (PTPN22),

Hidradenitis Suppurativa (HS) is a chronic autoinflammatory skin disease of the terminal hair follicle, whose pathogenesis is complex and multifactorial. Alongside a predisposing genetic background, environmental/epigenetic factors, alterations in the skin microbiome, and dysregulation of both innate and adaptive immune responses contribute to the persistent inflammatory network. Despite advancing knowledge, the exact molecular and cellular mechanisms underlying HS have yet to be untangled. Moreover, the lack of experimental disease models that closely mimic disease phenotypes or pathophysiolo

Similar to other malignancies, immune dysregulation is a key feature of acute myeloid leukemia (AML), manifesting as suppressed anti-leukemia immune cells, immune evasion by leukemia blasts, and disease progression. Various immunosuppressive factors within the AML microenvironment contribute to the weakening of host immune responses and the efficacy of cellular immunotherapy. To address these challenges, strategies targeting immunosuppressive elements within the AML microenvironment aim to bolster host or adoptive immune effector cells, ultimately enhancing leukemia treatment. Additionally, th

Psoriasis is a multifactorial disease that affects 0.84% of the global population and it can be associated with disabling comorbidities. As patients present with thick scaly lesions, psoriasis was long believed to be a disorder of keratinocytes. Psoriasis is now understood to be the outcome of the interaction between immunological and environmental factors in individuals with genetic predisposition. While it was initially thought to be solely mediated by cytokines of type-1 immunity, namely interferon-γ, interleukin-2, and interleukin-12 because it responds very well to cyclosporine, a reversi
