Moral injury in orthopaedic surgery: Distinguishing burnout from system-level ethical conflict.

Orthopaedic surgeons experience some of the highest rates of burnout, depression, and suicidal ideation among all surgical specialties, yet existing wellness frameworks may not fully account for the specific nature of their distress. When surgeons are prevented from providing timely, indicated care by system constraints, pressured to participate in clinical decisions that conflict with their professional judgment, or repeatedly exposed to preventable disability and suffering, the resulting harm is not simply exhaustion. It is moral injury-a deeper disruption of professional identity, trust, an

Beyond principlist autonomy: The moral meaning of feeding and parenteral nutrition in palliative cancer care.

Parenteral nutrition (PN) in palliative oncology presents profound ethical challenges where the desire to nourish intersects with dependency, vulnerability, and the limits of medicine. For patients with incurable malignancy who cannot maintain nutrition enterally, PN may extend life for months yet impose significant physical, emotional, and moral burdens on patients and families. Prevailing principlist frameworks privilege a thin, procedural conception of autonomy that fails to capture the interdependence inherent in these situations. This essay argues that relational autonomy, enriched by vir

Caring for patients with multimorbidity: moral distress and life satisfaction among doctors and nurses in Portugal.

BACKGROUND: The prevalence of multimorbidity (MM), defined as the co-occurrence of two or more chronic conditions in an individual, presents complex challenges for healthcare providers (HCPs). In Portugal, nearly half of the general population experiences MM, intensifying demands on HCPs. Doctors and nurses often encounter fragmented care pathways, inadequate guidelines, and frequent ethical dilemmas, which can lead to moral distress (MD). MD arises when HCPs are unable to act in accordance with their ethical beliefs due to institutional barriers, and it has consequences for well-being and emp

Abortion in conflict zones: moral responsibilities of humanitarian agencies in providing safe abortion care.

Unsafe abortion remains a leading cause of maternal mortality globally, disproportionately affecting women in conflict zones where health systems are fractured, legal protections are weakened, and access to reproductive care is severely limited. Armed conflict increases the incidence of unwanted pregnancy through sexual violence, displacement, and disruption of contraceptive services, while also heightening barriers to timely, comprehensive abortion care. Despite growing international recognition of abortion as a life-saving intervention, humanitarian agencies often face legal, cultural, and o

Knowing or not knowing: the practical and moral complexity of diagnostic-seeking pathways in situations of medical uncertainty.

In this paper, we discuss the practical and moral complexities of diagnostic-seeking pathways for three uncertain medical conditions: fibromyalgia, myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), and long COVID. We analyse qualitative data collected in the UK between 2023 and 2025, comprising in-depth interviews with medical professionals, researchers, and patients, as well as documentary sources. Drawing on the anthropology and sociology of diagnosis, this paper discusses how chronic illness can be marked by challenging and unstable pathways, influenced by the organisation of the

'Outbreak distress': Characterising moral distress among international healthcare workers responding to mpox.

In May 2022, a global outbreak of mpox (formerly known as monkeypox) was declared an international public health emergency. During this time, healthcare workers scrambled to respond to the outbreak amidst a lack of resources, knowledge gaps and pressures related to COVID-19. This period posed a risk of heightened moral distress, defined as distress arising from a situation in which a healthcare worker knows the right thing to do but is externally constrained from doing so. An international survey of healthcare workers was developed to understand their experiences of responding to mpox, includi

Temporal perception, attentional bias, and moral reasoning in multiple sclerosis: a moderated mediation analysis.

OBJECTIVE: This study examines the relationships between future time perspective, attentional biases, and moral judgment in people with multiple sclerosis (pwMS) compared to healthy controls, testing a preregistered moderated mediation model derived from Socioemotional Selectivity Theory. METHOD: Ninety participants (45 relapsing-remitting pwMS, EDSS ≤4; 45 matched controls) completed neuropsychological assessments, a moral judgment task based on 20 validated dilemmas, two measures of future time perspective (the Future Time Perspective Scale and an implicit word-completion task), and a dot-p

"Not all bathrooms are created Equal": Moral experiences of maneuvering in inaccessible infrastructure with physical disability.

How do people with physical disabilities characterize their encounters with inaccessible infrastructure? I draw on interviews and focus groups with older adults with spinal cord injuries from the Midwestern United States to argue that participants experienced inaccessible space as morally harmful, damaging their sense of worth and dignity. They developed strategic bodily "maneuvers" to squeeze through narrow corridors, scale ledges, navigate the back of rooms and buildings, and avoid filth and garbage-resulting in situations of exclusion and marginalized inclusion. Maneuvering established inac

Moral sensitivity and professional quality of life in EMS personnel: a cross-sectional multicenter study.

INTRODUCTION: Moral sensitivity plays a crucial role in clinical decision-making and patient interactions among Emergency Medical Services (EMS) personnel. Although moral sensitivity and professional quality of life have been studied in hospital-based healthcare providers, little evidence exists regarding EMS personnel, who face unique challenges such as heavy workloads, workplace violence, and frequent ethical dilemmas in unpredictable environments. These gaps highlight the necessity of investigating the relationship between moral sensitivity and professional quality of life in this context.

A Rescoping Investigation of Burnout vs Moral Injury in Urology and the Broader Health Care Field.

INTRODUCTION: Urology burnout has been extensively studied, but despite numerous interventions its prevalence remains high. Recent insights suggest that what is often labeled as burnout may actually be moral injury, the challenge of knowing the care patients need but being unable to provide it due to factors beyond the health care provider's control. This study seeks to reclassify factors traditionally linked to burnout in urology as manifestations of moral injury and proposes holistic strategies to address these systemic issues. METHODS: A literature review was performed to identify the most

Reducing financial misreporting behaviour with noninvasive brain stimulation: the moderating effect of moral judgement.

Building upon the distinct functions of the right dorsolateral prefrontal cortex (rDLPFC) and the right temporoparietal junction (rTPJ), this study investigates how moral judgement moderates the influence of these brain regions on financial misreporting-an effect that remains largely unknown. Employing transcranial direct current stimulation (tDCS), this study temporarily altered activity in these areas to investigate their influence on financial misreporting during a profit reporting task. Study 1 recruited university students, while Study 2 focused on finance professionals. The results showe

Augmented Reality 3MDR Therapy for the Treatment of PTSD and Comorbid Moral Injury: A Case Study.

Rates of veteran suicide, post-traumatic stress, and moral injury remain alarmingly high and continue to rise. While virtual reality and augmented reality technologies have shown promise in improving post- traumatic stress disorder (PTSD) severity, research on these innovative methods is still limited. Multi- modal memory desensitization and reconsolidation (3MDR), a cutting-edge therapy that combines Virtual Reality Exposure Therapy or Augmented Reality Exposure Therapy with a "walk and talk" therapy, has demonstrated significant potential in enhancing participant engagement and substantially

Cognitive loopholes of crime: Mapping the Codevelopment of moral disengagement within perceptions of risks and rewards.

Prior research has examined individuals' perceptions of punishments (PP) and rewards (PR) for crime, as well as their use of moral disengagement (MD), to understand why adolescents and young adults commit crimes. However, the joint development of these cognitions as a broader risk-perception mechanism remains understudied. This paper explores the independent and relational development of these processes in justice-involved youth. Data from 1,170 male participants (42.1% Black, 34.0% Hispanic, 19.2% White, 4.6% Other) in the Pathways to Desistance study were analyzed using a three-variable auto

Moral distress among family caregivers of people with cancer in palliative care: A qualitative study.

The study aimed to understand the triggering events of moral distress according to the family caregivers of people with cancer in palliative care. This is a longitudinal qualitative approach study, using the concept of moral distress as an interpretative reference. Ten family caregivers participated in in-depth interviews. The data were analyzed according to the inductive thematic analysis technique, and two themes were identified. In the first theme, entitled "The repercussions following diagnosis," participants reported the uncertainties they experienced following their family member's cance

Wrongness and Blame Judgments and Their Dynamics: Toward a Three-Input Processing Model of Moral Judgment.

In moral psychology, several approaches to moral judgments coexist, with sometimes contradictory results for different types of judgments. In the current research, we combine two views of moral judgment into a novel three-input processing model. As a first empirical test of this model, the present research investigates the influence of these three classic inputs (i.e., intent, outcome, and causality) on wrongness and blame judgments as well as their underlying dynamics. This preregistered experiment (N = 145) re-uses an adapted mouse-tracking paradigm to analyze these influences over time. Res
