Nerve repair and nerve transfer are the optimal approaches to restore function to denervated muscle. When the distal-most portion of the nerve entering the muscle is compromised, these are not possible and direct muscle neurotization (DMN) is considered. A patient with simultaneous posterior and anterior interosseous nerve syndromes is presented. During induction of anesthesia, the paralyzed muscles exhibited denervation sensitivity to succinylcholine. Following exploration and repair of a posterior interosseous nerve defect, full function returned in the dorsal muscles as well as the flexor pollicis longus and pronator quadratus muscles.

To study the motor recovery after increasingly late nerve suture, we severed 75 rabbit peroneal nerves and reanastomosed them by microsurgical epineural technique after a delay of up to 12 months. Pontine hemorrhage is a life-threatening emergency which may result in high mortality and devastating disability. Trigeminal denervation is usually caused by neoplasms and trauma but rarely occurs after pontine hemorrhage. Herein, we present a patient with delayed trigeminal motor denervation who presents with progressive atrophy of the right temporalis and masseter muscles 1 year after pontine hemorrhage. Details of the pathogenesis and correlation between clinical, electrophysiological, and radiological findings are addressed.

A 65-year-old woman with multiple chronic cranial neuropathies had spinal accessory innervated muscles that were virtually invisible to electromyography. Ultrasound imaging revealed the extensive atrophy and increased echogenicity that corresponded to the thinness of the muscles and their loss of insertional activity. In patients with severe atrophy of trapezius or sternocleidomastoid muscles, ultrasound may help in identify chronically denervated muscle.
