HLA-A*11:01 and HLA-C*03 Binding to FKBP1A Influences the Efficacy of Tacrolimus in Myasthenia Gravis Subgroups.

Tacrolimus (TAC) is recommended for the patients with drug tolerance or respond poorly to other conventional immunosuppressants in myasthenia gravis (MG), but its effectiveness is influenced by dose and genetic polymorphism. Studies suggest that HLA polymorphisms are significantly associated with susceptibility to MG, but the relationship between HLA and TAC efficacy in MG has not been explored. In this paper, we investigated the correlation between TAC therapeutic efficacy and HLA in different MG subgroups. HLA-C*03 was associated with a better response to TAC in patients with early-onset MG,…

Targeting Autoimmunity in Myasthenia Gravis: From Conventional to Novel Therapeutic Approaches.

Myasthenia Gravis is a chronic autoimmune neuromuscular disorder characterized by fluctuating skeletal muscle weakness, most commonly involving ocular, bulbar, respiratory, and limb muscles. This weakness stems from autoantibodies, predominantly immunoglobulin G, against neuromuscular junction components such as acetylcholine receptors or muscle-specific kinase, which ultimately reduces synaptic transmission. MG causes a severe functional impact on normal patients, which often contributes to the reduced quality of life and life-threatening myasthenic crises. This article provides an extensive …

Germinal centers are associated with poor prognosis in patients with thymoma-associated myasthenia gravis.

BACKGROUND: Germinal centers (GCs) can be observed in the thymic tissues of individuals with thymoma-associated myasthenia gravis (TAMG). This study aimed to investigate the association between the presence of GCs in the thymus and the clinical outcomes of patients with TAMG following thymoma resection. METHODS: A retrospective review was conducted on patients diagnosed with TAMG who underwent surgical removal of the thymoma. Upon reviewing thymic tissue slides to determine the presence of GCs, patients were divided into GC-positive and GC-negative groups. The association between the presence…

Autoreactive T cells and thymic atrophy pathway in thymic hyperplasia patients with myasthenia gravis.

BACKGROUND: T cells play a vital role in antigen presentation, immune tolerance imbalance, and secretion of pathogenic cytokines in the progression of Myasthenia Gravis (MG). Previous studies have founded that patients with thymic hyperplasia accompanied by MG exhibit autoreactive T cells in thymus that can recognize self-antigens at the neuromuscular junction (NMJ), leading to the destruction of acetylcholine receptors (AChR) and the appearance of muscle weakness. Furthermore, Thymic hyperplasia is associated with T-cell-related autoimmune diseases, and increased expression of WNT4 and FOXN1 …

Therapeutic approach to myasthenia gravis treatment in the era of new biologics - review of existing and upcoming therapies.

Myasthenia gravis (MG) is a heterogenous autoimmune disease affecting the neuromuscular junction, which in many patients leads to devastating symptoms significantly influencing patients' lives or causing life-threatening episodes. The incidence and prevalence of myasthenia are growing worldwide. By definition, MG is characterized by muscle weakness exacerbated with activity and improving with rest. A total of 15-20% of patients during the course of the disease develop myasthenic crisis (MC), defined as respiratory insufficiency requiring intubation, with a high mortality rate. Approximately 30…

[Management of Pregnancy Due to Concern for Preeclampsia in a Woman with Myasthenia Gravis, Chronic Hypertension, and Epilepsy - A Case Report].

Myasthenia gravis (MG) is an autoimmune disease impairing neuromuscular ‎transmission due to antibodies against acetylcholine receptors. Preeclampsia is a ‎pregnancy-specific hypertensive disorder that can lead to seizures and cerebral ‎complications, typically prevented with IV magnesium sulfate.‎ In MG patients, magnesium is contraindicated as it inhibits acetylcholine release, ‎worsening muscle weakness and risking respiratory failure.‎ We present a pregnant MG patient with additional preeclampsia risk factors: nulliparity, ‎age, epilepsy, gestational diabetes and chronic hypertension. Due …

Case report: Efgartigimod combined with intravenous methylprednisolone in a case of co-occurrence of unexplained myasthenia gravis, inflammatory myopathy, and fulminant myocarditis.

The co-occurrence of myasthenia gravis (MG), inflammatory myopathy (IM), and fulminant myocarditis (FM) has rarely been reported. The few previous articles were mostly case reports or small sample data analysis, and the etiology was mostly attributed to thymoma and/or the application of immune checkpoint inhibitors (ICIs). We report a 74-year-old woman with an acute onset of general weakness, chest tightness, shortness of breath, dysarthria, masticatory weakness and dysphagia, without myalgia or rash. Acetylcholine receptor antibody was positive. Serum creatine kinase, transaminase, and myoglo…

Enhanced antibody detection via live cell-based assay among RIA negative myasthenia gravis patients: toward improved diagnosis and clinical correlates.

BACKGROUND: Radioimmunoprecipitation assay (RIA) is the gold standard for antibody detection in myasthenia gravis (MG), while fixed cell-based assays (CBAs) are widely used as second-line or first-line tests. Live CBA represents an alternative approach and its combined use with fixed CBA may improve diagnostic sensitivity. Our aim was to evaluate the diagnostic yield of fixed and live CBA for AChR and MuSK antibodies in a large cohort of RIA seronegative MG patients and to investigate clinical associations and outcomes. METHODS: Sera from 124 AChR- and MuSK-RIA seronegative MG patients and 72…
