Tuberculosis (TB) remains a global health problem. There is an intense effort to identify correlates of protective immunity and to design new TB vaccines. CD8 T cells are thought to play a significant role in controlling Mycobacterium tuberculosis infection. Relatively little has been published about the antigens and epitopes targeted by mycobacteria-specific CD8 T cells. Here we present an update of our 1999 overview of human CD8 T cell epitopes in mycobacterial antigens and discuss related issues relevant to TB diagnosis and vaccine development.

BACKGROUND: Results of medical therapy for Mycobacterium xenopi pulmonary infection remain unreliable. Pulmonary resection may be beneficial to patients whose disease is localized and who can tolerate a resectional operation.
METHODS: Eighteen patients underwent pulmonary resection between 1991 and 2000: 14 men and 4 women, with a mean age of 50 +/- 12 years (range 27 to 68 years). Indications for operation were either therapeutic (n = 9) or diagnostic (n = 9). Four patients received antimycobacterial chemotherapy before their operation and 2 patients were HIV positive.

CONTEXT: Washington State has a relatively low incidence rate of tuberculosis (TB) infection. However, from May to September 1997, 3 cases of pulmonary TB were reported among medical waste treatment workers at 1 facility in Washington. PURPOSE: Our goal was to describe the CT appearances of Mycobacterium xenopi pulmonary infection.
METHOD: A retrospective study period of 75 months was used. Eight immunocompetent patients fulfilled the American Thoracic Society criteria for M. xenopi infection and had CT scans of the thorax. CT scans were reviewed by two observers, and decisions were reached by consensus.

Most infections of soft tissue injury sites are caused by Staphylococcus or Streptococcus and respond to beta-lactam antibiotics. Occasionally, a patient does not respond to routine antibiotics, and other possibilities must be considered. A detailed history of the event that caused the soft tissue injury can be important in the diagnosis. Two cases of Mycobacterium marinum soft tissue infection (ie, fish tank granuloma) are presented.

The immunoreactivity of four recombinant Mycobacterium intracellulare beta-galactosidase fusion proteins, which correspond to 22, 40, 43 and 85 kDa M. intracellulare antigens, was assessed. Lymphoproliferative assays demonstrated that Escherichia coli lysates containing each of the fusion proteins stimulated T cells in vitro. Granulomatous infections of the genitourinary tract are rare, especially those caused by nontuberculous mycobacteria. A case of prostatitis due to Mycobacterium avium complex in an immunocompetent man is reported. The patient had sterile pyuria, and a Mantoux skin test, using 5 tuberculin units, was positive (induration, 10 mm in diameter). Pathologic examination of the prostate revealed necrotizing granulomata with acid-fast bacilli, and repeated performance of urine cultures before initiating therapy yielded M. avium complex.

Cell walls of Mycobacterium leprae, prepared by differential solvent extraction, were shown to contain arabinogalactan, mycolates, and peptidoglycan. In addition, amino acid analysis revealed the unexpected presence of large amounts of protein that retained potent immunological reactivity. Iron chelated to the exochelins from Mycobacterium neoaurum was taken up by a suspension of M. leprae, prepared from the liver of an infected armadillo, over 15 hr. No uptake occurred when the iron was chelated with exochelins from M. bovis BCG or M. smegmatis or to a single exochelin from M. vaccae. Uptake appeared to be by facilitated diffusion since it was not inhibited by either HgCl2, NaN3, or 2,4-dinitrophenol. This was similar to the mode of uptake of ferriexochelin into M. neoaurum itself.

We report the results of the study by transmission electron microscopy of normal Mycobacterium leprae in the tissues of experimentally infected armadillos. Several fixation procedures were used and compared to those previously employed in the study of M. leprae in lepromatous leprosy patients. The results show that the ultrastructure of M. leprae is identical in both hosts. The demonstration of a symmetric membrane profile in M. leprae in armadillos confirms our previous results. A 53-year-old woman had multiple ulcerative skin lesions caused by Mycobacterium intracellulare. The original lesions had appeared on her face and upper part of her chest at least 11 years earlier and had progressed slowly to involve large areas of her head and trunk. This unusual skin infection demonstrates chronicity and destructiveness.

Ten cases of skin infection due to Mycobacterium marinum are reported from Israel. Most of the infections were contracted in natural bathing pools at Ein Feshka on the shores of the Dead Sea, south of Jericho. The lesions closely resembled those of cutaneous leishmaniasis prevalent in this region. The condition does not seem to have been detected previously in the Middle East.
