Rhabdomyolysis is a common and potentially lethal clinical syndrome that results from acute muscle fiber necrosis with leakage of muscle constituents into blood. Myoglobinuria is the most significant consequence, leading to acute renal failure (ARF) in 15%-33% of patients with rhabdomyolysis. Rhabdomyolysis occurs from inherited diseases, toxins, muscle compression or overexertion, or inflammatory processes, among other disorders. In some cases, no cause is found. An 82-year-old woman with pulmonary aspergillosis developed dark-colored urine, markedly elevated creatine kinase (3,125 U/L) and myoglobin (3884.4 ng/mL), and delayed myocardial injury (high-sensitivity troponin I: 26301.6 pg/mL) on day 2 after intravenous micafungin (200 mg/day) initiation. Symptoms and biomarkers resolved upon discontinuation, suggesting micafungin-induced rhabdomyolysis and myocardial injury. This first-reported case highlights the need for vigilance in elderly, multimorbid patients receiving polypharmacy, expanding understanding of micafungin's safety profile.

Myoglobinuric acute kidney injury (AKI) occurs as a result of rhabdomyolysis. Here, we present a protocol based on mouse primary renal tubular epithelial organoids, or "tubuloids", to model myoglobinuric AKI in vitro. We describe how to establish and maintain the culture of mouse tubuloids and how to trigger myoglobin-induced injury. Moreover, we provide instructions on how to quantify tubuloid damage. The erythrocyte membrane proteins and glycoproteins and urinary polypeptides have been examined in a patient exhibiting intermittent pigmenturia associated with conga drumming. Significant excretion of haemoglobin, albumin and probably erythrocyte carbonic anhydrase but not myoglobin occurred during the acute phase of the conga drumming-induced pigmenturia. This usually ceased within 24-48 h. We found no evidence of aberrant erythrocyte membrane components on electrophoresis with either protein staining or a range of 125I-labelled lectins used for detection.

A 50-year-old woman with schizophrenia overdosed on risperidone, lormetazepam, and lorazepam and subsequently developed rhabdomyolysis, with elevated creatine kinase (CK) and myoglobin levels. Intravenous treatment stabilized her condition. Risperidone was restarted at a regular dose, and rhabdomyolysis did not recur. She was discharged after 29 days, and she continued outpatient care without further complications. There have been no reports of rhabdomyolysis caused by overdosing on risperidone alone. This case is important for understanding the risk profile of this drug.

Acute rhabdomyolysis with myoglobinuric renal failure occurred in a 66-year-old woman who was in hyperosmolar non-ketotic diabetic coma. No previous description of such a case has been found. The clinical picture was characterized by the typical findings of hyperosmolar coma, in addition to excessive serum creatine kinase and myoglobin levels and massive myoglobinuria with acute renal failure. The rhabdomyolysis became fully manifest only under insulin treatment, possibly the result of insulin-induced hypophosphataemia, which seems to be of importance in the causation of the rhabdomyolysis.

A case of acute renal failure after mercuric chloride poisoning is reported. Laboratory data revealed markedly elevated serum concentrations of aldolase and creatine phosphokinase, and the presence of pigment granular casts and myoglobin in the urine. The patient went into a diuretic phase after 12 days of oliguria and renal function returned to normal during the next 10 days. Besides direct nephrotoxicity, the contributory role of rhabdomyolysis in the pathogenesis of renal failure after mercuric chloride poisoning, has been stressed.
