The symptoms of a myopathy permanently affecting limb girdle muscles are reported in a 31-year-old woman who has been presenting an exertional muscle pain syndrome with myoglobinuria for 20 years. Investigations revealed a slightly decreased utilization of glycogen in muscle, while its storage affected only rare type 2 fibers. Active phosphorylase was undetectable and phosphorylase b kinase activity was clearly decreased in muscle cells, but normal in erythocytes, lymphocytes and cultured fibroblasts.

A 31-year-old man had a nine-year history of exercise-induced cramps and muscle pain without myoglobinuria. Results of laboratory investigations differentiated his condition from the known disorders of carbohydrate and lipid metabolism. Light and electron microscopic examination of a muscle biopsy specimen showed tubular aggregates confined to type II fibers. Although the relationship of tubular aggregates to muscle cramps is uncertain, this association has been described previously and may be significant.

A 47-year-old man suffering from a bipolar disorder and intermittent myoglobinuria presented with acute rhabdomyolysis with renal failure after starting therapy with valproic acid. On morphological examination, skeletal muscle revealed increased lipid storage. Biochemically, decreased enzyme activity of carnitine palmitoyltransferase (CPT) type II with carnitine levels in the lower limit was found. Genetic analysis detected the common Ser113Leu substitution on one allele of the CPT2 gene. We conclude that valproic acid should be avoided in patients with CPT type II deficiency.

The author reviewed records of 10 patients who had experienced acute loxapine overdose. The most frequent medical complications were CNS depression, sinus tachycardia, hypertension, and hypothermia; 6 patients had had generalized major motor seizures, 1 had had recurrent paroxysmal atrial tachycardia, and 2 had had transient renal insufficiency from rhabdomyolysis and myoglobinuria. Other clinical effects from loxapine overdose were predominantly anticholinergic. The author recommends that loxapine-overdose patients receive ECG monitoring and treatment of medical complications in an intensive …

We studied a 25-year-old man with paresis of the limbs and neck, scapular atrophy, facial weakness, exercise intolerance and frequent episodes of myoglobinuria. Muscle histochemistry and biochemistry revealed a combined defect of myophosphorylase and AMP deaminase. Molecular genetic analysis showed that the patient was homozygous for the two most common mutations associated with myophosphorylase and AMP deaminase deficiencies. This is the second documented case of genetic 'double trouble', which should be looked for in patients with unusual severe phenotypes.

Carbon monoxide poisoning in a family of 3 persons resulted in renal failure with neurological damage in a 40-year-old husband and a fatal neurological injury in 35-year-old wife whereas the newborn child survived without any ill effects. Rhabdomyolysis and myoglobinuria secondary to anoxia was the probable cause of acute renal failure. The recognition of nontraumatic rhabdomyolysis-related acute renal failure is important in preventing fatality if neurological salvage is done at the right time.

The exertional muscle pain syndrome in adults is expressed by recurrent episodes of pain and/or episodes of acute rhabdomyolysis, sometimes with transient myoglobinuria or even acute renal failure. The enzymatic origin of this syndrome is suggested by a familial history, by its usual onset during childhood or adolescence, and by its recurrent attacks usually, but not exclusively, caused by exercise. The aetiological diagnosis, suggested by the clinical context and some dynamic tests, is almost always based on good histological and biochemical examinations of the muscle biopsy.

Carnitine palmitoyl transferase (CPT) II deficiency is usually manifested around puberty by exercise induced myoglobinuria. Two Ashkenazi Jewish sibs with the rare antenatal form of CPTII deficiency are reported. On the 5th gestational month periventricular calcifications and markedly enlarged kidneys were found in both of them. The activity of CPTII in lymphocytes was undetectable and both sibs were homozygous for the 1237delAG mutation. Because of the serious consequences of homozygosity for this mutation, genotype determination of all Ashkenazi patients with the adolescent form of CPTII def…

A case of Rhabdomyolysis occurred in a healthy boy 9 years of age after general anaesthesia. Succinylcholine, nitrous oxide and isoflurane were used for induction and maintenance of anaesthesia. Patient developed severe muscle pain, myoglobinuria, haemoglobinuria. Also creatinine phosphokinase was elevated up to 10,694 IU/L. Management was prompt and he was discharged home the third day in good condition. The injection of succinycholine may have precipitated rhabdomyolysis. This is a rare complication of succinylcholine without the disastrous outcome of renal failure, hyperkalaemia or cardiac …

The lipid storage myopathies are produced by a biochemical defect that directly or indirectly interferes with the metabolism of fatty acids in the muscle fiber. The oxidation of fatty acids is performed by enzymes lying in the mitochondrial membranes and matrix. The phenotype is very similar between the different biochemical defects, presenting with two large groups: One with early-onset, metabolic crisis and multisystemic failure, and one with late-onset, predominant myopathy and myoglobinuria. The diagnosis of these disorders has advanced significatively in the last ten years, permitting the…

A 28-year-old woman, treated for schizophrenia, developed severe hypotonic hyponatremia (serum Na: 109 mEq/L) after several days of compulsive water drinking. The patient was admitted in a coma and required intensive supportive therapy. Rhabdomyolysis quickly followed with high serum creatine phosphokinase levels and myoglobinuria. A high volume alkaline diuresis was initiated. Renal failure or compartment syndrome did not complicate the clinical picture. The mechanisms causing water intoxication and hyponatremia are discussed as are the possible pathogenetic explanations behind acute hyponatr…

Combinations of lovastatin and other drugs have been reported to cause rhabdomyolysis and myoglobinuria. The authors report such a case in a 72-year-old man who had been receiving atenolol, acetylsalicylic acid (ASA), dipyridamole, lovastatin, danazol, prednisone and doxycycline. The ASA, lovastatin and danazol were discontinued. The symptoms resolved, and laboratory test results were normal within 2 weeks. Lovastatin was strongly suspected; danazol was the most likely potentiator by diminishing the metabolism of lovastatin and its metabolites in the liver or by having a direct toxic effect on…

The occurrence of masseteric muscle spasm (MMS) in children is thought to be frequent and to be associated usually with malignant hyperpyrexia (MH). We have found a lower incidence of MMS in children and 50% had no muscle abnormality. Clinical features that support a diagnosis of MH include high serum creatine kinase (CK) concentration and the presence of myoglobinuria. There is evidence to suggest that suxamethonium does increase jaw tone in adults and children, and an exaggerated response may be taken clinically to be MMS. However, MMS should still be regarded as an early warning sign of MH.

Neuroleptic malignant syndrome and acute myoglobinuric renal failure developed in three patients after they had received a combination of a major tranquilizer and a tricyclic antidepressant or after a large dose of haloperidol. Rhabdomyolysis was demonstrated by biochemical methods as well as by a muscle biopsy. Myoglobinuria was confirmed by radioimmunodiffusion and also was verified in a necropsy specimen. Dehydration and drug interaction with tricyclic antidepressants seem to be predisposing factors. Renal function recovered when the patient survived the acute episode, but neurologic sequel…

The main features of alcoholic rhabdomyolysis-skeletal muscle necrosis, marked elevation of serum creatine phosphokinase, and myoglobinuria-were produced in rats by a combination of relatively prolonged (2 to 4 weeks) exposure to ethanol and a brief period of food deprivation. This observation suggests that fasting may similarly trigger muscle injury during binge drinking in man. The effect of fasting is in part related to an increase in blood alcohol due to reduced alcohol clearance and in part caused by a fasting-induced potentiation of the toxic effects of high concentrations of alcohol of …

A 54-year-old woman had muscle disease that terminated fatally with complete body paralysis. The patient had renal failure, which progressed concurrently with her muscle weakness. Myoglobinuria was not demonstrated during the active stage of her illness. Renal biopsy specimens demonstrated immune deposits in the glomeruli, and, at autopsy, there was a generalized glomerulonephritis. We believe the muscle disease was a consequence of acute rhabdomyolysis, hyperacute polymyositis, or virus infection. Regardless of the nature of the muscle disease, an immune mechanism may have been responsible fo…

Toxoplasma polymyositis was diagnosed in a dog given repeated injections of glucocorticoids for a skin condition. The dog developed generalized lower motor neuron paralysis, with generalized hyperesthesia, myoglobinuria, and high serum muscle enzyme activities. Diagnosis of toxoplasmosis was confirmed by muscle biopsy and seroconversion. Despite supportive care, the dog's neurologic status deteriorated progressively until parenteral treatment with clindamycin was instituted. The dog made a gradual recovery, with complete return of locomotor function and muscle mass within 2 months.

A 64 year-old man, who had been ingesting 20 g of licorice daily for about five months, developed a progressive, diffuse muscle weakness, with myoglobinuria, severe hypokalemia and elevated serum muscle enzymes. Electromyography and muscle biopsy results were consistent with a necrotizing myopathy. Following licorice withdrawal and appropriate electrolyte administration, complete clinical recovery was observed within about ten days. Previous reports of similar cases are reviewed and causative factors of this kind of myopathy are discussed.

A 13-year-old boy developed cramps and myoglobinuria following exertion. Mitochondrial preparations from a skeletal muscle biopsy were deficient in carnitine palmitoyltransferase (CPT) activity when assayed by the hydroxamate and kinetic assays. The patient's fibroblasts were also deficient when assayed by the hydroxamate and kinetic assays, but not when tested by the DTNB (5,5'-dithiobis-[nitrobenzoic acid]) method. This disparity probably indicates a specific deficiency in fibroblasts of one of the two carnitine palmitoyltransferases, presumably CPT II.

Succinylcholine chloride, administered to horses anesthetized with halothane in oxygen and mechanically ventilated, caused slight but significant (P less than 0.01) increases in heart rate. Significant alteration in mean arterial blood pressure did not occur, and there were clinically insignificant increases in serum K+ and C1- concentration. Cardiac dysrhythmia and myoglobinuria did not occur. Thus, effects of halothane anesthesia and mechanical ventilation prevented cardiac dysrhythmia and hypertension and greatly reduced the tachycardia generally associated with siccinylcholine administrati…
