The efficacy of thread embedding acupuncture on pain score, neck disability index, and pressure pain threshold for myofascial pain therapy in the upper trapezius muscle.

OBJECTIVES: Myofascial pain characterized by myofascial trigger point (MTrP) is a common cause of musculoskeletal pain and the main cause of neck and shoulder pain in the working population. Thread embedding acupuncture (TEA) is a new modality that can provide long-term stimulation to prolong the same therapeutic effect as conventional acupuncture. This study aimed to determine the effect of TEA therapy on pain score, neck disability index (NDI), and pressure pain threshold (PPT) for myofascial pain in the upper trapezius muscle. METHODS: The research design in this study was a double-blind,

Palmitoylethanolamide and stearoylethanolamide levels in the interstitium of the trapezius muscle of women with chronic widespread pain and chronic neck-shoulder pain correlate with pain intensity and sensitivity.

Chronic widespread pain (CWP) is a complex condition characterized by central hyperexcitability and altered descending control of nociception. However, nociceptive input from deep tissues is suggested to be an important drive. N-Acylethanolamines (NAEs) are endogenous lipid mediators involved in regulation of inflammation and pain. Previously we have reported elevated levels of the 2 NAEs, the peroxisome proliferator-activated receptor type-α ligand N-palmitoylethanolamine (PEA) and N-stearoylethanolamine (SEA) in chronic neck/shoulder pain (CNSP). In the present study, the levels of PEA and S

Altered cortical morphology and structural covariance network in chronic neck and shoulder pain.

PURPOSE: Despite regional brain structural alterations reported in patients with chronic neck and shoulder pain (CNSP), the specific characteristics of surface-based morphometry (SBM) and structural covariance networks (SCNs) still remain unknown. This study applied cortical morphological and graph theoretical analysis to investigate cortical morphometric alterations and global and nodal network abnormalities in CNSP. METHODS: Fifty patients with CNSP and 50 matched healthy controls (HCs) were enrolled in this experiment. All participants underwent structural magnetic resonance imaging and cl

Severe neck pain: a report of 3 cases of spontaneous thyroid nodule hemorrhage.

Spontaneous intranodular thyroid hemorrhage (ITH) is a rare clinical occurrence. Previous case reports describe sudden severe neck swelling, often resulting in life-threatening respiratory distress that may necessitate urgent surgical intervention. Although thought to be rare, the true incidence of ITH is unknown, and ITH should be considered in the differential diagnosis of patients presenting with acute neck symptoms, due to its potential to cause airway compromise. We illustrate a more subtle presentation of ITH in a case series of 3 female patients with acute neck swelling and pain: a 34-y

Cervical facet joint interventions for neck pain: an anatomically and clinically focused review.

Cervical facet joints (CFJs) are a frequent source of neck pain, contributing to both localized and referred pain patterns. However, diagnosis remains difficult owing to the nonspecific nature of the clinical findings and limited accuracy of current diagnostic methods. Among available approaches, diagnostic blocks with local anesthetics are considered the most reliable technique for identifying CFJ-mediated pain. Although various treatment strategies are available for managing pain, including pharmacologic, rehabilitative, and surgical approaches, this review focuses on interventional treatmen…

Neck pain and emotional state in cervical spondylosis: A dual trajectory model analysis.

BACKGROUND: Neck pain, a primary symptom of cervical spondylosis, affects patients' physical and mental health, reducing their quality of life. Pain and emotional state interact; however, their longitudinal interrelationship remains unclear. In this study, we applied a dual-trajectory model to assess how neck pain and emotional state evolve together over time and how clinical interventions, particularly acupuncture, influence these trajectories. AIM: To investigate the longitudinal relationship between neck pain and emotional state in patients with cervical spondylosis. METHODS: This prospec…

Daoyin therapy in chronic neck pain: study protocol for a randomized controlled trial.

BACKGROUND: Daoyin therapy (DT), an ancient therapeutic approach with a history spanning thousands of years, has traditionally been employed to address musculoskeletal pain and psychosomatic disorders. However, the application of DT for chronic neck pain (CNP) has received limited attention in the existing literature, and systematic randomized clinical trials (RCTs) in this context remain scarce. This manuscript outlines an RCT protocol designed to investigate whether DT is more effective at alleviating CNP in adult individuals compared to other interventions. METHODS: A 12-week RCT was condu…

Pericardial Adhesion and Chronic Non-Specific Neck Pain following Thoracentesis: An Osteopathic Approach.

Cardiovascular diseases (CVDs) are the leading cause of death globally. Morbidity and disability related to non-fatal events are increasing exponentially. There are several symptoms that may arise after invasive therapeutic approaches such as coronary artery bypass graft (CABG), including chronic pain in anatomical areas connected to the mediastinum; these pains can be found not only initially after surgery but also years later. We present a case where non-specific neck pain (NNP), in a patient undergoing CABG five years earlier, was resolved with an osteopathic technique, working the pericard…

Prediction of Patient Satisfaction after Treatment of Chronic Neck Pain with Mulligan's Mobilization.

Chronic neck pain is among the most common types of musculoskeletal pain. Manual therapy has been shown to have positive effects on this type of pain, but there are not yet many predictive models for determining how best to apply manual therapy to the different subtypes of neck pain. The aim of this study is to develop a predictive learning approach to determine which basal outcome could give a prognostic value (Global Rating of Change, GRoC scale) for Mulligan's mobilization technique and to identify the most important predictive factors for recovery in chronic neck pain subjects in four key …

Bedside clinical tests to assess sensitization in office workers with chronic neck pain.

OBJECTIVE: This study aimed to assess pain sensitization in individual office workers with chronic neck pain through simple bedside quantitative sensory tests (QST) and to associate the findings with pain intensity and pain catastrophizing. METHODS: One hundred-and-four office workers with chronic neck pain were assessed using pressure pain threshold (PPT) considering pain sensitive if PPTs were lower than 155 kPa in the upper trapezius and 245 kPa in the tibialis anterior. Pain sensitive to temporal summation of pain (TSP) was considered if there was a difference of two points in the visual …
