We describe a novel reconstructive approach using a free vascularized tibial bone transfer with skin and tibial nerve components to salvage a complex upper extremity injury in the setting of a non-salvageable lower limb. This technique was applied in a patient who presented with severe blast injuries to the left upper arm and to the right upper and lower leg. Injuries of the right lower leg were so severe that reconstruction was deemed surgically not feasible. The harvested composite free flap included tibia, posterior tibial artery, tibial nerve and a skin paddle based on the posterior tibial

BACKGROUND: Lumbar disc herniation (LDH) causes compositional alterations within compressed nerve roots, resulting in low back pain (LBP). The ultrashort echo time magnetization transfer technique (UTE-MT) facilitates assessment of macromolecular changes in collagen- or myelin-rich tissues in nerve roots.
PURPOSE: To assess lumbar nerve root composition in LDH using UTE-MT.
STUDY TYPE: Prospective.
POPULATION: One hundred and seventy-six participants (age range, 20-89; 72 females) with LDH.
FIELD STRENGTH/SEQUENCE: 3T/UTE-MT, Carr-Purcell-Meiboom-Gill (CPMG).
ASSESSMENT: UTE-MT ratio (UTE

OBJECTIVES: Nerve transfer is a promising intervention for restoring hand and upper limb function after cervical spinal cord injury (SCI), but the timeline of neurophysiological recovery in humans remains unclear. This study aimed to define recovery profiles after nerve transfers to restore upper limb function.
METHODS: Individuals with traumatic cervical SCI who received nerve transfer surgery in at least 1 limb were evaluated during routine clinical visits for up to 2 years post-transfer. The primary outcome was the time to detect reinnervation, defined as the appearance of voluntary motor …

PURPOSE: In this study, we proposed a new tendon transfer method for thumb function reconstruction to treat radial nerve injuries. We specifically focused on enhancing thumb radial abduction by transferring the palmaris longus (PL) to the extensor pollicis brevis (EPB) while preserving the first compartment.
METHODS: Eight freshly frozen cadaver specimens were used to compare our proposed technique (transferring the PL to the EPB while preserving the first compartment) with the Tsuge method (transferring the extensor pollicis longus (EPL) and fixing the abductor pollicis longus to the flexor

Stroke and traumatic brain injury lead to upper motor neuron syndrome, which is characterized by muscle spasticity or paresis of varying severity depending on the lesion’s location and extent. Current treatments are mostly symptomatic with limited efficacy and significant side effects. Nerve transfer techniques, such as the contralateral L4 ventral root transfer in animal models and C7 root transfer in both animal and clinical studies, have been shown to reduce spasticity and improve function in upper motor neuron syndrome; however, they lack selectivity. Our hypothesis is that using a selecti…

BACKGROUND: Women with spinal cord lesions have loss of sensation in the lower body, negatively affecting sexuality in many ways: decreased, lost, or changed genital sensation and difficulties with orgasm. Restoring genital sensation in men with low spinal lesions using a nerve transfer (to maximize sensation, sexuality, and quality of life [TOMAX] procedure) has shown the potential to enhance sexual functioning and satisfaction. This procedure was adapted to a female version, in which the dorsal clitoral nerve was transferred to the ilioinguinal nerve to restore genital sensation. We report t…

BACKGROUND: In complete brachial plexus injury, phrenic nerve (PN) is frequently used in neurotization for elbow flexion restoration. The advancement in video-assisted thoracoscopic surgery (VATS) allows full-length PN dissection intrathoracically for direct coaptation to recipient without nerve graft.
PURPOSE: We report our experience in improving the surgical technique and its outcome.
METHODS: Seven patients underwent PN dissection via VATS and full-length transfer to musculocutaneous nerve (MCN) or motor branch of biceps (MBB) from June 2015 to June 2018. Comparisons were made with simil

BACKGROUND: Nerve transfer surgery is sometimes offered to patients with acute flaccid myelitis (AFM). The objectives of this study were to evaluate surgical efficacy, assess which clinical and neurophysiological data are valuable for preoperative planning, and report long-term outcomes.
METHODS: This is a single-center, retrospective case series of patients with AFM who received nerve transfer surgery. All patients had preoperative electromyography and nerve conduction studies (EMG/NCS). Matched control muscles that did not receive nerve transfer surgery were defined in the same cohort.

BACKGROUND: Targeted muscle re-innervation (TMR) is increasingly being used for treatment of postamputation pain and myoelectric prosthesis (MYP) control. Palmaris longus (PL) is a potential target following transradial amputation. The purpose of this study was to determine the branching pattern of the median nerve (MN) as it pertains to the PL motor branch entry point (MEP) and to present clinical results of patients who had PL used as a target.
METHODS: Eight cadaveric arms were dissected and branching patterns of the MN were documented. Additionally, we reviewed adult patients from a prosp

INTRODUCTION: The ulnar nerve palsy is a distressing injury, resulting in clawing of hand. The cause of clawing is due to paralysis of the interosseous muscles in the presence of functioning long extensor and long flexors of fingers. Various methods have been proposed to correct this deformity which include both static and dynamic procedures. In this study, we share our experience with flexor digitorum superficialis tendon transfer using Zancolli's modification for anti-claw correction.
MATERIALS AND METHODS: It was a retrospective case series study. A record of 53 patients was included in th

PURPOSE: To compare functional outcomes of single versus double tendon transfer for foot drop correction and toe drop prevention in posttraumatic common fibular nerve palsy.
METHODS: A retrospective study was conducted on data from patients with posttraumatic common fibular nerve palsy treated by tendon transfer between 2001 and 2018. In cases of single tendon transfer (STT) the tibialis posterior (TP) tendon was transferred anteriorly through the interosseous membrane to a new insertion on the lateral cuneiform. In cases of double tendon transfer (DTT), the same TP tendon transfer was combin

Background: Multiple sclerosis (MS) has traditionally been regarded as a disease confined to the central nervous system (CNS). However, neuropathological, electrophysiological, and imaging studies have demonstrated that the peripheral nervous system (PNS) is also involved, with demyelination and, to a lesser extent, axonal degeneration representing the main pathophysiological mechanisms. Aim: The purpose of this study was to assess PNS damage at the lumbar plexus and sciatic nerve anatomical locations in people with relapsing-remitting MS (RRMS) and healthy controls (HCs) in vivo using magneti

Trapezius paralysis following injury to the spinal accessory nerve can be a debilitating complication resulting from lymph node biopsy, radical neck dissection, or penetrating trauma in the region of the posterior cervical triangle. Disruption of the delicate muscular balance in the shoulder girdle may result in lateral scapular winging, ipsilateral upper extremity radiculopathy, and limited shoulder function and range of motion. Spontaneous recovery with nonoperative management is possible in some patients, and restoration of function after reparative neural procedures has been observed in pa

Background: Injury to the common peroneal nerve disrupts the motor control pathway to ankle dorsiflexors and evertors, as well as toe extensors, resulting in pathological gait and foot drop. Direct external compression on the fibular head is the most frequent cause of peroneal nerve impairment and has poor prognosis. Methods and Patients: Here, we report the surgical outcome of 21 patients with foot drop (9 males and 12 females) who underwent nerve transfer procedure of either the superficial peroneal nerve or the tibial nerve fascicles to the motor branch of the tibialis anterior and to the d…

Lower extremity spasticity is a common sequela among patients with acquired brain injury. The optimum treatment remains controversial. The aim of our study was to test the feasibility and effectiveness of contralateral nerve root transfer in reducing post stroke spasticity of the affected hindlimb muscles in rats. In our study, we for the first time created a novel animal hindlimb spastic hemiplegia model in rats with photothrombotic lesion of unilateral motor cortex and we established a novel surgical procedure in reducing motor cortex lesion-induced hindlimb spastic hemiplegia in rats. PURPOSE: Facial paralysis is a congenital or acquired disorder of varying severity leading to an asymmetric or absent facial expression. It represents an important debilitation from both esthetic and functional points of view. In this article we report our experience with patients treated with gracilis muscle transplantation innervated by the motor nerve to the masseter muscle. We discuss the surgical technique and the functional and esthetic results and evaluate the effectiveness of this donor nerve in providing adequate innervation to the muscle transfer for lower facial reanimation.
