Ultrasound-Confirmed Median Nerve Pseudoconduction Block due to Anatomical Variation.

INTRODUCTION/AIMS: Conduction block (CB) at non-entrapment sites is an important electrodiagnostic (EDX) feature supporting acquired demyelinating neuropathies. However, anatomical variation may produce pseudoconduction block, mimicking true CB. After encountering a patient with median nerve pseudoconduction block due to anatomical variation, we aimed to assess the frequency of this variation, its impact on EDX interpretation, and the role of neuromuscular ultrasound (NMUS) in improving diagnostic accuracy. METHODS: We describe an index patient with median nerve pseudoconduction block in the

Intracutaneous Amyloid Deposition is Associated With Nerve Conduction Studies Deterioration in Presumed Asymptomatic Pathogenic Variant TTR Carriers.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv amyloidosis) is an autosomal dominant systemic disease, with an overall poor prognosis. Markers of disease onset are urgently required to optimize the timing of treatment initiation. Nerve conduction studies (NCS) are an objective, reproducible, and non-invasive tool for following large nerve fiber involvement. Our objective was to determine whether the presence of intracutaneous amyloid deposition (ICAD) was associated with a higher risk of developing the disease, defined as a decline observed on nerve conduction studies, in a population

Acute effects of spinal transection on EPSPs produced by single homonymous Ia-fibers in soleus alpha-motoneurons in the cat.

1. Excitatory postsynaptic potentials (EPSPs) generated in soleus motoneurons by single homonymous Ia-fibers were measured using intracellular recording and the spike-triggered averaging technique. Two groups of barbiturate-anesthetized adult cats were studied: one with the spinal cord intact and the other with the spinal cord severed at thoracic segment 13 (T13) several hours prior to recording. 2. In cord-transected cats, single homonymous Ia-fibers produced EPSPs in soleus motoneurons that were, on average, larger and faster rising relative to normal, as they are for those produced in media

Unveiling a Regional Variant of Demyelinating Guillain-Barré Syndrome: Nerve Conduction Study Evidence in Bifacial Weakness With Paresthesias.

BACKGROUND AND AIMS: Guillain-Barré syndrome (GBS) is categorized into acute inflammatory demyelinating polyneuropathy (AIDP) and axonal GBS. Bifacial weakness with paresthesias (BFP) is regarded as a rare regional variant of AIDP, yet its electrophysiological characteristics remain inadequately defined. This study aimed to clarify the pathophysiological basis of BFP and its classification under AIDP through serial nerve conduction study (NCS) analysis. METHODS: NCS data of six patients diagnosed with BFP were retrospectively analyzed. Four patients underwent serial NCSs, while two received a

Dynamics of Nerve Conduction Studies in Patients With Guillain-Barré Syndrome.

INTRODUCTION/AIMS: The value of electrodiagnostic subtyping of Guillain-Barré syndrome (GBS) is still debated. This study aimed to determine the diagnostic yield, timing, and changes of the electrodiagnostic subtyping in patients with GBS in serial nerve conduction studies (NCS). METHODS: Data were extracted from the International GBS Outcome Study (IGOS) database. Serial NCS were available for 469 patients. For the serial NCS analysis, the intervals between the first and second study were defined as ≥ 7 and ≤ 42 days after onset of weakness. All NCS were classified according to the electrodi

Intraparenchymal stem cell transplantation for spinal cord injury: a review of clinical trials.

Traumatic spinal cord injury (SCI) results in severe and often permanent motor, sensory, and autonomic impairment, and current treatments offer limited restorative benefit. Stem-cell based approaches have been investigated to promote neural repair, but peripheral delivery is limited by poor cell localization and engraftment. Intraparenchymal (IP) transplantation, which delivers cells directly into injured spinal cord, is intended to improve local retention, survival, and integration. This review synthesizes early-phase clinical trials from the United States, Canada, Europe, and Australia, eval

Multi-channel intraneural vagus nerve recordings with a novel high-density carbon fiber microelectrode array.

Autonomic nerves convey essential neural signals that regulate vital body functions. Recording clearly distinctive physiological neural signals from autonomic nerves will help develop new treatments for restoring regulatory functions. However, this is very challenging due to the small nature of autonomic nerves and the low-amplitude signals from their small axons. We developed a multi-channel, high-density, intraneural carbon fiber microelectrode array (CFMA) with ultra-small electrodes (8-9 µm in diameter, 150-250 µm in length) for recording physiological action potentials from small autonomi

Preclinical evaluation of a novel bilayer collagen nanofiber nerve wrap for accelerating axonal regrowth and myelination in a canine peripheral nerve injury model.

Peripheral nerve injury remains a major clinical challenge due to unsatisfactory functional recovery after severe nerve damage. Although nerve wraps have emerged as a promising strategy for clinical peripheral nerve repair, conventional nerve wraps still exhibit insufficient topographical guidance during regeneration. In this study, we developed a bilayer collagen nanofiber nerve wrap via blow-spinning technology, consisting of an outer random fibrous layer for structural support and an inner aligned nanofibrous layer for longitudinal neural guidance. To further evaluate its regenerative effic

Melatonin-incorporated brain extracellular matrix hydrogel enhances NSCs mitochondrial metabolism to promote neuroregeneration via the AMPK-PGC-1α-NRF1/TFAM axis after spinal cord injury.

Spinal cord injury (SCI) results in severe and debilitating neurological damage. Mitochondria play a crucial role in the differentiation of neural stem cells (NSCs) and neural regeneration. However, mitochondrial dysfunction occurs following SCI, manifesting as dysregulation of oxidative stress and ATP depletion, which impede neural regeneration. In this study, we developed a multi-functional, injectable hydrogel platform by integrating a brain-derived extracellular matrix (BEM), the neuroprotective agent melatonin (MT), and NSCs. We hypothesized that this NSCs@MT/BEM system would create a syn

Spinal mechanisms underlying potentiation of hindpaw responses observed after transient hindpaw ischemia in mice.

Transient ischemia produces postischemic tingling sensation. Ischemia also produces nerve conduction block that may modulate spinal neural circuits. In the present study, reduced mechanical thresholds for hindpaw-withdrawal reflex were found in mice after transient hindpaw ischemia, which was produced by a high pressure applied around the hindpaw for 30 min. The reduction in the threshold was blocked by spinal application of LY354740, a specific agonist of group II metabotropic glutamate receptors. Neural activities in the spinal cord and the primary somatosensory cortex (S1) were investigated

Changes in axonal physiology and morphology after chronic compressive injury of the rat thoracic spinal cord.

The spinal cord is rarely transected after spinal cord injury. Dysfunction of surviving axons, which traverse the site of spinal cord injury, appears to contribute to post-traumatic neurological deficits, although the underlying mechanisms remain unclear. The subpial rim frequently contains thinly myelinated axons which appear to conduct signals abnormally, although it is uncertain whether this truly reflects maladaptive alterations in conduction properties of injured axons during the chronic phase of spinal cord injury or whether this is merely the result of the selective survival of a subpop
