Preoperative Tumor-Informed ctDNA for Prediction of Nodal Metastases at Radical Cystectomy in Bladder Cancer.

BACKGROUND: This study aimed to evaluate whether pre-surgical tumor-informed circulating tumor DNA (ctDNA) predicts pathologic nodal status at radical cystectomy and its potential role in risk stratification for future biomarker-guided surgical strategies. METHODS: The study prospectively analyzed 40 patients with non-muscle-invasive and muscle-invasive bladder cancer who underwent radical cystectomy with pelvic lymph node dissection (LND) and pre-surgical tumor-informed ctDNA testing. Compared with pathologic nodal findings, ctDNA status was classified as positive or negative. Diagnostic per

HALP score as an independent prognostic biomarker for recurrence and progression in non-muscle-invasive bladder cancer: A retrospective cohort study.

OBJECTIVE: This study aimed to evaluate the prognostic utility of the HALP (Hemoglobin-Albumin-Lymphocyte-Platelet) score in predicting tumor recurrence in patients with non-metastatic bladder cancer (NMIBC) without muscle invasion. METHODS: A retrospective cohort analysis was conducted involving 428 NMIBC patients followed for at least five years. HALP scores were calculated using preoperative hemoglobin, albumin, lymphocyte, and platelet measurements. Tumor recurrence was defined as the appearance of new lesions during follow-up, while muscle invasion was classified as progression. Determin

Addressing the challenges of health economic modelling in the context of suboptimal evidence base - case study based on a comparison between photodynamic diagnosis and narrow band imaging in non-muscle invasive bladder cancer.

INTRODUCTION: Standard economic models, which have been developed to reflect typical data availability for pharmaceutical products, often struggle with non-pharmaceutical interventions like diagnostics and medical devices, where clinical trial data is more likely to be short-term or non-comparative. This paper explores methodological challenges in evaluating Photodynamic Diagnosis (PDD) via blue light cystoscopy (BLC) versus Narrow Band Imaging (NBI) for non-muscle invasive bladder cancer (NMIBC). METHODS: A semi-Markov state-transition cost-utility model was developed for a health technology

FGFR3 gene mutations screening in non-muscle invasive bladder cancer (NMIBC) in the Tunisian population.

BACKGROUND: Mutations occurring in the Fibroblast Growth Factor Receptor (FGFR3) gene are thought to be associated with the incidence and prognosis of non-muscle invasive bladder tumors (NMIBC). Yet, their prognostic significance in the Tunisian population remains unclear. Herein, we aim to investigate their prognostic impact in NMIBC in terms of recurrence, progression, and survival. METHODS AND RESULTS: It is a monocentric retrospective study including 42 NMIBC patients. DNA was isolated from formalin-fixed paraffin-embedded tissue samples. Polymerase Chain Reaction (PCR) followed by Sanger

Clinical Outcomes in Bacillus Calmette-Guérin-Exposed Non-Muscle-Invasive Bladder Cancer.

IMPORTANCE: Bacillus Calmette-Guérin (BCG) therapy is the standard of care for non-muscle-invasive bladder cancer (NMIBC), yet many patients experience recurrence and progression, and clinical adherence to recommended regimens is low. OBJECTIVE: To examine the clinical burden, treatment patterns, and outcomes of patients with BCG-exposed NMIBC in the US. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study examined Optum's deidentified Market Clarity Data (Optum Market Clarity), linking US electronic health records and insurance claims from January 1, 2007, to June 30, 2024. Fruit and vegetable consumption, dietary and plasma carotenoids and risks of recurrence and progression in patients with non-muscle-invasive bladder cancer.

PURPOSE: Fruit and vegetable consumption may decrease bladder cancer risk, but research on its relation to outcomes of non-muscle-invasive bladder cancer (NMIBC) is scarce. We investigated the association of (changes in) pre- and postdiagnosis fruit and vegetable consumption, as well as postdiagnosis dietary and plasma carotenoids, with risks of recurrence and progression in patients with NMIBC. METHODS: Data from patients with newly diagnosed NMIBC between 2014 and 2021 from the prospective multi-centre cohort UroLife was used. Patients reported prediagnosis diet (n = 1396), and diet at 3 an

Metastatic non-muscle invasive bladder cancer: histology-specific metastatic patterns and survival in a national cancer database analysis.

BACKGROUND: Metastasis development in bladder cancer patients without any evidence of muscle invasion is known as metastatic non-muscle invasive bladder cancer (mNMIBC). Given its recent recognition, this condition is largely underexplored. The objective of our study was to analyze the baseline characteristics, metastatic patterns, and survival outcomes of this condition based on different histology subtypes. METHODS: Data from patients with mNMIBC were analyzed across different histology subtypes. Demographic and clinical variables were compared using Chi-square and Fisher's exact analysis.

Intratumoral and Peritumoral Fat CT‑Based Radiomics for Predicting Recurrence Risk in Non-Muscle-Invasive Bladder Cancer: A Two-Center Study.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) has a high risk of recurrence, and multiple surgeries increase the disease burden on patients. Using computed tomography (CT)-based machine learning, this study established a pre-treatment recurrence prediction model incorporating tumor and peritumoral fat characteristics. This approach may guide early clinical interventions. METHODS: In this retrospective study, 208 NMIBC patients who underwent enhanced CT before transurethral resection of bladder tumor (TURBT) with intravesical chemotherapy were collected from two hospitals. The radiomi

Induction and maintenance of sequential intravesical gemcitabine/docetaxel for intermediate and high-risk non-muscle invasive bladder cancer with different dosage protocols.

INTRODUCTION: The combination of sequential intravesical gemcitabine and docetaxel (Gem/Doce) chemotherapy has been considered a feasible option for BCG (Bacillus Calmette-Guérin) treatment in non-muscle invasive bladder cancer (NMIBC), gaining popularity during BCG shortage period. We seek to determine the efficacy of the treatment by comparing Gem/Doce induction alone vs induction with maintenance, and to evaluate the treatment outcomes of two different dosage protocols. METHODS: A bi-center retrospective analysis of consecutive patients treated with Gem/Doce for NMIBC between 2018 and 2023

Adapting patient priorities care for older adults with non-muscle-invasive bladder cancer: A qualitative inquiry.

INTRODUCTION: Shared decision-making involves understanding a person's goals and healthcare preferences, especially for older adults with cancer. When making treatment decisions, older adults with non-muscle-invasive bladder cancer (NMIBC) face tradeoffs due to aging and multimorbidity. Our objective was to obtain initial feedback from older adults with NMIBC and their urologists to begin adapting an existing written goal-elicitation tool, Patient Priorities Care (PPC), for shared decision-making in NMIBC. METHODS: We recruited 5 dyads of older adults with NMIBC (age ≥65) paired with their ur

Tumor immune microenvironment dynamics and outcomes of prognosis in non-muscle-invasive bladder cancer.

Agents that target PD-1 and PD-L1 have been developed in the treatment of bladder cancer (BC). However, the diversity of immune cell infiltration in non-muscle-invasive BC (NMIBC) and the dynamics of the microenvironment as it progresses to muscle-invasive/metastatic disease remains unknown. To assess tumor immune activity, hierarchical clustering was applied to 159 BC samples based on cellular positivity for the defined immune cellular markers (CD3/CD4/CD8/FOXP3/CD20/PD-1/PD-L1/LAG3/TIGIT), divided into two clusters. There was a "hot cluster" (25%) consisting of patients with a high expressio

rBCG-LTAK63 Outperforms BCG in Bladder Cancer Immunotherapy: Dendritic and T Cell Coordination Drives Superior Tumor Control in a Mouse Model.

Bacillus Calmette-Guérin (BCG) remains the standard treatment for non-muscle invasive bladder cancer (NMIBC), yet approximately 30% of patients fail to respond. To enhance therapeutic efficacy, we developed rBCG-LTAK63, a recombinant BCG strain, as a novel immunotherapeutic candidate. In vitro, rBCG outperformed BCG in MB49 cell/splenocyte co-cultures by enhancing T cell activation and improving spheroid growth control. In vivo, rBCG demonstrated superior antitumor efficacy, significantly reducing the growth of subcutaneously implanted MB49 tumor cells. Immune profiling revealed that rBCG uniq
