Revisiting human nose anatomy: phylogenic and ontogenic perspectives.

This review suggests revisiting nose anatomy by considering the ethmoidal labyrinths as part of the olfactory nose and not as paranasal sinuses. Phylogenetically, the olfactory and respiratory organs of the most primitive vertebrates are separated. Exaptation, a mechanism of evolution, may explain the fusion of the olfactory and respiratory organs in dipnoi. The respiratory and olfactory noses remain anatomically separated by the transverse lamina in most mammals, whose olfactory labyrinth is a blind recess housing the ethmoturbinates. In humans, the partitioning between the olfactory cleft an…

[Persistent extramedullary plasmacytoma of the nose after radiotherapy].

Solitary extramedullary plasmacytomas of the head and neck are rare tumors. These tumors are known to be very radiosensitive and therefore radiotherapy is often considered as the treatment of choice. We present the case of a solitary extramedullary plasmacytoma of the nose which did not respond sufficiently to radiotherapy, i.e. persistence of the tumor led to surgical treatment. After a discussion of the recent literature we primarily recommend the excision of this type of tumor whenever possible.

Three-dimensional anthropometric analysis of the Chinese nose.

Information about normal nasal dimensions is essential to rhinoplasty, but reports of anthropometric measurements of the Chinese nose are limited. Three-dimensional (3D) measuring apparatuses have been introduced into craniofacial anthropometry and have demonstrated advantages over conventional methods. This study aims to introduce a new 3D method for anthropometry and to provide guidance for plastic surgeons treating the Chinese nose. A total of 289 young Chinese adults (146 males and 143 females) were recruited for this study and a 3D stereo photogrammetry system (3DSS -II, Shanghai Digital …

Ear, nose, and throat effects of high altitude.

High altitude changes human physiology and can result in illnesses such as acute mountain sickness, high-altitude cerebral edema, and high-altitude pulmonary edema. The physiological impacts of high-altitude illnesses occur secondary to extravasation of fluid from the intravascular space into the extravascular space during a rapid ascent. Headache, hearing disturbances, vestibular disturbances, epistaxis, sleep apnea, coughing, respiratory tract infections, and nasal obstruction are main ear, nose, and throat complaints of individuals travelling to high altitude. These complaints can cause del…

Reconstruction of bilateral cleft lip and nose deformity.

Bilateral cleft lip and nose deformity can be divided into several types according to the extent of the cleft, protruding premaxilla, size of prolabium and nose deformity. Many repair techniques introduced in the literatures were not perfect because of the change of facial profile under the influence of facial growth. The author uses 1-stage cheiloplasty with nose correction for bilateral cleft lip and nose deformity. Early lip adhesion is used before definitive corrections in wide-cleft patients. The lateral mucosal flaps are used for the lining of alveolar cleft. The lateral orbicularis oris …

Ear, nose and throat manifestations in pemphigus vulgaris.

BACKGROUND: Pemphigus vulgaris (PV) is an autoimmune disease characterized by mucocutaneous intraepithelial blisters and pathogenic autoantibodies against desmoglein 3. There are two clinical forms: mucosal (MPV) and mucocutaneous (MCPV). The frequency of ear, nose and throat (ENT) involvement in PV is not clearly defined. Only a few isolated individual cases have been reported. OBJECTIVES: The objective of our study was to determine the incidence of ENT involvement in patients with PV. PATIENTS: We studied prospectively all 16 patients diagnosed with PV and treated in the Department of Derm…

Rhinoplasty: septal saddle nose deformity and composite reconstruction.

BACKGROUND: Saddle nose is one of the most challenging deformities in all of rhinoplasty surgery. Recent advances in aesthetic reconstructive surgical techniques warrant discussion of this subject. METHODS: A review of saddle nose cases revealed that an important subgroup exists, which has been designated the septal saddle nose deformity. The pathophysiology was weakening or loss of septal support, and not the classic dorsal overresection. A prospective study of 25 consecutive cases was then completed, with emphasis on analysis, classification, and treatment. RESULTS: Prior attempts at class…

Ear and nose foreign body removal in children.

The removal of foreign bodies in children is very common in the otolaryngologist's daily routine. We present a prospective series of 187 cases of ear and nose foreign bodies removed from children aged 0-12 years old in a 6-month period--95 females (50.8%) and 92 males (49.2%); 78 children (41.71%) aged from 1 to 2 years, 66 (35.29%) from 3 to 5 years. There were 93 cases (49.73%) of ear foreign bodies, in which a bean was the most common (24.73%), and 94 cases (50.27%) of nose foreign bodies, in which sponge fragments predominated (36.17%). In 82 cases (45.85%), the removal was performed withi…

[CT diagnosis of the nose-eye-related disease].

The findings of paranasal and (or) orbital CT scannings of 283 cases who suffered from the nose-eye-related diseases were studied retrospectively in this paper. Of 283 cases, which were proved either by the operation or histopathology, 128 were neoplastic diseases, 120 were traumatic fractures, 23 inflammatory lesions and 12 fibrous dysplasia. The combined use of bony windows and soft tissue windows were the most effective methods of accurately detecting nose-eye-related disease. Meanwhile, vascular contrast were used for neoplastic lesions and some other diseases suspected of fungul nasal-sin…

Accessory nose associated with unilateral incomplete cleft lip.

We describe a 6-month-old Turkish girl who was found to have a mass near her nose at birth and incomplete cleft lip. No other clinical and radiological abnormalities were demonstrable elsewhere in her body. Histological examination of the excised mass showed an organoid structure that was consistent with an accessory nose.

Ear-nose-throat abnormalities in the CHARGE association.

A comprehensive evaluation of the otolaryngological abnormalities in 50 patients with colobomata, heart defect, atresia of the choanae, retarded growth or development, genital hypoplasia, and ear anomalies or deafness (CHARGE) was performed. All the patients had ear abnormalities; 96% (48/50) had malformed pinnae, and 54% (27/50) had facial nerve palsies. Only 8% (4/50) had normal hearing, the commonest hearing defect being severe conductive or mixed loss. Eighty-four percent (42/50) of computed tomographic scans of the temporal bone were abnormal, the characteristic abnormality being the comb

Management of saddle nose deformity in atrophic rhinitis.

Correction of a saddle nose deformity due to atrophic rhinitis is a formidable task. The thick and puckered skin secondary to long standing disease makes the creation of a dorsal subdermal pocket difficult. On the one hand, these patients tolerate synthetic implants poorly and on the other they show an unusually high rate of absorption of autologous bone graft. Our experience of treating 15 patients with saddle nose deformity secondary to atrophic rhinitis is presented.

[Corrective rhinoplasty for post-traumatic deformity of nose].

From 1981 to 1986 the author has used osteotomy technique for corrective rhinoplasty in 15 cases. They were localized depression or deviation of the nose. The depression deformity was treated by osteotomy and elevating by making cuts along the midline of the nose and on each lateral cartilage. Then the dorsum of the nose was repositioned. All 15 cases obtained satisfactory cosmetic result without any complication.

Open v closed reduction of the fractured nose.

A stepwise study of nasal fractures includes an initial investigation that was carried out to discover why so many patients were awaiting rhinoplasty, which led us to find a 30% to 40% failure rate in nasal manipulation operations. We then studied the mechanics of nasal fracture by striking fresh cadaver noses. We found that if the nasal bones were deviated by more than half the bridge width, there was a concomitant C-shape fracture of the bony and cartilaginous septum. Two trials were performed, and patients with nasal deviation after trauma were allocated to either manipulation or manipulati…
