Newborn clinical outcomes of the AWHONN late preterm infant research-based practice project.

OBJECTIVE: To describe the neonatal health risks (hypothermia, hypoglycemia, hyperbilirubinemia, respiratory distress, the need for a septic workup, and feeding difficulties) experienced by late preterm infants (LPIs) from a large multisite study and determine how these risks were affected by gestational age at birth. DESIGN: Descriptive analysis of prospective data obtained as part of the AWHONN Late Preterm Infant Research-Based Practice Project. SETTING: Fourteen hospitals located through the United States and Canada. PARTICIPANTS: Late preterm infants (802) born at gestational ages betw

The association between shift patterns and the quality of hand antisepsis in a neonatal intensive care unit: An observational study.

BACKGROUND: Healthcare-associated infections represent a major burden in neonatal intensive care units. Hand antisepsis is the most important tool for prevention, however, compliance among healthcare workers remains low. OBJECTIVES: To prospectively evaluate the influence of different work shifts (extended working hours, night shifts) on the quality of healthcare workers' hand antisepsis. DESIGN: Observational study. SETTINGS: Two equivalent "Level III" neonatal intensive care units at the University Hospital Vienna, Austria. PARTICIPANTS: Seventy healthcare workers, 46 nurses and 24 physi

The newborn foot: diagnosis and management of common conditions.

An assessment of the foot should be a part of every newborn physical examination. The newborn foot, although complex in structure, can be examined quickly in any office setting. Many foot deformities are diagnosed immediately after birth, allowing for earlier and often more successful treatment. A neonate with a foot deformity can be a source of anxiety to parents. Adequate knowledge of commonly encountered neonatal foot problems enables the nurse to give appropriate anticipatory guidance to the parents. Foot abnormalities usually occur as isolated findings in an otherwise healthy newborn. How

Long-term alterations in the maternal behavior of neonatally undernourished rats.

The effect of early food and sensory deprivation on the maternal responsiveness of female rats was investigated. Animals that were neonatally undernourished by daily mother-litter separation (involving both food and sensory deprivation) showed significant deficits in maternal care, consisting of a reduction in nest rating, nursing time, and retrieving responses. Moreover, they exhibited exaggerated grooming and circling movements in comparison with the controls. Dams neonatally undernourished by the nipple-ligation of their mothers (a method that minimizes sensory deprivation) displayed less a

Preparing Parents for Discharge from the Neonatal Unit, the Transition, and Care of Their Preterm Children at Home.

Preparing parents to care for their preterm children is one of the principal challenges faced by nursing professionals within the neonatal care contexts. This process seeks for parents to acquire the skills to safely provide the differential care required by children at home given their prematurity condition. The preparation for discharge is complex and multidimensional, involving aspects that have to do with the knowledge, skills for caring, security, and trust to transit and take care of the children at home. This process is conducted in the neonatal unit gradually, in function of the clinic

Neonatal Hypernatremic Dehydration.

Newborn infants experience significant fluid shifts following delivery as a result of the contraction of extracellular water. Without prompt identification and treatment, significant weight loss can result in neonatal hypernatremic dehydration. This case study presents an infant with significant weight loss, dehydration, and electrolyte imbalances. The differential diagnosis for hypernatremia is presented along with clinical presentation, treatment, and discharge education recommendations.

Improving the selective neonatal BCG program.

OBJECTIVES: The aim of the work was to develop and implement an improved selective neonatal Bacille-Calmette-Guerin (BCG) vaccine program in which neonates are systematically assessed, and those identified as at "high risk of being exposed to tuberculosis" are given neonatal BCG. DESIGN: Service models used in other parts of the country were assessed and their limitations and difficulties were carefully considered. Possible service models for local use were then considered. A selective neonatal BCG program involving universal neonatal screening was implemented. SAMPLE: A program was implemen

Part 1. Injuries to the brachial plexus: mechanisms of injury and identification of risk factors.

Upper-arm weakness (paresis) or paralysis indicates peripheral-nerve damage to the brachial plexus, a network of lower cervical and upper thoracic spinal nerves supplying the arm, forearm, and hand. Physical findings reflect muscle paralysis from spinal nerve roots. The mechanism of injury includes maternal, obstetric, and infant factors that apply traction on or compression to the anatomically vulnerable brachial plexus. Nerve regeneration can occur if nerve tissue components are preserved. Recovery is affected by multiple factors, including the type and site of injury, intervention timing, a

A comparison of four methods of normal newborn temperature measurement.

PURPOSE: The purpose of this study was to: (a) compare newborn temperature measurements obtained by digital disposable, electronic, and tympanic thermometers with glass mercury thermometers, and (b) compare financial implications of each method. METHODS: In this correlational study, 12 perinatal and neonatal nurses obtained temperature measurements of 184 newborns between 1 and 168 hours of age. The stratified convenience sample was selected using medical records numbers. Temperature instruments included glass thermometer, tympanic thermometer, electronic thermometer, and a digital thermomete

Complete congenital heart block: a review and case study.

Complete congenital heart block (CCHB) is a rare disease of the newborn that carries significant morbidity and mortality. It generally occurs as a result of the presence of maternal autoantibodies that are transferred to the fetus and affect the fetal heart, or it may be associated with a congenital structural abnormality of the heart. Infants with CCHB are at risk for diminished cardiac output and the subsequent development of congestive heart failure. Many infants require the placement of a cardiac pacemaker. It is essential that the nurse caring for these infants have a good understanding o

Cardiac anomalies restricting blood flow to the left atrium.

This article continues the series describing the presentation and treatment of congenital cardiac disease in the neonatal period. Cardiac anomalies that restrict the return of blood to the left atrium are reviewed. These include total anomalous pulmonary venous connection (with emphasis on TAPVC with obstruction), mitral atresia and mitral stenosis, and pulmonary vein obstruction. Incidence, embryology, pathophysiology and hemodynamics, clinical presentation, differential diagnosis, and medical and nursing management, as well as surgical repair and prognosis, are discussed for each of these de

Sweating It Out: When Meconium Ileus Is Not Cystic Fibrosis-A Case Report.

Meconium ileus (MI) is the result of the accumulation of thick, dry, inspissated meconium that creates a bowel blockage, most commonly in the terminal ileum. These pockets of meconium prevent passage of stool beyond the point of obstruction, which leads to distention of the proximal bowel, bowel wall thickening, and distal microcolon. Occurring most commonly (90%) in conjunction with cystic fibrosis (CF), the occurrence of MI without CF is rare. The literature describes the incidence of MI associated with CF occurring in as many as 24.9% of those who have a pair (homozygous) of the most common

Hyperbilirubinemia in the healthy term newborn.

Jaundice is a common clinical problem in newborns. Although the need to diagnose and treat hyperbilirubinemia in the healthy term newborn has been controversial, recent reports of detrimental neurologic effects from elevated serum bilirubin levels in the healthy newborn make such scrutiny prudent. Until 1994, when the American Academy of Pediatrics developed guidelines for managing hyperbilirubinemia in the healthy term newborn, no standard of care was defined.

Nonimmune hydrops fetalis.

Hydrops fetalis is a relatively rare phenomenon that presents itself in the delivery room in an extremely acute manner. Prompt resuscitation, an understanding of the condition and its presentation, and immediate treatment can make the difference between life and death in these cases. The focus of this article is on nonimmune hydrops, the type of hydrops seen in the clinical setting. Maternal and fetal pathophysiology, current theories, diagnostic evaluations, neonatal pathophysiology, clinical manifestations, treatment options, pertinent research, and the needs of the family are discussed.
