Optic neuritis and multiple sclerosis. An immunological study.

On examination in the first weeks after onset of the disease, an antibody-dependent lymphocyte cytotoxicity (ADLC) against myelin basic protein (MBP) was demonstrated in 32 (71%) of 46 patients with ON/only and in 8 of 10 patients with ON/MS. The findings were positive in 21 out of 22 CSF samples. The intensities of the cytotoxicity in the serum and in the cerebrospinal fluid were largely in agreement. No correlation was found between the cytotoxicity in the CSF and the IgG content. The ADLC against MBP is highly specific for MS. Only 5% of 200 patients with other neurological complaints had p

Visual parameters associated with recovered retrobulbar optic neuritis.

Visual acuity, color vision, pupillary reaction, induced Pulfrich phenomenon, kinetic fields, static fields, afterimage testing, and ophthalmoscopic evaluation were studied in nine patients with a history of retrobulbar neuritis. The most consistently reliable test for determining the presence of an old optic nerve defect in these patients was meridional 0 to 180 degrees static perimetry. There was a uniform decrease in brightness discrimination to either side of the foveal peak.

Optic Neuritis As the Initial Presentation of Syphilis.

Syphilis is transmitted via sexual contact and spreads through the lymphatic and hematologic systems. Ocular syphilis is an uncommon yet serious manifestation of syphilis that can cause significant vision impairment. This report presents a case of ocular syphilis in a 41-year-old otherwise healthy woman who presented with left-sided blurry vision and ocular discomfort. Examination confirmed decreased visual acuity and optic disc swelling in the left eye. Magnetic resonance imaging findings suggested optic neuritis. After ruling out other causes of optic neuritis and considering the presentatio…

Unilateral optic neuritis as a presentation of neurobrucellosis.

Neurobrucellosis manifesting as optic neuritis is a rare disease in childhood. We report a case of neurobrucellosis in a 11 year old girl leading to visual impairment and headache. Physical examination revealed mild oedema of right tibiotarsic joint and optic neuritis. Investigations showed CSF pleocytosis and a Brucella serum agglutination titer of 1/640. Complete reversal of the symptoms was observed after appropriate antibiotic treatment. To our knowledge only four cases of neurobrucellosis manifesting with visual impairment in childhood are previously reported in literature.

New developments in the treatment of optic neuritis.

Acute optic neuritis (ON) has various etiologies. The most common presentation is inflammatory, demyelinating, idiopathic, or "typical" ON, which may be associated with multiple sclerosis. This must be differentiated from "atypical" causes of ON, which differ in their clinical presentation, natural history, management, and prognosis. Clinical "red flags" for an atypical cause of ON include absent or persistent pain, exudates and hemorrhages on fundoscopy, very severe, bilateral, or progressive visual loss, and failure to recover. In typical ON, steroids shorten the duration of the attack, but …

MRI of optic neuritis in a rat model.

Neuritis of the optic nerve is one of the most frequent early symptoms of multiple sclerosis. There are only scarce data correlating magnetic resonance imaging (MRI) contrast alterations with the underlying pathology, that is inflammation, demyelination, and axonal damage. Here we studied optic neuritis in a rat model of experimental autoimmune encephalomyelitis by comparing in vivo MRI findings from multiple techniques (T1, T2, proton density, magnetization transfer) to histopathology. We further assessed a breakdown of the blood-brain barrier by using Gd-DTPA and indirectly estimated the int…
