Orbital cellulitis.

Orbital cellulitis (OC) is an inflammatory process that involves the tissues located posterior to the orbital septum within the bony orbit, but the term generally is used to describe infectious inflammation. It manifests with erythema and edema of the eyelids, vision loss, fever, headache, proptosis, chemosis, and diplopia. OC usually originates from sinus infection, infection of the eyelids or face, and even hematogenous spread from distant locations. OC is an uncommon condition that can affect all age groups but is more frequent in the pediatric population. Morbidity and mortality associated…

Orbital reconstruction.

PURPOSE OF REVIEW: Post-traumatic orbital reconstruction is a complex issue that involves both the soft tissue and bony injury. The current literature focuses primarily on bony reconstruction, with fewer investigators evaluating soft-tissue injuries. This article will review recent advances in orbital reconstruction, including presurgical planning software, intra-operative navigation, patient-specific implants, and intra-operative imaging. RECENT FINDINGS: Traditional techniques for diagnosis and orbital injuries continue to be refined; however, advances in computer-aided surgery are allowing…

[Orbital inflammation].

Orbital inflammation is a generic term encompassing inflammatory pathologies affecting all structures within the orbit : anterior (involvement up to the posterior aspect of the globe), diffuse (involvement of intra- and/or extraconal fat), apical (involvement of the posterior orbit), myositis (involvement of only the extraocular muscles), dacryoadenitis (involvement of the lacrimal gland). We distinguish between specific inflammation and non-specific inflammation, commonly referred to as idiopathic inflammation. Specific orbital inflammation corresponds to a secondary localization of a "genera…

[Orbital cellulitis].

Orbital cellulitis is uncommon in ophthalmologic practice. The majority of cases arise from direct spread of sinus infection or eyelid infection. Clinically, orbital cellulitis is divided into two forms: the preseptal form, anterior to the orbital septum, and the retroseptal form, posterior to the orbital septum. Management and prognosis differ widely between the two types. The retroseptal form or "true" orbital cellulitis is a severe disease with potentially disastrous consequences for vision and survival. Clinical examination and urgent CT scanning are indispensable for correct diagnosis, ev…

[Orbital tumors].

Computed tomography (CT) and magnetic resonance imaging are the modalities of choice in the assessment of orbital neoplasms. Careful interpretation of the characteristic radiological features usually leads to the correct diagnosis; however, some of the lesions look very similar and are difficult to differentiate from each other. This contribution provides an overview of tumors of the eye and orbit and their appearance on CT and magnetic resonance imaging.

Orbital rhinosporidiosis.

Rhinospordiosis, a chronic granulomatous disease of the mucocutaneous tissue, is endemic in India and other parts of South East Asia. It commonly affects the upper respiratory tract, but involvement of other sites has also been reported. A 27-year-old male patient reported with features simulating severe obital cellulitis underwent emergency managemt for his systemic problems. Subsequent CT scan and histopathological examination confirmed the diagnosis of rhinoporidiois. Definitive surgical treatment was done to remove the suspected papillomatous mass which led to marked improvement of the pat

Orbital trauma.

Assessment of the orbit for orbital trauma is best achieved expeditiously with CT in the determination of extent of injury and the presence of foreign body. MR imaging has a limited role but is valuable in examining the optic nerve and globe for injury and has proven to be an adjunct modality in the assessment of orbital injury.

[Orbital myositis].

Orbital myositis is a subgroup of the nonspecific inflammatory syndrome or orbital pseudotumor and is characterized by a primary inflammation of extraocular muscles. The authors describe a 70-year-old patient with acute proptosis, ocular pain and right ophthalmoplegia, whose orbital computed tomographic scan showed enlargement of the homolateral extraocular muscles. Clinical presentation and complementary tests were compatible with the diagnosis of orbital myositis however, because of the particular aspects, which included retinal central vein occlusion, optic nerve lesion, distension of the s…

Orbital cysticercosis.

BACKGROUND: Human cysticercosis is secondary to an infestation by cysticercus cellulosae, the larval form of Taenia solium. Cysticercosis is endemic to regions with poor sanitation. The purpose of this report is to present a large series of patients with orbital cysticercosis and to discuss the current treatment. METHODS: A retrospective chart analysis of all patients with orbital cysticercosis from an urban practice in southern India was performed. The clinical features, the results of investigations, the therapies instituted, and the outcomes realized were recorded. RESULTS: Twenty patient…

[Orbital hematomas].

BACKGROUND: Orbital hematomas may occur spontaneously, as a result of vascular anomalities, or they may be induced by trauma or occur following paranasal sinus surgery. The retrobulbar hematoma requires special attention because of its potential compression of the optic nerve may compromise vision or cause blindness. PATIENTS AND METHODS: We report on four cases: two subperiostal orbital hematomas, a spontaneous retrobulbar hematoma, and one orbital hematoma due to trauma. RESULTS: In one case a vascular anomality was detected by angiography. Though temporary blindness occurred in this case,…

Orbital implants.

Several aspects of orbital implants are reviewed. Hydroxyapatite has become one of the most commonly used implants for anophthalmic patients and has many of the characteristics desirable in an implant, although reports of significant complication rates are noted. Recent editorials acknowledge the advancement that hydroxyapatite has given in treating anophthalmic patients but add a note of caution in its extensive use. It may be reasonable to limit the use of hydroxyapatite to patients who are highly motivated. Other recent advances in surgical technique, evaluation, and treatment of complicati…

Orbital-tuberculosis.

Three cases of orbital tuberculosis are presented, two as cold abscess in the orbit and one as a chronic indolent sinus. A simple clinical test like Fine Needle Aspiration Cytology (FNAC) can demonstrate some or all of the characteristic pathological features like caseating necrosis, epitheloid cells and Langhan's giant cells. Important corroborative evidence can be obtained from the Tuberculin test and a positive therapeutic trial. While there are many other causes or orbital swelling and abscess, tuberculosis should be considered an important differential diagnosis in endemic areas. Simple t…

Orbital hypertelorism.

Excessive distance between the orbits is only one manifestation of a complex deformity that affects several skeletal and soft-tissue structures. This article discusses the classification, preoperative planning, and surgical procedures used in the reconstruction of orbital hypertelorism.

Orbital imaging.

Appropriate axial or coronal computed x-ray tomography is the most inexpensive method to reliably supply detail in orbital disease. Magnetic resonance imaging can provide additional information and may be characteristic in some conditions. Magnetic resonance imaging is imperative for detailed imaging of disease at the orbital apex, optic canal, or chiasm. Color-coded Doppler ultrasonography allows examination of physiological blood flow within orbital vasculature and recent reports have reported the changes of flow in various orbital diseases. Limited results with computed tomographic or video…

Orbital cysticercosis.

Orbital cysticercosis is a rare condition. We report here 3 cases with orbital cysticercosis who presented with proptosis and ptosis (Case no. 1 and 2) and focal seizures (Case no. 3). All of them had a vision of 6/6. Diagnosis of cysticercosis was made on CT Scan. The lesions isolated in Cases 1 and 2 and were excised. Drug therapy was given to treat any persisting infestation. Case no 3 had multiple brain cysticerci in addition to the orbital one. However, the patient was lost to follow-up.
