Barriers to delivering trauma-focused interventions for people with psychosis and post-traumatic stress disorder: A qualitative study of health care professionals' views.

OBJECTIVES: Trauma-focused interventions have been shown to be effective treatments for post-traumatic stress disorder (PTSD), and clinical guidelines support their use with people with psychosis. Despite this, they are used relatively infrequently in this population. We sought to explore UK health care professionals' perceptions of what impedes or facilitates the use of trauma-focused interventions among people with psychosis and PTSD. DESIGN: A qualitative study using constructivist grounded theory methodology. METHODS: We conducted semi-structured interviews with 18 health care profession

A Moderated Mediation Analysis of New Work-Related Stressors, Psychological Distress, and Absenteeism in Health Care During a Pandemic: Is Recognition the Cure for Preventing Falling in Battle?

OBJECTIVES: We aimed to evaluate the effects of new work-related stressors on psychological distress and absenteeism and the role of recognition in these relationships. METHODS: Moderated path analyses were carried out on a sample of 1128 health care workers. RESULTS: Increased workload related to COVID-19 (coronavirus disease 2019) ( β = 1.511, P ≤ 0.01) and fear of COVID-19 ( β = 0.844, P ≤ 0.01) were directly associated with a higher level of psychological distress and indirectly ( β = 2.306, P ≤ 0.01; and β = 1.289, P ≤ 0.05, respectively) associated with a higher level of absenteeism. Health System Leadership for Psychological Health and Organizational Resilience During the COVID-19 Pandemic: Protocol for a Multimethod Study.

BACKGROUND: Since the World Health Organization declared COVID-19 a global pandemic, health systems and health system leaders have faced unprecedented challenges through the various stages of the crisis. Canada and other health systems were largely ill-prepared to handle this crisis. The longevity of the pandemic has profoundly affected health care systems and compounded the rates of negative psychological outcomes in health systems' leaders and staff, rates of emotional exhaustion, and burnout. OBJECTIVE: The purpose of this study is to investigate the experiences of health system leaders an

Organizational Climate and Decision Aid Sustainability in Lupus Care: Mixed Methods Study.

BACKGROUND: Digital decision aids (DAs) are increasingly used in health care to support shared decision-making and promote patient engagement. In the context of systemic lupus erythematosus (SLE), a complex autoimmune disease characterized by diverse symptoms and uncertain prognoses, DAs offer guidance to patients in navigating treatment risks and benefits. Although numerous studies have examined the initial implementation of evidence-based tools, there is limited evidence on the organizational factors that influence their long-term sustainability in clinical practice. This gap is particularly

[Focus on methods for detection of sentinel nodes in breast cancer].

The sentinel node procedure (GS) is the recommended technique for axillary surgical exploration in localized breast cancer with no clinical or radiological lymph node involvement. This surgical technique is based on a dual isotope and colorimetric detection. Although it allows a significant reduction in morbidity compared to axillary dissection (CA), this procedure induces a number of organizational constraints, in particular for the radioisotope injection. Specially for this reason, other GS methods have emerged in recent years, some of which appear promising (detection by fluorescence and ma

Champion-Led Deprescribing for Persons with Dementia in Primary Care: A Qualitative Study in Accountable Care Organizations.

BACKGROUND: Clinical champions are a known strategy for implementing evidence-based practices; however, their application in de-implementing potentially inappropriate medications (PIMs) among persons with dementia is underexplored. We conducted a pragmatic cluster-randomized clinical trial of a champion-led deprescribing intervention in accountable care organization (ACO) primary care settings. OBJECTIVE: To (1) understand clinical champions' perspectives of their deprescribing projects and (2) identify key contextual factors that influenced champions' deprescribing PIMs for persons with deme

Intersectional equity in Brazil's remote rural municipalities: the road to efficiency and effectiveness in local health systems.

OBJECTIVE: The Brazilian remote rurality has been classified more reliably only recently, according to demographic density, proportion of urban population, and accessibility to urban centers. It comprises 5.8% of the municipalities, in nearly half of the states, with a population of 3,524,597 (1.85%). Remote rural localities (RRL) have reduced political/economic power, facing greater distances and barriers. Most health strategies are developed with the urban space in mind. We aim to understand how RRL are positioned concerning efficiency/effectiveness in health, compared to other urban-rural t

When equal resources fail to produce equal opportunity: a critical narrative mini review of implementation equity in school-based physical activity for adolescents.

Schools are the most universal platform for promoting adolescent physical activity (PA), yet uniform mandates have not produced equal opportunities. This Mini Review is a critical narrative (integrative) review: its objective is conceptual integration across three literatures that have developed separately, participation disparities, implementation determinants, and PA policy analysis, rather than pooled effect estimation, and it is reported in line with SANRA. We searched PubMed and the Web of Science Core Collection for evidence published between January 2021 and June 2026, supplemented by s

A scoping review on community health care centers: Evidence in support of Italian policymakers to redesign the Italian primary care system.

BACKGROUND: The COVID-19 pandemic underscored the central role of primary health care in managing patients with complex and chronic health conditions. In response, the Italian government launched a reform of the National Health Service, introducing Community Health Care Centers ("Case della Comunità") as a key pillar of the new primary care model. Evidence is needed on which organizational features of such centers best support integrated, high-quality care. OBJECTIVE: This scoping review synthesized international evidence on community health care centers characterized by integrated care and c

Mapping ethical concerns in algorithm-driven period and fertility tracking technologies.

OBJECTIVE: The proliferation of algorithms in period and fertility tracking technologies has increased reliance on data and analytics to interpret menstrual cycle symptoms and guide health and fertility-related action. We set out to map the ethical concerns of the (often invisible) algorithmic influence on users' experience of, and behavior related to their menstrual cycle and fertility. STUDY DESIGN: Reviewing literature and media, we map six ethical concerns of algorithms in period and fertility tracking technologies - inconclusive evidence, inscrutable evidence, misguided evidence, unfair

Preventing and responding to child sexual abuse: Organizational efforts.

BACKGROUND: Child sexual abuse (CSA) remains an ongoing threat to the wellbeing of children who might be victimized, to the liberty of individuals who might engage in abusive behavior, and to the reputations and livelihood of organizations in which abuse might occur. In the U.S., millions of children participate in youth-serving organizations (YSOs), and it is known that a proportion of CSA occurs in these settings. Despite the severity of these threats, there is little knowledge of the steps that organizations take to prevent and respond to CSA. OBJECTIVE: Our study aimed to index current me
