CASE REPORT: A 63-year-old man presented with progressive left heel pain limiting weight bearing over 5 years with a history of probable nasal rhinosporidiosis (45 years ago). Examination showed calcaneal tenderness, and MR imaging revealed a posteroinferior calcaneal lytic lesion suggestive of osteomyelitis. Cultures were negative, but histopathology confirmed Rhinosporidium seeberi. Surgical debridement, local amphotericin B, and prolonged dapsone achieved excellent outcomes without recurrence at 18 months.
CONCLUSION: High suspicion and isolation of the organism are crucial for the treatme…

A woman in her 50s, with multiple comorbidities, underwent coronary artery bypass grafting and presented with a deep sternal wound infection (DSWI) a month later. Despite initial debridements, polymicrobial infection continued to develop and persist, leading to chronic sternal osteomyelitis which involved multiple costocartilages and the majority of her manubrium. Over a number of months, she underwent serial debridements, antibiotic treatment and investigations to manage her condition, with multidisciplinary collaboration helping resolve her infections. Successful closure and reconstruction o…

Subungual exostosis (SE) is a rare benign osteocartilaginous tumor, most often affecting the great toe. While complications such as nail deformity and recurrence are known, the coexistence of osteomyelitis within SE has not previously been reported. We describe three adolescent patients with SE complicated by osteomyelitis at the same anatomical site. Diagnosis was established by clinical examination, magnetic resonance imaging (MRI), and intraoperative bone cultures. All patients underwent surgical excision with debridement, application of intramedullary antibiotic-loaded bone void filler, an…

INTRODUCTION: Foot ulcer is one of the common problems in patients with diabetes mellitus. The spectrum of diabetic foot infection is wide, ranging from cellulitis and soft tissue infection to osteomyelitis (OM). OM requires further testing and investigations and even longer treatments. The aim of this prospective study was to assess the performance of serum markers, namely, the erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and procalcitonin (PCT) in the diagnosis and follow-up of patients with diabetic foot osteomyelitis (DFO) and to find correlation between serum levels of …

BACKGROUND: Pelvic involvement has been reported in 3%-14% of acute hematogenous osteomyelitis (AHO) cases in children. One guideline suggests need for a longer antibiotic course in pelvic AHO, however, recent data are lacking. We describe the clinical course of children with pelvic AHO and compare it to nonpelvic AHO.
METHODS: A retrospective review of patients with a diagnosis of AHO admitted to Texas Children's Hospital from January 2012 to December 2020 was conducted. Patients 6 months-<19 years old and with ≤14 days of symptoms at admission were eligible. Patients with sickle cell diseas…

Cervical cancer most commonly spreads hematogenously to the lungs, liver, and bone. However, it rarely metastasizes to the foot. There is only one other case of cervical cancer with metastasis to the foot. In addition, the initial imaging of metastatic disease has difficulty in differentiating from infectious or other inflammatory processes, particularly in a clinical setting highly suspicious of infectious sources. Here, we present a rare case of cervical cancer metastasizing to the calcaneus masquerading as osteomyelitis, highlighting the importance of diagnostic imaging in conjunction with …

The value of microbiology and histopathology in the diagnosis of neuropathic foot osteomyelitis remains poorly understood. In this retrospective cohort study, we evaluated the concordance of microbiology and histopathology results from bone resections and found similar proportions of bacterial growth in samples with and without histopathologic evidence of osteomyelitis.

BACKGROUND: Simple radiography in conjunction with pertinent medical history and a comprehensive physical examination is typically adequate for diagnosing chronic osteomyelitis (CO). However, radiographic manifestations of CO lack specificity; therefore, the concordance among specialists in this regard has not been systematically assessed. This study aimed to compare and evaluate the proficiency of orthopedic surgeons and radiologists in identifying radiographic indicators present in simple radiographs for diagnosing CO.
METHODS: This cross-sectional study was a correlational investigation ut…

We present a case of Tissierella bacteremia in the setting of polymicrobial osteomyelitis. Tissierella is a Gram-variable bacterium that has been rarely documented as the etiologic organism in human infections such as septic arthritis or otitis media, and even more rarely reported as an organism associated with bacteremia. The patient presented with septic shock and the physical exam was notable for gangrene of bilateral feet. Infectious workup was significant for acute osteomyelitis and septic arthritis of his feet in the setting of Tissierella bacteremia, which was successfully treated with …

Diabetic foot osteomyelitis (OM) requires a longer duration of therapy, a greater need for surgery and implies a higher rate of recidivism, a higher amputation risk, and lower treatment success. But do all bone infections behave the same way, require the same treatment, or imply the same prognosis? Actually, in clinical practice we can verify there are different clinical presentations of OM. The first one is that associated with the infected diabetic foot attack. It requires urgent surgery and debridement since "time is tissue." Clinical features and radiographs are enough for the diagnosis, a…

Tuberculosis (TB) is a pulmonary disease with potential extrapulmonary manifestations that is caused by the bacteria Mycobacterium tuberculosis. Despite advancements in treatment, TB remains a worldwide public health concern. TB osteomyelitis accounts for approximately 3-5% of all extrapulmonary TB cases. We present a case of humeral TB osteomyelitis in a 22-month-old female with no pulmonary or systemic symptoms. This case offers insight into the diagnosis and management of TB osteomyelitis. A 22-month-old previously healthy Haitian-American female presented with a one-month history of a palp…

We describe a case of osteomyelitis of the rib caused by methicillin-susceptible Staphylococcus aureus. The patient presented with a subtle, indolent course leading to a suspected 2-year delay in diagnosis. This case highlights that the diagnosis of rib osteomyelitis, which can readily mimic other diagnoses, such as costochondritis, intraabdominal infections, pneumonia, or malignancies warrants a high index of suspicion. Albeit rare, pediatricians should be aware of the possibility of rib osteomyelitis in healthy children to help ensure a prompt diagnosis and appropriate, timely management.

Tuberculosis is a communicable disease that is a major cause of ill health. It is one of the top ten causes of death worldwide and the leading cause of death from a single infectious agent. Its most common clinical presentation is pulmonary involvement. However, approximately 23-30% of tuberculosis patients have extrapulmonary symptoms. A rare (1%) clinical presentation of tuberculosis is foot and ankle infection. This is complicated by the fact that the diagnosis of osteoarticular tuberculosis is difficult. Our case was a 66-year-old multi-pathological pensioner, who, while working in the cou

Osteomyelitis (OM) is an infectious disease of the bone predominantly caused by the opportunistic bacterium Staphylococcus aureus (S. aureus). Typically established upon hematogenous spread of the pathogen to the musculoskeletal system or contamination of the bone after fracture or surgery, osteomyelitis has a complex pathogenesis with a critical involvement of both osteal and immune components. Colonization of the bone by S. aureus is traditionally proposed to induce functional inhibition and/or apoptosis of osteoblasts, alteration of the RANKL/OPG ratio in the bone microenvironment and activ…

Actinomycosis is caused by Actinomyces species and is relatively rare in humans. Because of the special collateral blood flow, osteomyelitis is less common in the maxilla than the mandible. Although there are few case reports for jaw osteomyelitis, actinomycotic osteomyelitis associated with phenytoin therapy has not been reported before. The data show that antiepileptic drugs induce suppression of the immune system. This report presents a rare case of a 58-year-old man on phenytoin with actinomycotic osteomyelitis, and reviews the relevant literature.
