Deep learning algorithm classification of tympanostomy tube images from a heterogenous pediatric population.

IMPORTANCE: The ability to augment routine post-operative tube check appointments with at-home digital otoscopes and deep learning AI could improve health care access as well as reduce financial and time burden on families. OBJECTIVE: Tympanostomy tube checks are necessary but are also burdensome to families and impact access to care for other children seeking otolaryngologic care. Telemedicine care would be ideal, but ear exams are limited. This study aimed to assess whether an artificial intelligence (AI) algorithm trained with images from an over-the-counter digital otoscope can accurately

Pediatric tympanostomy tube assessment via deep learning.

PURPOSE: Tympanostomy tube (TT) placement is the most frequently performed ambulatory surgery in children under 15. After the procedure it is recommended that patients follow up regularly for "tube checks" until TT extrusion. Such visits incur direct and indirect costs to families in the form of days off from work, copays, and travel expenses. This pilot study aims to compare the efficacy of tympanic membrane (TM) evaluation by an artificial intelligence algorithm with that of clinical staff for determining presence or absence of a tympanostomy tube within the TM. METHODS: Using a digital oto

Ear Fullness as a Symptom of Endolymphatic Hydrops in non-Ménière's Patient.

OBJECTIVE: 1) To determine if unexplained ear fullness might be a symptom of endolymphatic hydrops (EH) by using Electrocochleography (ECochG) SP/AP area and amplitude ratios. 2) To assess if individuals with unexplained ear fullness without vertigo differ significantly from individuals with ear fullness due to Ménière's disease (MD). MATERIALS AND METHODS: In a case-control study in our tertiary care center, we evaluated 62 ears across 49 patients, including 18 normal healthy ears across 12 control patients, 26 ears with unexplained ear fullness across 20 patients (6 had bilateral symptoms o

The use of telehealth services to facilitate audiological management for children: A scoping review and content analysis.

Approximately 32 million children globally present with disabling hearing loss. Despite evidence describing the negative consequences of hea … ncluded studies demonstrated improved access to and coverage of rural areas. Services such as video otoscopy and synchronous (online) hearing testing in remote areas were successfully implemented. Challenges included lack of diagnostic studies, inadequate training of staff and the need to standardize protocols and procedures in order to ensure that tele-au …

Hearing function after betahistine therapy in patients with Ménière's disease.

INTRODUCTION: Preventing or reversing hearing loss is challenging in Ménière's disease. Betahistine, as a histamine agonist, has been tried in controlling vertigo in patients with Ménière's disease, but its effectiveness on hearing problems is not known. OBJECTIVE: To examine the effect of betahistine on hearing function in not-previously-treated patients with Ménière's disease and to define possible contributors in this regard. METHODS: A total of 200 not-previously-treated patients with definite unilateral Ménière's disease received betahistine by mouth (initial dose, 16mg three times a da

Congenital aplasia of the external ear canal and total dehiscence of mastoid-tympanic segment of the facial nerve as a cause of recurrent facial paralysis.

Facial nerve (FN) macrodehiscence, in contrast to microdehiscence, generally occurs as a result of chronic otitis media and is rarely seen congenitally. A patient with normal hearing who had no history of ear disease or ear operation came to the clinic with recurrent facial paralysis (FP) and frequent blockage of the external ear canal with epithelial debris complaints. In the explorative ear surgery, we observed that the posterior wall of the external ear canal or tympanic ring was absent, a large external ear cavity was covered with a thin skin, the FN was under the skin, and the tympanic-ma

Selective epitympanic dysventilation syndrome.

OBJECTIVES/HYPOTHESIS: Although middle ear aeration is certainly related to eustachian tube (ET) function, other anatomic factors may play an important role in ventilation of these spaces, particularly the epitympanum. STUDY DESIGN: A prospective case series study group with retrospective case series comparison. METHODS: Eight patients affected by non-self-cleaning attic retraction pockets or attic cholesteatoma of the middle ear and normal tubal function (verified by type A tympanogram or Williams' test) underwent endoscopic surgery. Anatomic findings were noted and collected, with particul

Roller coaster-induced barotrauma.

We present the case of a 24-year-old male who presented to the otolaryngology clinic for otalgia and aural fullness after riding a roller coaster. We present his clinical course and outcome and briefly discuss the mechanism of injury and otologic barotrauma. Roller coasters have been reported as the cause of many different types of injuries, but this is the first reported case of otologic barotrauma. As the engineering for roller coasters continues to advance in terms of increasing speed, otolaryngologists need to be aware of a new etiology of otologic barotrauma.

Evaluation of plasma fibronectine level as a probable indicator for tympanosclerosis.

OBJECTIVES: Wound healing, epithelial regrowth and collagen synthesis are very important factors in the repair of the traumatised tympanic membrane. The aim of the present study was to determine the role of plasma fibronectine in the aetiopathogenesis of tympanosclerosis. METHODS: This prospective study included 58 patients with and 49 without tympanosclerosis. No inflammation or trauma was noted in either patient group. All patients underwent otoscopic and otomicroscopic examination, and the degree of tympanosclerosis was graded from mild (stage I) to severe (stage III). Following otological

Single-center 10-year experience in treating patients with vascular tinnitus: diagnostic approaches and treatment outcomes.

OBJECTIVES: Vascular tinnitus is the most common form of pulsatile tinnitus, particularly when the tinnitus corresponds with the pulse of patients. In this study, we reviewed the 10-year clinical data on vascular tinnitus of our tinnitus clinic to investigate the frequency of the underlying etiologies, to introduce a diagnostic protocol, and to evaluate the treatment outcomes. METHODS: We retrospectively collected the data of 57 patients who were diagnosed as vascular tinnitus between April 2001 and December 2011. Careful history taking, otoscopy, thorough physical examinations, audiometry, l…

When the simple migrated hair results in distressing ear symptoms.

We present 3 patients who complained of distorted hearing, tinnitus and otalgia resulting from scalp and facial hair impacting against the tympanic membrane. Removal of the migrated hair with microsuction relieved symptoms immediately. In 2 cases, the patients had presented to the primary care physician earlier, shortly after a haircut. The family physicians had correctly detected the migrated hair but failed to identify this as the cause of the patient's symptoms. Increased awareness of this potentially common complication will enable the family physician to identify and treat these patients
