Development of plasma ghrelin level as a novel marker for gastric mucosal atrophy after Helicobacter pylori eradication.

BACKGROUND AND AIM: The severity of atrophic gastritis is significantly associated with the risk of gastric cancer. Although the current gold standard for assessing the gastric cancer risk is esophagogastroduodenoscopy with a pathological examination, the development of less-invasive biomarkers is warranted for efficient risk stratification of gastric cancer. Serum pepsinogens (PGs) are biomarkers used to predict the extent of gastric mucosal atrophy; however, they are not an accurate reflection of gastric mucosal atrophy after Helicobacter pylori eradication. The present study was conducted t…

Chronic atrophic gastritis and metachronous gastric cancer in Japanese alcoholic men with esophageal squamous cell carcinoma.

BACKGROUND: The risk of metachronous gastric cancer is high in Japanese with esophageal squamous cell carcinoma (SCC), especially in alcoholic men, suggesting a common background underlying the gastric and esophageal cancers. METHODS: Endoscopic follow-up ranging from 7 to 160 months (median, 47 months) after the initial diagnosis was performed in 99 Japanese gastric-cancer-free alcoholic men (56.8 +/- 6.4 years) with esophageal SCC detected by an endoscopic screening examination. Chronic atrophic gastritis (CAG) assessed by the serum pepsinogen test and Helicobacter pylori status was compare

Unhealed duodenal ulcers despite Helicobacter pylori eradication.

BACKGROUND: Our aims were 1) to study the influence of several factors (age, sex, smoking, previous ulcer disease, ulcer size, chronic gastritis, serum gastrin and pepsinogen I levels, therapy regimen and, especially, eradication of Helicobacter pylori) on duodenal ulcer healing; 2) to evaluate the frequency of duodenal ulcer healing failure despite eradication of H. pylori, to study why this failure occurs, and to verify its evolution without antisecretory therapy; and 3) to confirm whether a week's prescription of omeprazole is sufficient to obtain ulcer healing. METHODS: Three-hundred and

[Current situation and future of gastric cancer screening examinations: from the viewpoint of gastric X-ray screening].

In Japan, screening examinations for gastric cancer have been conducted for 50 years because of the high prevalence of this type of cancer. Gastric X-ray screening examinations have contributed to the detection early gastric cancer. In 2001, Hisamichi et al. reported that X-ray examination is a valid technique in gastric cancer screening. However, they indicated that problems in the uncertainty of accuracy and the risk of radiation exposure at a young age. On the other hand, endoscopic screening examination, pepsinogen-method and H. pylori method are not acceptable because of incident risk. Ou

[Effect of Helicobacter pylori infection on the apoptotic and cell proliferative processes of the gastric mucosa].

The role of the Helicobacter pylori in the induction of gastric carcinogenesis seems to be certain. The exact pathomechanism is not yet known, although the special genetic conditions both of the microbe and the host just as the imbalance of the apoptotic and proliferative processes caused by the chronic inflammation are assumed to be involved. The latter one can be proved by the use of different cell proliferative and apoptotic markers. The results are inconsistent whether the eradication therapy causes the regression of premalignant atrophic gastritis. The regression of the MALT-associated ly

Generation of gastric proton pump atp4a knockouts in Astyanax mexicanus: a fish model for insights into the mechanisms of acidification by oxynticopeptic cells.

The gastric proton pump H+/K+-ATPase (HKA) is the highly conserved acid secretory mechanism of the gnathostome stomach. HKA is a heterodimeric pump composed of α and β-subunits. In this study, we have explored the involvement of this enzyme in the transcriptional regulation of pathways linked to acid secretion (Cl- and K+ movement across the cell membrane) and peptic digestion (pepsinogens) in the stomach of the teleost Astyanax mexicanus. To this end, we generated the first nonmammalian knockout line for the gastric proton pump, atp4a (HKA-α-subunit), in A. mexicanus using CRISPR-Cas9 gene ed…

Investigation of c-Fos/c-Jun Signaling Pathways in Periostracum Cicadae's Inhibition of EMT in Gastric Tissue.

Background/Objectives: Periostracum Cicadae (PC) is commonly used to treat chronic atrophic gastritis (CAG), but its underlying mechanisms are unclear. We investigated the therapeutic effects, active ingredients and molecular mechanisms of PC on CAG. Methods: We analyzed the components in the serum extract of PC by UHPLC-Q-Orbitrap-MS/MS. Then, we used rat and cell models to assess the impact of PC on CAG and employed network pharmacology and bioinformatics to predict key targets and active ingredients. Finally, we confirmed hub targets through experiments and molecular docking. Results: A tot

Proton Pump Inhibitor Challenge to Confirm Diagnosis of Atrophic Gastritis of the Stomach: A Proposal.

BACKGROUND AND AIMS: Chronic atrophic gastritis (CAG) is a known precancerous condition that can lead to the development of gastric cancer (GC). Low serum pepsinogen (PG) I levels have been proposed as a non-invasive marker for chronic atrophic gastritis of the stomach body, but the adequate upper cut-off for diagnosis remains controversial, as values ranging from 30 to 50 mcg/L are currently considered as a "grey zone". We aimed to identify patients with chronic atrophic gastritis (CAG) of the stomach body amongst subjects with PG-I levels ranging between 30 and 50 mcg/L by means of a proton

A randomized controlled trial for chemoprevention of gastric cancer in high-risk Japanese population; study design, feasibility and protocol modification.

We have initiated a population-based, double-blind, randomized controlled trial to examine the effects of supplementation of beta-carotene and vitamin C on the incidence of gastric cancer. The subjects were participants in an annual health screening program conducted by four municipalities in Akita prefecture, one of the regions with the highest mortality from gastric cancer in Japan. We measured their serum levels of pepsinogens (PGs) I and II, and asked persons diagnosed with chronic atrophic gastritis (defined as PG I < 70 ng/ml and PG I/PG II ratio < 3.0) to take diet supplements containin…

Helicobacter pylori infection, chronic atrophic gastritis and major cardiovascular events: a population-based cohort study.

OBJECTIVE: There is debate whether infection with Helicobacter (H.) pylori, the main inducer of chronic atrophic gastritis (CAG), is a risk factor for cardiovascular disease and premature mortality. METHODS: Serological measurements of H. pylori infection and pepsinogen (PG) I and II were obtained in a population-based German cohort of 9953 older adults (50-74 years). Cox regression was employed to estimate hazard ratios (HR) and 95% confidence intervals (CI) for myocardial infarction, stroke, cardiovascular and all-cause mortality during five-year follow-up. RESULTS: According to serology,

[Aggressive factors in the physiopathology of peptic ulcer. Recent findings].

The peptic ulcer (PU) is characterized by definite gastric cyto-secretory profiles. In case of duodenal ulcer (DU) and pre-pyloric gastric ulcer (PPGU), there is a prevalence of hyperparietalism with hyperchloridria, while in ulcer with body-fundic localization (BFGU) normo-hypoparietalism with normo-hypochloridria prevails. As well, the total peptic activity follows a superimposable course: it increases in cases of DU and PPGU, while it remains in the normal range in case of BFGU. With reference to the qualitative variations of pepsin, in course of PU the amount of pepsin 1 increases signific

Characteristics of teen-age patients with juvenile duodenal ulcer. Relation between inherited hyperpepsinogenemia I and duodenal ulcer.

We analyzed environmental factors, family history of peptic ulcer, gastric acid secretion, and serum levels of pepsinogen I (PG I) and gastrin in 56 juvenile patients with duodenal ulcer and 39 normal teenage subjects. Basal acid output and maximal acid output were significantly higher in our duodenal ulcer patients than in controls without ulcer (both, p < 0.01), and patients with duodenal ulcer showed significantly higher serum levels of PG I and gastrin than the controls (both, p < 0.001). There were no significant differences in any environmental factor between the patients and controls. F

Teprenone, but not H2-receptor blocker or sucralfate, suppresses corpus Helicobacter pylori colonization and gastritis in humans: teprenone inhibition of H. pylori-induced interleukin-8 in MKN28 gastric epithelial cell lines.

BACKGROUND: The role of teprenone in Helicobacter pylori-associated gastritis has yet to be determined. To investigate the effect of teprenone on inflammatory cell infiltration, and on H. pylori colonization of the gastric mucosa in H. pylori-infected patients, we first compared the effect of teprenone with that of both histamine H2 receptor antagonists (H2-RA) and sucralfate on the histological scores of H. pylori gastritis. We then examined its in vitro effect on H. pylori-induced interleukin (IL)-8 production in MKN28 gastric epithelial cells. MATERIALS AND METHODS: A total of 68 patients
