[Pathophysiological mechanism of the developing radicular cyst of the jaw].

The radicular cysts are result of inflammatory process in the periapical tissues associated with necrotic and infected pulps. Humoral and cellular immune responses play a central role in the pathogenesis of these lesions. The most important role in the growth of these lesion have proinflammatore cytokine TNF-alpha, IL-1 and IL-6. Cytokine can be secreted by macrophages, monocytes and other cells of the immune system and can participate in skeletal homeostasis including osteoclastic formation, and bone resorption in maxillofacial region. The aim of this study is to give a consise rewiew for mec…

Calcium hydroxide and a corticosteroid-antibiotic association as dressings in cases of biopulpectomy. A comparative study in dogs' teeth.

The subject of this paper was to study the behavior of the periapical tissues of dogs' teeth after biopulpectomy and dressing with calcium hydroxide or a corticosteroid-antibiotic association, before root canal filling with zinc oxide eugenol (ZOE) or Sealapex sealers. The teeth were overinstrumented and dressed for 7 days before the root canal filling. The animals were sacrificed 180 days after treatment and the specimens were prepared for morphological analysis. Specimens treated with Sealapex presented a higher number of cases with biological closure than ZOE. When the root canals were fill…

The Diagnostic Dilemma of an Odontogenic Jaw Lesion in a Pediatric Patient: A Case Report.

This paper presents a case report of a 10-year-old child patient reported with the chief complaint of a painless, hard swelling in the lower right back region of the jaw. The clinical and radiographic examination, including intraoral periapical radiograph (IOPA) and cone-beam CT (CBCT), was performed. Conservative treatment was planned based on the clinical and radiological diagnosis of the cyst. But the histological examination revealed unicystic ameloblastoma (UA). This clinical case of UA, which was misdiagnosed as a combination of a radicular cyst and a dentigerous cyst, is being presented

Morphological Assessment of the Effect of Nanosecond Diode Laser Radiation With a Wavelength of 1265 nm on Periodontal Tissue in the Treatment of Apical Periodontitis: An Experimental Study.

Introduction: There is a morphological assessment of the damaged area and the inflammatory response of periodontal tissues after therapy. It seems relevant to evaluate the reaction of the periapical tissues of the teeth of experimental animals after the treatment of simulated apical periodontitis using laser photodynamic therapy (PDT) and non-pigment laser transcanal microablation using morphological research. Methods: The lower incisors of 15 rabbits were used to simulate acute apical periodontitis of pulpal origin. Subsequently, the traditional treatment of teeth canals using mechanical clea…

Periapical bone response to bacterial lipopolysaccharide is shifted upon cyclooxygenase blockage.

OBJECTIVES: Infection, inflammation and bone resorption are closely related events in apical periodontitis development. Therefore, we sought to investigate the role of cyclooxygenase (COX) in osteoclastogenesis and bone metabolism signaling in periapical bone tissue after bacterial lipopolysaccharide (LPS) inoculation into root canals. METHODOLOGY: Seventy two C57BL/6 mice had the root canals of the first molars inoculated with a solution containing LPS from E. coli (1.0 mg/mL) and received selective (celecoxib) or non-selective (indomethacin) COX-2 inhibitor. After 7, 14, 21 and 28 days the

Inferior alveolar nerve paresthesia after overfilling of endodontic sealer into the mandibular canal.

The present study describes a case of endodontic sealer (AH Plus) penetration within and along the mandibular canal from the periapical zone of a lower second molar after endodontic treatment. The clinical manifestations comprised anesthesia of the left side of the lower lip, paresthesia and anesthesia of the gums in the third quadrant, and paresthesia and anesthesia of the left mental nerve, appearing immediately after endodontic treatment. The paresthesia and anesthesia of the lip and gums were seen to decrease, but the mental nerve paresthesia and anesthesia persisted after 3.5 years. This

Histopathological observations of periapical repair in teeth with radiolucent areas submitted to two different methods of root canal treatment.

Dogs' teeth with induced chronic periapical periodontitis were treated endodontically by two different methods, and the results were compared. A total of 40 root canals from the upper and lower premolars of two dogs were prepared chemomechanically. In method 1, a high-concentration (5.25%) hypochlorite solution was used during the instrumentation of the root canal, and an antibacterial dressing was applied between sessions, followed by filling of the root canal. In method 2, a low-concentration (0.5%) sodium hypochlorite solution was used as an adjunct to mechanical debridement, and the root c

Nonsurgical Management of Horizontal Root Fracture with Necrotic Apical Segment and Coronal Cervical Abfraction using Mineral Trioxide Aggregate: Case Report.

Horizontal root fractures are relatively rare dental findings and often pose diagnostic and treatment challenges. In addition to a horizontal root fracture, this case report also demonstrates a finding of a noncarious cervical abfraction lesion, which involves the loss of dental hard tissue from the cervical areas of teeth through processes unrelated to caries. A 22 -year-old healthy female patient was complaining of the maxillary left first premolar area with a history of dental trauma. Clinically, the tooth presented with a noncarious cervical abfraction lesion. Endodontic testing showed a n

Direct Clinical Evidence of Long-term Hard-tissue Repair after Miniature/Tampon Pulpotomy: An Accidental Re-entry Case after Six Years.

Vital pulp therapy (VPT) has become an established biologically-based treatment for preserving pulp vitality in mature permanent teeth with irreversible pulpitis. Although long-term clinical success has been widely reported, direct clinical evidence demonstrating the biological outcome beneath the pulp-capping material years after treatment is exceptionally rare because successfully treated teeth seldom require re-entry. A mature mandibular first molar diagnosed with symptomatic irreversible pulpitis associated with symptomatic apical periodontitis underwent a miniature pulpotomy. Because conv

Bone remodelling after minimally invasive surgical management of a recurrent florid cemento-osseous dysplasia in a Caucasian woman - 18 years follow-up of a unique case - A case report.

INTRODUCTION AND IMPORTANCE: Florid cemento-osseous dysplasia (FCOD) is a multifocal fibro-osseous tumour originating from the periodontal ligament that presents as being rare, benign and slow-growing. The lesion is characterised by the replacement of regular bone-structure with fibrous tissue and dysplastic bone. Depending on localisation, the initial characteristics of FCOD resemble those of periapical lesions of inflammatory origin. CASE PRESENTATION: We report on the current findings relating to the case of a 39-year-old Caucasian woman, who initially had persistent paresthesia of the rig

Production of interleukin-1 by polymorphonuclear leukocytes resident in periradicular tissue.

Twenty-one patients undergoing endodontic surgery were identified. Periradicular tissue samples were recovered, and those showing significant numbers of polymorphonuclear leukocyte (PMN) infiltration were prepared for immunoperoxidase identification of interleukin (IL)-1 alpha and IL-1 beta-producing cells using specific polyclonal antibodies. In selected tissue specimens, 90% or more of the PMN's were found to stain positively for IL-1 alpha and IL-1 beta. In addition, significant numbers of plasma cells and tissue histiocytes stained positively for these IL's. Cell suspensions from selected

Dental Mesenchymal Stem Cells in Inflamed Microenvironment: Potentials and Challenges for Regeneration.

Adult dental mesenchymal stem cells (DMSCs) are multi-potent stem cells that are involved in dental tissue repair and regeneration. DMSCs are able to differentiate into multiple lineages, including odontogenic, osteogenic, neurogenic, adipogenic, chondrogenic, hepatogenic lineages and insulinproducing cells. However, the DMSCs from functioning, impacted or exfoliated teeth may not be available when needed. Recently, DMSCs have been found in pulpal, periapical and periodontal tissue with inflammation from deciduous and immature/mature permanent teeth. DMSCs from inflamed tissue (iDMSCs) possess

Regenerative Endodontic Treatment of a Maxillary Mature Premolar.

Regenerative endodontic treatment was performed on a mature maxillary premolar diagnosed as chronic pulpitis. The root canals were chemomechanically prepared and placed intracanal medicaments at the first appointment. Then 2 weeks later, a blood clot was created in the canals, over which mineral trioxide aggregate was placed. At 6-month follow-up, cementum-like tissue seemed to be formed in the root canal along with nearly recovered pulp vitality. At 12-month recall, the radiographic results revealed evidence of root wall thickening. At 30-month recall, no periapical lesion was found. This cas
