Delivery of perinatal care in India: priorities and policies.

Delivery of appropriate perinatal care at primary, secondary and tertiary levels is becoming an urgent and important issue in countries of t … lmonary conditions, cerebral birth trauma and neonatal infections as the leading necropsy causes of perinatal death. Several studies have also documented low birth weight as a major contributory factor in perinatal and neonatal mortality. In India a large proportion of perinatal deaths are preventable. High fetal and childhood mortality adversely affect the accep …

The effect of stay in a maternity waiting home on perinatal mortality in rural Zimbabwe.

A hospital-based cohort study was carried out in a district hospital in Zimbabwe to evaluate the effect of a maternity waiting home on perinatal mortality. Information on antenatal risk factors, use of antenatal care, access to the hospital and stage of labour on arrival was collected for each woman delivering at the hospital during the period 1989-1991 (n = 6438). Women who stayed in the maternity waiting home had a lower risk of perinatal death compared to women who came directly from home to the hospital during labour. The crude relative risk of perinatal death for the women coming from hom…

Pentoxifylline Alleviates Perinatal Hypoxic-Ischemia-Induced Short-term Memory Impairment by Suppressing Apoptosis in the Hippocampus of Rat Pups.

PURPOSE: Perinatal hypoxic-ischemic brain damage is a major cause of acute mortality and chronic neurologic morbidity in infants and children. We investigated the effects of pentoxifylline, a methylxanthine derivative and type-4 phosphodiesterase inhibitor, on short-term memory and apoptotic neuronal cell death in the hippocampus following perinatal hypoxic-ischemia in newborn rats. METHODS: We used a step-down avoidance task to evaluate short-term memory and 3ʹ-5ʹ-cyclic adenosine monophosphate (cAMP) assay to detect cAMP levels. We evaluated apoptosis using a terminal deoxynucleotidyl trans

Obstetric practice and outcome of pregnancy in Cardiff residents 1965-73.

Trends in management and outcome of pregnancy in Cardiff residents from 1965 to 1973 were reviewed. The mean age and parity of parturients fell. Hospital delivery became almost universal, monitoring the fetus during pregnancy was introduced, and induction and acceleration of labour became commonplace. These developments were not associated with any striking change in either the total perinatal death rate or the timing or cause of perinatal death. Possibly a real change in perinatal mortality between 1965 and 1973 was masked by random fluctuation of small numbers, or possibly factors peculiar t…

Gestational age-specific perinatal mortality rates for assisted reproductive technology (ART) and other births.

Primary outcome measures were rates of stillbirth, neonatal and perinatal deaths. Adjusted odds ratio (AOR) and 95% confidence interval (CI) were used to estimate the likelihood of perinatal death. Perinatal mortality rate in a south Indian population.

Perinatal mortality rate was assessed for 13964 consecutive births in SAT Hospital, Trivandrum, South India, during a period of one year. The overall perinatal mortality rate was 42.75, stillbirth rate 24.41 and early neonatal mortality rate 18.79. The perinatal mortality rate in multiple pregnancy was 156.65. Preventable causes of perinatal mortality still make a major contribution to perinatal deaths in developing countries.

Results. There were 322 perinatal deaths resulting in a perinatal mortality rate (PMR) of 290/1000 total births. Intrauterine death rate in gastroschisis following the introduction of an antenatal surveillance program: Retrospective observational study.

AIM: To investigate whether an antenatal surveillance protocol including ultrasound and cardiotocograph monitoring reduces intrauterine death (IUD) in cases of gastroschisis. Secondary outcomes included neonatal death rate, mode of delivery and rate of intervention before planned time of delivery. METHODS: This was a retrospective observational study of all women with antenatally diagnosed gastroschisis who were managed according to the surveillance program between 2002 and 2015 in a tertiary fetal medicine and pediatric surgical center covering the Wessex region of England. Population density and perinatal mortality in Norway and Sweden 1975-1988.

Data from the medical birth registries in Norway and Sweden were used to study geographical variations in perinatal mortality in the two countries. The study population comprised 1.4 million single births during 1975-79 and 1985-88. Perinatal death (n=9,834) for infants with a birthweight of 1,000 g or more was the main measure of outcome. In both periods and in both countries, perinatal mortality was inversely associated with population density and annual number of births, with relative risks amounting to 1.4 in Norway and 1.2 in Sweden. Among live births, these gradients were observed in dea…

Perinatal mortality in twin pregnancies: implications for clinical management.

Perinatal deaths were studied among 374 consecutive twin births at a large medical center. The perinatal death rate per 1,000 births was 112.3 for twins compared with 15 for singletons. Neonatal deaths were twice as frequent as fetal deaths and were most often the result of prematurity. Recommendations to improve twin survival include early recognition of twin pregnancy, bed rest and a nutritious diet for the mother, and prompt recognition and management of complications, especially premature labor.

Bereavement and maternal mental disorders during the perinatal period: a nationwide register-based study in Sweden.

Background: Perinatal mental disorders are common, yet few robust risk factors are known. Stressful life events such as loss of a close family member due to death could serve as potential clinical predictors for which mothers develop perinatal mental disorders. This study investigated the association between bereavement and the risk of first-onset perinatal mental disorders.Method: This nationwide register-based study included 1,986,490 pregnancies in Sweden (1,097,126 mothers). The exposure was death of a family member (sibling, parent, partner or child) occurring any time before pregnancy an

Sonographic markers associated with adverse neonatal outcomes among fetuses with gastroschisis: an 11-year, single-center review.

BACKGROUND: Gastroschisis complicates 1 in 2000 births and is readily identifiable during prenatal ultrasound scans. Outcomes in fetuses that are affected by gastroschisis vary widely from stillbirth or neonatal death to uncomplicated surgical correction, which makes prenatal counseling challenging. OBJECTIVE: The goal of our study was to identify sonographic markers that are associated with perinatal death and morbidity that include significant bowel injury, necrotizing enterocolitis, and the need for bowel resection in fetuses with gastroschisis. STUDY DESIGN: We identified a cohort of fet…

Predicting the impact of in vitro fertilisation and other forms of assisted conception on perinatal and infant mortality in England and Wales: examining the role of multiplicity.

The increased risk of perinatal and infant mortality observed among in vitro fertilisation (IVF) births and other assisted conception births is thought to be largely attributable to multiplicity. Using mortality statistics and estimates of the proportion of births following infertility treatment, we predicted the excess stillbirths and infant deaths associated with twins and higher order births resulting from assisted conception in England and Wales. According to our results, approximately 73 deaths could have been avoided in 2001 if all IVF infants had been born as singletons or as naturally

Perinatal mortality rates: adjusting for risk factor profile is essential.

Perinatal mortality has long been used as a comparative measure of health and health care across regions, countries and over time. Recently, … l trends. We also show the effect of standardising for these factors on the most frequent causes of perinatal death. We recommend the construction of a 'standard birth population' for calculating standardised perinatal mortality rates that would improve direct comparisons between populations.
