Periodic Leg Movements During Sleep.

Periodic leg movements during sleep (PLMS) are a frequent finding in nocturnal sleep registrations that include tibialis anterior electromyographic signals. Different PLMS scoring rules exist and can have a major impact on PLMS frequency, which tends to be underappreciated. There is no consistent evidence that frequent PLMS are a causal risk factor for clinically significant outcomes. Several critical open questions are identified that need to be addressed, including but not limited to the consideration of the full range of all sleep-related leg movement activity.

Opsoclonus myoclonus syndrome in a postpartum period.

BACKGROUND: Opsoclonus-myoclonus syndrome (OMS) is a rare neuroimmunologic disorder characterized by opsoclonus, myoclonic jerks mostly in the face and limbs, cerebellar ataxia, tremors, and encephalopathy. OMS is rare in adults and exceedingly rarer in pregnancy, as only a few cases in pregnancy have been reported. We present what we understand is the first case of postpartum OMS. METHODS AND RESULTS: We report and discuss a challenging case of OMS which started 6 weeks postpartum. Despite extensive infectious and malignancy evaluation, an underlying etiology was not readily apparent thus we

Locking and regularization of chimeras by periodic forcing.

We study how a chimera state in a one-dimensional medium of nonlocally coupled oscillators responds to a homogeneous in space periodic in time external force. On a macroscopic level, where a chimera can be considered as an oscillating object, forcing leads to entrainment of the chimera's basic frequency inside an Arnold tongue. On a mesoscopic level, where a chimera can be viewed as an inhomogeneous, stationary, or nonstationary pattern, strong forcing can lead to regularization of an unstationary chimera. On a microscopic level of the dynamics of individual oscillators, forcing outside of the

Thyrotoxic Periodic Paralysis: An Incidental Diagnosis!

Thyrotoxic periodic paralysis is a rare presentation of thyrotoxicosis where the patient develops a transient motor deficit secondary to acute hypokalemia. The thyroid hormone augments gene transcription and post-transcriptional modification of Na-K ATPase, a cell membrane protein that regulates the electrical potential of the cell. Na-K ATPase increases active transport of potassium (K+) ions into the intracellular compartment causing hypokalemia without total body potassium deficit. Severe hypokalemia affects depolarization of the muscle cell membrane, clinically evidenced as paralysis. Othe

A Literature Review on Thyrotoxic Periodic Paralysis.

Thyrotoxic periodic paralysis (TPP) is a rare disease of the muscles that presents with painless weakness of the muscles. The patients usually have hypokalemia and hyperthyroidism with elevations in the level of triiodothyronine (T3) and thyroxine (T4). The muscle weakness is usually transient, and the patients in many cases suffer from recurrent episodes of muscle paralysis. This flaccid muscle paralysis predominantly affects the proximal and lower extremities group of muscles more than the distal and upper extremity muscles. This condition is one of the drastic complications of Graves's dise

Effect of Hyperventilation on Periodic Repolarization Dynamics.

Heart and lung functions are closely connected, and the interaction is mediated by the autonomic nervous system. Hyperventilation has been demonstrated to especially activate its sympathetic branch. However, there is still a lack of methods to assess autonomic activity within this cardiorespiratory coupling. Periodic repolarization dynamics (PRD) is an ECG-based biomarker mirroring the effect of efferent cardiac sympathetic activity on the ventricular myocardium. Its calculation is based on beat-to-beat variations of the T wave vector (dT°). In the present study, we investigated the effects of

Musculus peroneus longus in foetal period.

BACKGROUND: The lateral compartment of the leg, due to its distal and concurrent superficial positioning, is a multiple trauma site. Detailed knowledge of compartimentum lateralis cruris (CLC) structure is crucial for physicians. Musculus peroneus longus (MPL) is located within the structures of the CLC most superficially. There is a lot of data on the morphology of the MPL but there is no publication analysing in detail its anatomy in the foetal period. The aim of the study was to determine the variability of metric and morphological parameters of MPL in a studied period of prenatal ontogenes

The honeymoon period after mass vaccination.

Vaccination is an effective method to prevent individuals from contracting diseases like measles and the flu. Its success is clearly demonstrated by the large declines in the incidence of many diseases (e.g., childhood diseases like measles) after the start of mass vaccination programs. However, what happens after this drop in incidence can be complicated. It is known that some diseases exhibit "honeymoon periods" (long periods of temporary low disease incidence after start of mass vaccination). These periods end with a natural resurgence of the disease, which is not due to any change in the s

[A case of ileosigmoid knotting in the postpartum period].

Ileosigmoid knotting is a rare cause of intestinal obstruction. We report a case of ileosigmoid knotting occurred during the postpartum period. The diagnosis was made by means of CT scan data and confirmed intraoperatively.

MeCP2: an epigenetic regulator of critical periods.

Complex adult behaviors arise from the integration of sequential and often overlapping critical periods (CPs) early in life and adolescence. These processes rely on a subtle interplay between the set of genes inherited from the parents, the surrounding environment and epigenetic regulation. Methyl-CpG-binding protein 2 (MeCP2) has been shown to recognize epigenetic states and regulate gene expression by reading methylated DNA. Here, we will review the recent findings revealing the role of MeCP2 during postnatal CPs of development using mouse models of Rett (RTT) syndrome.

Thyrotoxic Periodic Paralysis: A Spine Consultation.

As a consultant, the orthopaedic spine surgeon is often asked to evaluate patients with acute-onset extremity weakness. In some cases, patient's deficits can be attributed to nonspinal pathology; therefore, it is important to be aware of nonorthopaedic diagnoses when evaluating these patients. We report a case of thyrotoxic periodic paralysis that was initially confused by the consulting service with spinal pathology. A 32-year-old Hispanic man presented to our emergency department with rapid onset of lower extremity weakness. The consulting team ordered CT of the cervical and lumbar spine, as

Effects of vibration on cutaneous silent period.

Suppression of an ongoing muscle contraction following noxious digital stimulation is called cutaneous silent period (CSP) which is under the influence of several physiological factors. In this study, we aimed to evaluate the influence of group Ia afferents on the cutaneous silent period (CSP) by applying 2-min vibration. CSP was obtained from abductor pollicis brevis muscle after stimulating index finger. The recordings were repeated three times-before, during and after vibration-which was applied over the tendon of flexor carpi radialis muscle. Onset latency, duration and magnitude of total

Listeriosis during the period 2003-2016.

Listeria monocytogenes can cause invasive disease in immunocompromised patients. We have retrospectively reviewed the clinical characteristics, treatment and evolution of the cases of listeriosis registered at our center during a long period of 14 years. Listeria monocytogenes infection is potentially serious with high mortality. Therefore, a high index of suspicion and early directed antibiotic treatment are needed, especially for immunocompromised patients.

William Osler and the "fixed period" of creativity.

In 1905, William Osler was the pre-eminent physician in American medical circles but was unknown to the general public. The latter suddenly learned of him through damning newspaper accounts of his address announcing his retirement from the Johns Hopkins Medical School. In it Osler mentioned two "fixed ideas" he held-(1) that most major advances in civilization have been made by men under age 40 (the "fixed period") and (2) that those over 60 should retire because they create little of significance and sometimes stifled the initiatives of younger colleagues. He highlighted the second idea with
