[Septic temperatures in questionable ascites].

The case history of a 40 year old male with septical temperatures and ascites is presented. Because of skin-pigmentation, hypotension, low rate of electrolytes, augmented renin-activity, and septical temperatures there was a suspicion of Addison's disease. A protein-rich ascites without tumor cells and erythrocytes, but with lymphocytes gives the diagnosis of peritonitis tuberculosa. Addison's disease and peritonitis tuberculosa are late manifestations of an organ tuberculosis, about 20 years after pulmonal tuberculosis.

Intraperitoneal tuberculous abscess: Computed tomography features.

AIM: To evaluate the computed tomography (CT) features of intraperitoneal tuberculous abscess (IPTA). METHODS: Eight patients with IPTA confirmed by pathology were analyzed retrospectively. The clinical symptoms, medical images, and surgical findings were evaluated. Involvement of the intestine, peritoneum, viscera, and lymph nodes was also assessed. RESULTS: All 8 patients had a history of abdominal discomfort for 1 to 6 mo. Physical examination revealed a palpable abdominal mass in 6 patients. Three patients had no evidence of pulmonary tuberculosis (TB). All IPTAs (11 abscesses) were seen

Tubo-peritoneal infertility: comparision of pre-operative hysterosalpingography and laparotomy findings (Tikur Anbessa Hospital, 1995 - 2002).

OBJECTIVES: To assess the value of hysterosalpingography (HSG) in diagnosing abnormalities of the fallopian tube in infertile women suspected to have infertility associated with a tubo-peritoneal cause. PATIENTS AND METHODS: Laparotomy and hydrochromopertubation was performed on 294 women after HSG using oil soluble contrast medium. RESULTS: There were 241 women with HSG bilateral tubal block and 53 with patent tubes (19 bilateral and 34 unilateral). The operative findings were 214 bilateral obstructions and 80 tubal patencies (49 bilateral and 31 unilateral). About 2/3 of the true patent tu

Intestinal Tuberculosis: A Diagnostic Challenge.

Diagnosing intestinal tuberculosis (TB) with uncommon clinical manifestations is often challenging. Here, we report a case of an alcoholic patient who presented with vague symptoms and was later diagnosed with intestinal TB. This patient experienced multiorgan failure causing hemodynamic instability requiring ionotropic support; acute hypoxic respiratory failure managed with non-invasive positive pressure ventilation, hepatic failure, transudative peritoneal effusion, and transudative pleural effusion. These conditions clouded our judgment to pursue colonoscopy for a definite diagnosis and del

Shunting in tuberculous meningitis: a neurosurgeon's nightmare.

OBJECTIVE: In a developing country like India, tuberculosis is very common in spite of a mass vaccination programme. Meningitis, progressive arteritis, adhesive arachnoiditis and tuberculomas represent the wide spectrum of this potentially lethal disease. Hydrocephalus occurs in about one third of the patients with central nervous system tuberculosis. Majority of patients have large fourth ventricles with adhesive obstructions in the basal cerebrospinal fluid (CSF) cisterns. Aggressive CSF diversion does not always alter the course of the disease. Endoscopic procedures are rarely, if ever, suc

A Case of Delayed Refractory Mycobacterium mageritense Abdominal Wall Abscess in a Kidney Transplant Recipient.

INTRODUCTION: Mycobacterium mageritense (M. mageritense), a rare non-tuberculous mycobacterium (NTM), can cause infections in immunocompromised patients, including kidney transplant recipients. We present a case of an abdominal wall abscess caused by M. mageritense following a living donor kidney transplant. CASE PRESENTATION: A 58-year-old woman, post-ABO-incompatible kidney transplant, developed an abscess at the site of a removed peritoneal dialysis catheter. Initial antibiotics were ineffective, and pus cultures identified M. mageritense. Surgical drainage and levofloxacin-linezolid thera

Partial characterization of a factor extracted from sensitized lymphocytes that inhibits the growth of Mycobacterium tuberculosis within macrophages in vitro.

Spleen lymphocytes of BCG-immunized mice contain a soluble factor that inhibits in vitro the growth of the H37Rv strain of Mycobacterium tuberculosis within normal peritoneal macrophages. The water-soluble extracts of sensitized lymphocytes, disrupted by freezing and thawing, although less active than the corresponding viable cells retained a significant growth-inhibiting activity. Dialysis against distilled water, lyophilization, exposure to ribonuclease and deoxyribonuclease, and storage at -20 degrees C of the water-soluble extracts did not affect their antimycobacterial activity, whereas e

Clinical factors associated with surgical interventions in patients with intestinal obstruction caused by abdominal tuberculosis.

PURPOSE: Abdominal tuberculosis (TB), including Intestinal TB (ITB) and peritoneal TB (PTB), can cause intestinal strictures, leading to obstructions. However, the surgical indications for TB-related intestinal obstructions are yet to be established. This study investigates the clinical factors associated with these surgical interventions. METHODS: Eighty-eight consecutive patients with ITB or PTB were enrolled in this study. The severity of abdominal TB was evaluated by the number of computed tomography (CT) findings of ascites, peritoneal nodules > 10 mm, intestinal wall thickening, and per

[The symptoms and surgical tactics for complicated forms of the abdominal cavity tuberculosis].

The results of treatment of 12 patients, suffering complicated forms of abdominal tuberculosis and external intestinal fistulas, were presented. Late diagnosis of abdominal tuberculosis in the patients, suffering the complications phase of the disease, is caused by unclear symptoms presence in early stages of the disease. Clinical and laboratory indices in peritonitis of a phthisis origin are nonspeciphic. In 91% of patients, admitted to the hospital for complicated forms of abdominal tuberculosis and external intestinal fistulas, the operative treatment was indicated. Surgical intervention (m

Abdominopelvic tuberculosis simulating disseminated ovarian carcinoma with elevated CA-125 level: report of two cases.

We report the radiologic presentations of two cases of peritoneal-pelvic tuberculosis. The initial interpretation based on the adnexal masses, ascites, omental and peritoneal thickening, and elevated serum CA-125 levels led to an erroneous preliminary diagnosis of disseminated ovarian cancer. In both patients, histologic examination showed tuberculosis. The clinical and radiologic findings resolved significantly after several months of multidrug antituberculosis treatment.

[Computer tomography aspects in the diagnosis of tuberculosis in the area of the abdomen and urogenital tract].

The morphologic aspect of abdominal tuberculosis are evaluated by C.T. The necrotising tuberculosis of the genitourinary tract presents course cystic lesions of fluid density. Similar morphologic criteria are seen in ileocecal tuberculosis. The peritoneal tuberculosis is characterized by large soft tissue masses of a high density which can be found in nearly all areas of the abdomen. The increased morbidity of tuberculosis in people from foreign countries living in the Federal Republic of Germany is pointed out.

Use of adenosine deaminase (ADA) to diagnose suspected peritoneal tuberculosis in Rwanda: a cross-sectional study.

BACKGROUND: Peritoneal tuberculosis is the most common cause of low albumin gradient ascites in developing countries, but it can be easily confused with other causes of ascites. Peritoneal tuberculosis requires early recognition of symptoms and signs in order to make a quick diagnosis for appropriate treatment. Measurement of adenosine deaminase (ADA) level > 39 in ascites fluid is an established test to diagnose peritoneal tuberculosis. Many low-income countries do not currently test for adenosine deaminase in ascites fluid, including Rwanda. METHOD: Cross-sectional, descriptive study conduc

Disseminated mycobacterial infection in AIDS patients: abdominal US features and value of fine-needle aspiration biopsy of lymph nodes and spleen.

BACKGROUND: We evaluated the efficacy of abdominal ultrasound (US) and fine-needle aspiration biopsy (FNAB) in the diagnosis of disseminated mycobacteriosis (DM) in acquired immunodeficiency syndrome (AIDS). METHODS: Twelve AIDS patients (nine male, three female; age range, 22-43 years) with DM underwent abdominal US within 2 days after admission with 3.5- to 5-MHz convex probes and a 7.5-MHz linear probe. All patients underwent FNAB of one or two enlarged abdominal lymph nodes. Eight patients underwent FNAB of the spleen. The aspirated specimens were stained with acid fast for quick examinat
