Demyelinating sensorimotor neuropathy with congenital cataract, mental retardation, and unique, dysplastic perineurial cells within the endoneurium.

We report on a 27-year-old Caucasian female with congenital cataract and mental retardation complaining of progressive paresis and atrophy of the lower legs beginning at the age of 16 years followed by atrophy of the thighs and small hand muscles. Motor and sensory conduction velocities (CV) of the upper and lower limbs were reduced (distal peroneal nerve: 21 m/s; median nerve: motor CV: 28 m/s, sensory CV 30 m/s). In the sural nerve biopsy specimens there were unique endoneurial cells immunoreactive for antibodies against the epithelial membrane antigen with multiple surface indentations and

Pattern of myelinated fibre loss in the sural nerve in neuropathy related to type 1 (insulin-dependent) diabetes.

Sural nerve biopsies were obtained from 17 diabetic patients with neuropathy. All patients except three had both a symmetric distal sensory and autonomic polyneuropathy related to Type 1 (insulin-dependent) diabetes mellitus; 3 patients had a purely sensory polyneuropathy. Mean age was 34.5 years (range 18-53 years). The biopsies were compared with specimens from an age-matched control series. Myelinated fibre loss in the diabetic nerves was found to be nonuniform. Although patchy fibre loss has been considered to favour a vascular basis, an identical pattern of nonuniform loss was observed in

Myopathy and neuropathy associated with osteomalacia.

Thirty unselected females with proven osteomalacia were evaluated clinically, electromyographically and histopathologically for muscle dysfunction. Clinical evidence of myopathy was found in all the patients except one; the electromyograms were abnormal in 25 of them, and histopathological abnormalities, although slight and nonspecific, were seen in all the 17 patients who underwent muscle biopsy. Electromyographic findings revealed a myopathic pattern as evidenced by a significant reduction in motor unit potential duration and amplitude, and an increased percentage polyphasicity as compared t

Protection from neuropathy in extreme duration type 1 diabetes.

A proportion of individuals with type 1 diabetes mellitus for more than 50 years (medallists) may be protected from developing nephropathy, retinopathy and neuropathy. Detailed neuropathy phenotyping was undertaken in a cohort of 33 medallists aged 63.7 ± 1.4 years with diabetes for 58.5 ± 0.8 years and HbA1c of 65.9 ± 2.1 mmol/mmol. Medallists had a significantly higher HbA1c (P < .001), lower estimated glomerular filtration rate (eGFR) (P = .005) and higher albumin creatinine excretion ratio (ACR) (P = .01), but a lower total cholesterol (P < .001), triacylglycerols (P = .001), low density l

A new single-nucleotide deletion of PMP22 in an HNPP family without recurrent palsies.

In this study we describe four patients from the same kindred who were affected by an autosomal-dominantly inherited peripheral neuropathy. They presented an unusual combination of clinical, electrophysiological, and pathological findings in association with a new mutation of the PMP22 gene. Clinically, three patients had carpal tunnel syndrome symptoms and one patient had late-onset peroneal atrophy. Motor and sensory nerve conduction velocities were reduced without focal slowing at entrapment sites. Nerve biopsy disclosed diffuse hypomyelination with focal thickening of the myelin sheath in

Electrophysiological asymmetry in vincristine-exposed children with acute lymphoblastic leukemia: Evidence from bilateral nerve conduction studies.

Vincristine-induced peripheral neuropathy (VIPN) is a frequent complication of therapy for acute lymphoblastic leukemia (ALL) in children. Beyond acute toxicity, VIPN may affect motor development, balance, and quality of life in survivors. Although often assumed to be symmetric, the extent of electrophysiological asymmetry has not been systematically evaluated. This study aimed to quantify side-to-side differences in vincristine-exposed children using bilateral nerve conduction studies (NCS). Forty-seven bilateral NCS assessments were performed in 47 children with ALL (32 post-treatment surviv

Complete fifth ray amputation with peroneal tendon transfer--a staged surgical protocol.

Nonhealing neuropathic ulcers overlying the fifth metatarsal are frequently associated with cavus foot structure and are often complicated by osteomyelitis. Partial fifth ray amputation for metatarsal phalangeal joint ulceration and osteomyelitis is a time-proven procedure. Recurrent wounds and persistent osteomyelitis at the amputation stump or fifth metatarsal base create significant challenges in a cavus foot with neuropathy. Long-term success with removal of the entire fifth ray is largely dependent on preventing infection of the cuboid and maintaining peroneal tendon function. The describ

Common peroneal nerve decompression.

BACKGROUND: Common peroneal neuropathy occurs as a result of compression of the common peroneal nerve (CPN) in the fibro-osseous tunnel between the fibular neck and the tendinous arch of peroneus longus. High rates of spontaneous recovery have been reported. However, there is a subset of patients who require decompression of the CPN at the fibular neck. METHODS: We performed a retrospective analysis of patients that underwent decompressive surgery of the CPN. A total of 22 operations performed on 20 patients were analysed. Pre-operative workup included clinical examination, electrophysiologic

Electrophysiologic features of fibular neuropathy in childhood and adolescence.

INTRODUCTION: We studied patterns of nerve injury in pediatric common fibular (peroneal) neuropathy (CFN). METHODS: A retrospective analysis was performed on data from 53 children with CFN at a pediatric electromyography laboratory. RESULTS: Conduction block at the fibular head was present in 35% of patients. Deep fibular axonal loss was identified in 77%, while superficial fibular axonal loss was identified in 45%. The pathophysiology was predominantly axonal in 72%, mostly demyelinating in 6%, and mixed in 22%. Predominantly demyelinating lesions at the fibular head demonstrated sparing of

Peroneal nerve palsy after compression stockings application.

Peroneal nerve palsy can be caused by various etiology. We report unilateral peroneal nerve palsy after compression stockings application. A 64-year-old man underwent off-pump coronary bypass graft. Surgeon did not use saphenous vein for the bypass graft. Sedation was stopped after 3 h postoperative. After 16 h, for prophylaxis of deep vein thrombosis, knee-high elastic stocking was applied. After 1 h, he took off right stocking because of numbness but left stocking was kept. After 24 h postoperative, (8 h after stocking application) patient complained suddenly left foot drop. Manual muscle te

GLP-1 Receptor Agonist-Associated Slimmer's Palsy: Implications for the Peripheral Nerve Surgeon.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are increasingly used for weight management and cardiometabolic health. Common peroneal neuropathy (CPN) associated with rapid weight loss is attributed to adipose tissue reduction and subsequent nerve compression at the fibular head. Cases of "Slimmer's Palsy" have been described in conditions involving rapid weight loss, including anorexia, malignancy, and post-biliary surgery, yet it remains underrecognized as a potential complication of GLP-1RA therapy. This case report describes two nondiabetic patients who developed acute foot drop aft

Motor neuron disease mimics in practice: a case series.

In the majority of cases of motor neuron disease (MND), diagnosis is clinical and unambiguous. However, given the gravity of the diagnosis, it is crucial that treatable mimics are differentiated accurately. We present three cases referred to our clinic with possible MND with unusual features that led to an alternative diagnosis. (1) A middle-aged man with swallowing, then speech problems and foot drop. Tongue movements were slow and electromyography (EMG) showed fibrillations/positive sharp waves. Following investigation, a tonsillar tumour involving the tongue root was identified, and the foo

Ischaemic monomelic neuropathy: A rare cause of unilateral foot drop.

Ischaemic neuropathy is a rare phenomenon given the rich arterial collateral supply afforded to peripheral nerves by the vasa nervorum. We report an unusual case of unilateral foot drop secondary to long-segment popliteal artery occlusion. Without expedient vessel imaging and revascularisation of the occluded artery, this reversible cause of neurological deficit would likely have resulted in a poor functional outcome for our patient.

[Differential Diagnosis between Spinal Degenerative Disorder and Peripheral Nerve Entrapment Disease].

In the surgical treatment of spinal degenerative disorders, neural function improves to some extent upon successful neural decompression. However, we have witnessed cases with poor postoperative results. The causes include surgical procedure issues, such as insufficient decompression of neural tissues and overlooking sites to be decompressed, and neurological misdiagnosis of other causative lesions or multiple lesions. Here, as necessary knowledge for spine surgeons dealing with spinal cord degenerative disorders to prevent poor surgical results, we have discussed the differentiation of periph
