[Clinical presentations of Cockett's syndrome. 16 cases].

Cockett's syndrome (iliac compression syndrome) results from compression of the common iliac vein by the aortic bifurcation. The anatomical abnormality is frequent (15-20% of the population), but symptoms only appear when compression is pronounced. In a study of 16 patients, predominantly female, the following clinical presentations were observed: venous oedema of the left lower limb in 4 cases; symptoms suggesting iliac phlebitis in 3 cases; pulmonary embolism as first sign of peripheral venous thrombosis in 6 cases; and deterioration of angiodyplasia in 3 cases. In all patients, the diagnosi…

The clinico-pathological spectrum of Mondor's disease: an important surgical entity.

A total of five cases of Mondor's disease and/or Mondor variants are presented. Although the patients did not have the classical signs of extreme tenderness along the right lateral chest wall, Mondor's disease was detected in later forms at biopsy which was taken for various reasons, such as lump in the breast, discomfort, abnormal mammography, etc. The progression of the disease from a form of acute phlebitis to thrombophlebitis and phlebosclerosis with either recanalization or ultimate obliteration of the vein wall and lumen by sclerotic hyalinized tissue and/or calcification, is demonstrate…

Cephacetrile in the treatment of female pelvic inflammatory disease.

The efficacy and tolerability of cephacetrile administered by the intravenous or intramuscular route was evaluated in an open-label study of 55 hospitalized females diagnosed as having mild to severe pelvic inflammatory disease. Rapid improvement was observed in most patients. All patients were classified as clinically cured or significantly improved. Pain was absent or mild in 86% of the patients who received cephacetrile by intramuscular injection. Seventy-two percent of the patients given the drug intravenously experienced no localized pain. Phlebitis occurred in 6 of the 53 patients (11%) …

IgG4-Related Cutaneous Vasculitis Presenting as Multiple Subcutaneous Nodules.

A 66-year-old woman presented with multiple nonpruritic subcutaneous nodules measuring up to 1.8 cm on the left forearm. An excision was per … eld and an IgG4:IgG ratio of >0.5, supporting a diagnosis of IgG4-related disease with obliterative phlebitis of a medium-sized subcutaneous vein. This presentation expands the current classification of cutaneous IgG4-related disease and highlights the need for broader recognition of vascular and clinical manifestations in this disease spectrum.

PICC tip dislodgement causing massive pleural effusion and atelectasis with acute respiratory failure: a case report.

Peripheral intravenous central catheter (PICC) is a common tool for intravenous infusion for children who need central venous access. Although it is safe for physicians and nurses to place, complications like infection, occlusion, phlebitis, and bleeding can occur. We report a 5-month-old infant who suffered respiratory failure caused by catheter malposition resulting in massive fluid infusion into the thoracic cavity. Point-of-care ultrasound (POCUS) was utilized to identify a massive pleural effusion that prompted urgent drainage. Complications related to PICC in pediatric patients are not c…

Immunoglobulin G4-Related Disease Presenting as Recurrent Acute Pancreatitis.

Immunoglobulin G4-related disease (IgG4-RD) is a rare immune-mediated disease affecting multiple organs and tissues. There is often the presence of elevated serum Ig4 subtype with histological evidence of lymphoplasmacytic infiltration, fibrosis, and phlebitis. The mainstay of treatment is steroid therapy. This case report is based on a 24-year-old man with IgG4-related type 1 autoimmune pancreatitis (AIP) who also had elevated serum IgG4 subclass and histological features in keeping with IgG4-RD. The main complaints were dry cough, nasal congestion with sneezing, sore throat, and fever. Neces…

Sclerosing cholangitis may mimic radiological pattern of cholangiocarcinoma: Differential diagnosis and review of literature.

IgG4 related sclerosing cholangitis (IgG4-SC) is a chronic cholestatic autoimmune liver disease, characterized by obstruction of the biliary tract due to IgG4-positive plasma cell infiltrations associated with fibrosis, phlebitis and presence of eosinophils. The diagnosis of the disease is supported by elevation in serum IgG4, radiological imaging and histology. In most of the cases IgG4-SC is associated with a wide spectrum of symptoms and manifestations summarized by the definition of IgG4-related disease (IgG4-RD). When the IgG4-SC causes an interruption of the biliary tract, differential d…

A rare cause for lower back pain: a case of an IgG4-related periaortitis.

IgG4-related disease (IgG4-RD) are a group of autoinflammatory diseases often presenting as tumor-like lesions because of their infiltrative or mass forming behavior. They are characterized by a typical histology consisting of storiform fibrosis, high numbers of infiltrating IgG4-positive plasma cells, obliterative phlebitis, and a moderate presence of eosinophilic cells. Serum IgG4 levels can be elevated. We present a case of a 57 year-old male patient with immobilizing lower back pain, fever, and night sweats. We diagnosed IgG4-related periaortitis using serum IgG4 levels, abdominal ultrasou…

Behçet syndrome: from pathogenesis to novel therapies.

Behçet syndrome is a chronic disease hallmarked by inflammation of the blood vessels that is related to an autoimmune reaction caused by inherited susceptibility due to specific genes and environmental factors, probably components of infectious microorganisms, which turn on or get going the disease in genetically susceptible subjects. The more common clinical expression of the disease is represented by a triple-symptom complex of recurrent oral aphthous ulcers, genital ulcers, and uveitis, sometimes associated with inflammatory arthritis, phlebitis, iritis, as well as inflammation of the diges…

Anatomy and physiology of venous system vascular access: implications.

Ideal characteristics for long-term vascular access that can be managed in the home environment include a large vascular lumen in a high-flow blood system to minimize risks of obstruction and phlebitis, ease of sterile dressing maintenance, patient comfort, and longevity of the access site. Additionally, adaptation to placement at the bedside or in the home, technical ease and safety of placement, and minimal expense would be beneficial. With these considerations, possible anatomical sites would include the external and internal jugular veins, subclavian vein, cephalic vein, basilic vein, femo…

Coccidioidomycosis meningitis with massive dural and cerebral venous thrombosis and tissue arthroconidia.

To our knowledge we report the first case of meningitis from Coccidioides immitis associated with massive dural and cerebral venous thrombosis and with mycelial forms of the organism in brain tissue. The patient was a 43-year-old man with late-stage acquired immunodeficiency syndrome (AIDS) whose premortem and postmortem cultures confirmed C immitis as the only central nervous system pathogenic organism. Death was attributable to multiple hemorrhagic venous infarctions with cerebral edema and herniation. Although phlebitis has been noted parenthetically to occur in C immitis meningitis in the …

Diagnosis and management of the acute retinal necrosis (ARN) syndrome.

The acute retinal necrosis (ARN) syndrome represents a specific pattern of clinical presentation for certain herpes virus infections in the posterior segment of the eye. The classically described triad of the ARN syndrome consists of (1) an arteritis and phlebitis of the retinal and choroidal vasculature, (2) a confluent, necrotizing retinitis that preferentially affects the peripheral retina, and (3) a moderate to severe vitritis. Anterior segment inflammation, optic neuritis, and late retinal detachment are also common features of this disorder. Definitive evidence now implicates at least tw…

Phase II study of a new alkylating agent (PTT-119) in resistant-relapsed non-Hodgkin's lymphomas.

In a phase II study we evaluated the effect and toxicity of a new alkylating agent, PTT-119, in 26 patients with non-Hodgkin's lymphomas (NHL) resistant to or relapsed after other chemotherapy. PTT was scheduled by escalating the dose from 2.0 to 3.3 mg/kg every 3 weeks. Among 21 evaluable patients with NHL, 12 (57%) showed a good response (CR + PR) to PTT-119. Tolerance was acceptably good; no major side effects related to liver, cardiac, or renal toxicity were recorded. The most commonly recorded side effects were nausea and vomiting, alopecia, and phlebitis; diarrhea and drug-related fever …

[Carbon dioxide laser in arthroscopic surgery of the knee].

In surgery of the knee, combining CO2 laser and arthroscopy provides the benefits of both techniques: work at a distance from the lesion and bleeding control for the laser, patient's comfort and functional recovery for the arthroscopy. This original method requires a CO2 laser of at least 40 watts, CO2 as gas for arthroscopy and the appropriate manual instruments. Forty patients totalling 52 lesions were operated upon by this method with 11 initial failures and 5 complications: 1 case of synovial fluid leakage due to skin burn, 2 cases of phlebitis and 2 cases of spontaneously regressive fever…

Immunoglobulin G4-related disease in a dog.

Immunoglobulin G4-related disease (IgG4-RD), which affects many organ systems, has been recognized as a distinct clinical entity in human medicine for just over a decade but has not been previously identified in dogs. In humans, IgG4-RD is characterized by diffuse IgG4-positive lymphoplasmacytic infiltrates that commonly lead to increased serum concentrations of IgG4 and IgE, peripheral eosinophilia, tumorous swellings that often include the parotid salivary glands, obliterative phlebitis, and extensive fibrosis. Herein we describe the diagnosis, clinical progression, and successful treatment …
