The effect of surface modifications on titanium to enable titanium-porcelain bonding.

OBJECTIVES: Titanium-ceramic restorations are currently used, despite the pending problem of titanium-ceramic bonding, which has only been partially solved. The surface treatment of the metal proposed by the manufacturer promotes lower bond strength between titanium and porcelain when compared to the conventional metal-ceramic systems. The objective of this study was to evaluate the influence of acid and caustic baths on the bonding characteristics of specific titanium porcelain bonded to cast commercially pure titanium (CP Ti). METHODS: Eighty strips of cast CP Ti were obtained in dimensions

Blunt trauma of the aging eye: injury mechanisms and increasing lens stiffness.

OBJECTIVE: To investigate possible injury mechanisms in the eyes of elderly individuals and the effects of lens stiffness on model outputs indicative of injury as a function of age. METHODS: Three separate frontal impact scenarios, a foam particle (30 m/s), steering wheel (15 m/s), and air bag (67 m/s), were simulated with a validated finite-element model to determine the effects of changing lens stiffness on the eye when subjected to blunt trauma. The lens stiffness of the model was increased with increasing age using stiffness values determined from the literature for 3 age groups. RESULTS

Influence of a controlled pressure system on toothbrushing behavior.

The use of excessive brushing force has been shown to be a major cause of gingival abrasion. To aid in preventing over-vigorous brushing, the Philips/Jordan electric toothbrush incorporates a Controlled Pressure system (CPS) that causes the brush head to flex back when a toothbrushing force (TBF) in excess of a pre-determined threshold is exerted against the teeth or soft tissues. Two studies (I/II) were conducted to determine whether this mechanical feedback system is sufficiently sensitive to enable users to control their brushing behavior. In Study I, the learning pattern of brushing behavi

Effect of a semi-rigid ankle stabilizer on performance in persons with functional ankle instability.

STUDY DESIGN: Within-group repeated measures. OBJECTIVES: To determine the effect of a semi-rigid prophylactic ankle stabilizer (PAS) on performance of subjects with post-acute, unilateral ankle sprains who have mechanically stable ankles, but are functionally impaired. BACKGROUND: Most studies on PAS performance to date are limited to subjects with noninjured, nonimpaired ankles. No research has been reported to determine the effect PASs have on performance in subjects who have a mechanically stable, nonacute ankle sprain with functional impairment. METHODS AND MEASURES: Twenty-five subjec

Comparison of the flexible and standard laryngeal mask airways.

PURPOSE: To determine mucosal pressures, ease of insertion, mask position and oropharyngeal leak pressures for the flexible (FLMA) and standard laryngeal mask airway (LMA). METHODS: Forty anesthetized, paralysed adult patients were randomly allocated to receive either the FLMA or LMA. Microchip sensors were attached to the LMA or FLMA at identical locations corresponding to the base of tongue, hypopharynx, lateral pharynx, oropharynx, posterior pharynx and pyriform fossa. Mucosal pressure, oropharyngeal leak pressure (OLP) and mask position (assessed fibreoptically) were recorded during infla

Knee flexion to extension peak torque ratios and low-back injuries in highly active individuals.

The purpose of this study was to investigate for possible relationships between knee flexion to extension peak torque ratios (F1/Ext(rat)) and low-back injuries in highly active males and females. Forty-eight male (age 25.9 +/- 4.5 years) and 41 female (age 27.3 +/- 2.6 years) competitive rowers, and 20 male (age 26.6 +/- 6.0 years) professional ballet dancers volunteered for the study. Each subject performed a test of lumbar and knee flexor flexibility, isokinetic dynamometry and completed a self-administered questionnaire. Flexibility was assessed by using the sit-and-reach test. Juvenile kyphosis: effects of different variables on conservative treatment outcome.

We reviewed the records of 212 patients with idiopathic or Scheuermann-type juvenile kyphosis (Scheuermann's disease). The 200 patients available for follow-up were divided into three groups depending on the degree of angular deformity, and the influence of different variables on treatment outcome in each group was investigated. A very influential positive variable was combined treatment with a body cast plus brace; exercise treatment also produced acceptable results. Other variables that positively influenced the outcome of treatment were compliance with treatment, and (unexpectedly) elevated

Intrinsic flexibility of West Nile virus protease in solution characterized using small-angle X-ray scattering.

West Nile virus (WNV) is a mosquito-borne flavivirus with a rapidly expanding global distribution. Infection can cause severe neurological disease and fatality in humans. Efforts are ongoing to develop antiviral drugs that inhibit the WNV protease, a viral enzyme required for polyprotein processing. Unfortunately, little is known about the solution structure of recombinant WNV protease (NS2B-NS3pro) used for antiviral drug discovery and development, although X-ray crystal structures and nuclear magnetic resonance (NMR) studies have provided valuable insights into the interactions between NS2B-

Foot motion in children shoes: a comparison of barefoot walking with shod walking in conventional and flexible shoes.

The increased prevalence for flatfoot and hallux valgus in modern societies may be the consequence of inadequate footwear in childhood. Based on the assumption that barefoot walking represents the best condition for the development of a healthy foot the objective of this study was to monitor the influence of commercial footwear on children's foot motion during walking. Furthermore, an attempt was made to reduce this influence by changing the physical properties of standard footwear. Children's barefoot motion pattern was monitored by a marker-based optical 3D-tracking method using a multi-segm

Backbone dynamics and thermodynamics of Borrelia outer surface protein A.

Nuclear spin relaxation experiments performed at 298K, 308K and 318K are used to characterize the intramolecular dynamics and thermodynamics of outer surface protein A (OspA), a key protein in the life-cycle of Borrelia burgdorferi, the causative agent of Lyme disease. It has recently been demonstrated that OspA specifically binds to the gut of the intermediate tick host (Ixodes scapularis), and that this interaction is mediated, at least in part, by residues in the C-terminal domain of OspA that are largely inaccessible to solvent in all X-ray structures of this protein. Our analysis of 15N r

Effects of masticatory muscle function and bite-raising on mandibular morphology in the growing rat.

The aim of this series of investigations was to study the effect of masticatory muscle function on the growth pattern and on the internal structure of the mandible during growth. The muscular and dentoskeletal growth adaptation to prolonged bite-raising and the role of the functional state of the masticatory muscles in this adaptation were also to be elucidated. Differences in masticatory muscle function were induced in young rats by altering the consistency of the diet. Bite-raising was produced by the insertion of posterior bite blocks. Morphometric analysis of the internal bone structures o

A method for permanent transvenous left ventricular pacing.

LV-based pacing has recently been reported to be of benefit in patients with severe cardiac failure and left bundle branch block. LV permanent pacing has been reported using epicardial leads but the surgical mortality is excessive. A transvenous approach is now favored. In this regard, cannulation of the coronary sinus and of one of its tributaries using only the permanent electrode is feasible but technically challenging. We describe a "long guiding sheath" method using catheterization, and a long radiopaque and peelable sheath. Once the coronary sinus is cannulated with the electrophysiologi

Placing a bone graft more posteriorly may reduce the risk of pedicle screw breakage: analysis of an unexpected case of pedicle screw breakage.

Telemeterized internal spinal fixation devices were implanted in a patient with degenerative instability and a narrow spinal canal in order to measure the fixator loads during daily activities. Anterior interbody fusion was performed three weeks later. During walking, the typical maximum flexion bending moments were 10 N m in the left and 5 N m in the right fixator. On removal of the implants three months later, a fatigue fracture was found not on the high loaded left side but in the upper right pedicle screw. The crack started on the caudal side of the cross-sectional area and progressed cran

The Triple Wire Technique for Delivery of Endovascular Components in Difficult Anatomy.

We describe a novel endovascular technique in which three 0.014″ guidewires are placed in parallel through a 0.035″ lumen catheter, in order to create a stiff platform to allow for delivery of 0.035″ profile devices through challenging anatomy. Three illustrative cases are presented: a difficult aortic bifurcation during lower extremity intervention, a tortuous internal iliac artery during placement of an iliac branch device, and salvage of a renal artery after inadvertent coverage during proximal cuff deployment for type 1a endoleak. We also quantify the relative stiffness of the triple 0.014
