Serum antiganglioside antibodies were investigated in motor neuron disease (MND) and in chronic or acute immune-mediated motor-dominant neuropathies. IgM antibody binding to GM1 and GA1, reacting with Gal beta 1-3GalNAc epitope, was seen in 5 out of 26 cases of MND. IgM antibody binding to Gal beta 1-3GalNAc epitope was also detected in 3 out of 3 cases with motor dominant neuropathy with multifocal conduction block (MNMCB). The IgM M-protein in a case with motor dominant neuropathy bound to GM1, GD1b, GM2 but not to GA1. This M-protein may recognize carbohydrate epitope including sialic acid.

Two patients developed Guillain-Barré syndrome after varicella-zoster infection; a 4-year-old boy after chickenpox and a 74-year-old man after shingles. Both had decreased CD8-positive lymphocytes in the blood during the course of their illness. These cases and others reported in the literature suggest that varicella-zoster infection is a significant but rare trigger factor of Guillain-Barré syndrome. An alteration in the balance of helper and suppressor lymphocytes may be an important pathogenetic mechanism.

A 70-year-old patient is described who developed symptoms and signs of a Guillain-Barré-type neuropathy and encephalopathy beginning 6 weeks after a cutaneous varicella zoster eruption. The patient made a complete symptomatic recovery over a period of 4 months. It is suggested that the delayed onset neuropathy and encephalopathy in this patient may have been due to an allergic inflammatory response to an antigen common to peripheral and central myelin.

Sphincteral disorders are very rarely seen in a Guillain-Barre syndrome. The authors report a case of such a syndrome in a woman, in the course of which a retention of urine appeared making diagnosis more difficult. The cause actually was mechanical.

Guillain-Barré syndrome (GBS) is an acute inflammatory polyradiculoneuropathy. In two-thirds of patients, it is preceded by an upper respiratory or gastrointestinal tract infection. Temporally associated cases of GBS following COVID-19 vaccination have been described with different COVID-19 vaccines. In this study, we report three cases of GBS patients following COVID-19 vaccine. Two of the studied patients received the Sinopharm vaccine and one patient received the AstraZeneca vaccine. All patients were diagnosed with acute motor axonal neuropathy (AMAN) type of GBS, on nerve conduction studi…

INTRODUCTION: Papilledema is a common sign in ophthalmology and is typically associated with increased intracranial pressure (ICP) in neurological diseases. Since the beginning of the 20th century, some cases of papilledema have been reported in association with acute or chronic inflammatory neuropathies.
CASE REPORT: We describe a 42-year-old man with acute-onset inflammatory polyradiculoneuropathy and bilateral papilledema.
CONCLUSIONS: Based on a personal case report and from an extensive review of the medical literature, we identify 2 distinct patterns. First, radiculoneuropathy may be a…

Immunotherapy has been investigated in a small subset of peripheral neuropathies, including an acute one, Guillain-Barré syndrome, and 3 chronic forms: chronic inflammatory demyelinating polyradiculoneuropathy, multifocal motor neuropathy, and neuropathy associated with IgM anti-myelin-associated glycoprotein. Several experimental studies and clinical data are strongly suggestive of an immune-mediated pathogenesis. Either cell-mediated mechanisms or antibody responses to Schwann cell, compact myelin, or nodal antigens are considered to act together in an aberrant immune response to cause damag…

INTRODUCTION: Guillain-Barre syndrome (GBS) is an acute polyradiculoneuropathy with a variable clinical presentation and is the most common cause of acute palsy in children. The classic presentation is an acute monophasic, nonfebrile, postinfectious illness manifesting as an ascending flaccid palsy with areflexia. Aim. Analysis of children with GBS in 6 years observation.
MATERIAL AND METHODS: In the present study we describe a population of 21 children (19 boys and 2 girls) with GBS hospitalized in the years 2009-2014 in our department. In this group 20 children (95%) had flaccid palsy of li…

We describe experience with the use of mycophenolate mofetil (MMF) in neurological diseases. Although only small series of patients or case reports were described, MMF is promising in immune-mediated neuromuscular disorders. MMF has been used for the treatment of polymyositis, chronic inflammatory demyelinating polyradiculoneuropathy, and multifocal motor neuropathy. These studies showed that MMF is well tolerated and may be useful in some patients. MMF can be effective alone but mainly as an adjuvant therapy by reducing steroid requirements or the frequency of infusions of IVIg. MMF has also …

An immune response to the peripheral nerve basic protein P2 may be operative in the pathogenesis of the Guillain-Barré syndrome (GBS). A method is described for the purification of P2 of human origin. Purified P2 was used to investigate whether lymphocytes derived from peripheral blood of GBS patients are capable of producing P2-specific antibodies after stimulation with the antigen in vitro. Peripheral blood lymphocytes (PBL) from 5 GBS patients, from 3 patients with chronic idiopathic polyradiculoneuropathy (CIP) and from 3 normal controls were cultured in vitro in the presence of P2. PBL fr…

BACKGROUND: CIDP is a rare autoimmune disorder with varying symptom presentations. This heterogenous nature challenges standardizing disease monitoring and assessing treatment response. This study aimed to provide a comprehensive overview of clinical outcome assessments (COAs) used in CIDP.
METHODS: A structured literature review and mapping exercise was conducted to identify COAs reported in CIDP literature published between 2010 and 2023. Supplementary searches were conducted in other relevant sources. Key publications pre-2010 were identified through handsearching or consultation with clin

BACKGROUND AND OBJECTIVES: Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is considered an immune-mediated heterogeneous disease that involves both cellular and humoral immunity. The advent of the new concept of node-paranodopathy in recent years has boosted the identification of more antibody-positive CIDP variants patients. Cases of Caspr1 autoantibodies are the least common. Here, we reported two patients with Caspr1 autoantibodies and summarized their clinical features and treatment responses.
METHODS: Do statistical analyses on the clinical manifestations and laboratory…

The most common autoimmune neuropathies include the acute inflammatory polyneuropathy [the Guillain-Barré Syndrome(s)]; chronic inflammatory demyelinating polyneuropathy (CIDP), multifocal motor neuropathy (MMN) and IgM anti-MAG-antibody mediated paraproteinemic neuropathy. These neuropathies occur when immunologic tolerance to peripheral nerve components (myelin, Schwann cell, axon, and motor or ganglionic neurons) is lost. Based on the immunopathologic similarities with experimental allergic neuritis induced after immunization with nerve proteins, disease transfer experiments with the patien

Autoimmune neuropathy models have been refined in animals over the past 50 years to enable links to be made between antibody signatures and disease pathogenesis; however, the application of these models to identify biomarkers has not been fully developed. Models for chronic inflammatory demyelinating polyneuropathy (CIDP) must ensure that adequate distinctions are made from variants of other neuropathic states such as Guillain-Barré syndrome (GBS) in order to be of greatest clinical use. One of the main hurdles to overcome is the diverse pathogenesis of CIDP, which must be reflected in any pot
