Cystic adventitial disease of the popliteal artery: diagnosis and treatment.

PURPOSE: This paper represents the presentation of a case of cystic adventitial disease of the popliteal artery and an exhaustive review of the literature with an emphasis on optimal means for diagnosis and treatment of this unusual disease. METHODS: We collect all reported cases of cystic adventitial disease of the popliteal artery since the last review in 1987 with an additional review of these cases reported in the U.S. since the initial description of the disease. RESULTS: We found 264 cases of cystic adventitial disease (CAD) of the popliteal artery from 1954 to 1995, with 38 reported i

[Pseudothrombophlebitis syndrome in Reiter's syndrome].

Although the pseudothrombophlebitis syndrome has been studied for a long time it still presents some difficulties to be diagnosed and we can not distinguish it from real thrombophlebitis only by clinical methods. The author presents a Reiter's syndrome patient with recurrent Baker's cyst rupture that develops into pseudothrombophlebitis and discusses some aspects related to this diagnosis and treatment. He calls the attention to this diagnosis mainly in those patients presenting clinical signs of thromboplebitis of the inferior member and knee arthropathy.

Cystic adventitial disease of the popliteal artery. Clinical aspects and etiology.

Cystic adventitial disease of the popliteal artery is a rare and benign disease. The appearance of claudication in a young male nonsmoker and the typical angiographic findings usually confirm the pathology. The treatment of choice is incision evacuation of the cyst. Long-term follow-up of two cases proved to be excellent with this therapy. A saphenous bypass graft provides a good alternative if incision evacuation fails to restore good peripheral pulses. The etiology of this lesion proves to be similar to a ganglion and is well demonstrated in two cases.

[Pitfalls during phlebology consultation].

For a patient suffering with his legs, the pain is usually associated with the presence of one or more varices that can be seen. The physician must not fall into this trap. Questioning is vital. Establishing the kind of pain and the conditions under which it occurs may elok for (1) a rheumatological etioloty: sciatic or crural nerve pain, calcaneal pain: (2) a surgical etiology: an inguinal or particularly a crural hernia, a popliteal cyst; (3) a medical etiology: exogenous polyneuritis, hypokalaemia resulting from the abuse of laxatives or of diuretics, etc.

Clinical utility and potential of ultrasound in osteoarthritis.

Osteoarthritis (OA) is the most common disorder of human joints. Imaging is necessary in daily practice for OA patients, and conventional radiography remains the gold standard. However, conventional radiography is not sensitive in the early stage of OA and cannot clearly detect inflammatory condition. Recently, ultrasonography (US) is widely used in musculoskeletal field and US performs better or at least equally well for identification of osteophytes and morphologic degeneration of cartilage in OA patients. US provides relevant additional diagnostic information on pathologic changes in soft t

Ruptured Baker's cyst with compartment syndrome: an extremely unusual complication.

A 69-year-old man presented with sudden onset of pain with acute tense swelling of his left leg. Initially he was treated empirically with antibiotics for cellulitis while the possibility of deep vein thrombosis was ruled out. His symptoms gradually worsened with progressive distal neurological deficit and increasing pain. Further investigations suggested that he had a ruptured Baker's cyst in the calf with development of compartment syndrome.

Evaluation of the knee pathology by ultrasonography.

Fourteen patients undergoing total knee replacement for the arthritic conditions of the knee were evaluated by ultrasonography (USG) prior to surgery. USG findings (synovium, fluid, femoral cartilage, cyst and loose bodies) were compared with surgical findings. USG and surgical findings matched for synovial pathology in 57%, for presence fluid in 71% and for cartilage damage in 45%. Loose bodies and cyst present in one patient was not picked up by USG. Baker's cyst was detected in one patient.

[Pseudo-thrombophlebitis syndrome: diagnostic and therapeutic considerations of 2 cases].

Pseudo-thrombophlebitic syndrome is a frequent clinical entity but many times insufficiently diagnosed. Described more than a century ago by Baker it is characterized for being clinically undistinguishable from a true thrombophlebitis, being its cause the presence of a synovial cyst in the knee joint (Baker's cyst) which can be complicated or not (breakage or dissection). Diagnosis is easy and is based mainly in echography and arthrography. The anticoagulant therapy used in true thrombophlebitis is contraindicated in this syndrome.

Preoperative localization of cystic lesions in the knee using ultrasound-guided injection of indigo carmine.

PURPOSE: To evaluate the feasibility and effectiveness of preoperative localization of cystic lesions in the knee using ultrasound-guided indigo carmine injection. METHOD: Twenty-three cysts in the knee in 23 patients (M:F = 15:8, mean age, 42 years) were localized preoperatively by ultrasound-guided indigo carmine injection. These included 12 meniscal cysts, 7 popliteal cysts, and 4 ganglion cysts. To stain the lesions, 0.2-3 mL of indigo carmine was injected into the cyst using a 22-gauge spinal needle. After localization, the patient was immediately transferred to the operating room and su…

Meniscal and ganglion cysts of the knee: MR evaluation.

Meniscal and ganglion cysts frequently present as palpable masses of the knee but occur at different locations than do popliteal cysts. Meniscal cysts also may be discovered incidentally on studies performed for suspected internal derangement. Sixteen cystic lesions of the knee were evaluated with MR, including 11 meniscal cysts and five ganglion cysts. Scans were performed at 1.5 T by using a transmit/receive extremity coil or a receive-only surface coil. Standard spin-echo imaging, including at least one long-TR/asymmetric-TE sequence, was performed in all cases. In six patients, gradient-ec…

False aneurysm of the popliteal artery complicating acupuncture.

This is the first reported case in which acupuncture needle injury of the popliteal artery caused the development of a false aneurysm. The patient presented with rupture of the aneurysm and was successfully managed by arterial repair.

Treatment failure after rotational atherectomy and balloon angioplasty in recurrent cystic adventitial disease of the popliteal artery: a case report.

Cystic adventitial disease (CAD), which usually affects the popliteal artery, is a rare vascular condition in which fluid accumulates in the sub-adventitial layer, compressing the lumen. Historically, surgical treatment is preferred over endovascular or minimally invasive techniques, due to its lower recurrence rates. Here, the case of a 67-year-old male patient, in whom rotational atherectomy was performed for recurrent CAD following surgical cyst excision and patch angioplasty is reported. The patient's symptoms recurred one day after the rotational atherectomy procedure and repeat computed
