Increase in spleen volume of rats on experimental model of pre-hepatic portal hypertension.

BACKGROUND: Pressure increase in the portal venous system, caused by reduction of blood supply to the liver, develops collateral circulation of splanchnic vessel that reaches the venous system of superior vena cava. The spleen works as an auxiliary capacitance system which compensates the increase in portal pressure that may cause splenomegaly. AIM: To reproduce experimental model of pre-hepatic portal hypertension by reducing the caliber of the vein, allowing the study of alterations in the rat's spleen in this situation. METHODS: Fifteen Wistar rats were divided into three groups of five.

Idiopathic hepatic arterio-portal fistula: report of one case.

Hepatic arterio-portal fistula is a rare cause of portal hypertension in children; it is an abnormal communication of hepatic artery and portal venous system, the most common causes being trauma or malignancy. There were only 11 cases reported in English literature and were not ever reported in Taiwan. We report a 9-year old boy with idiopathic hepatic arterio-portal fistula presented as intractable hematemesis due to esophageal and gastric varices. He had received sclerotherapy twice, and Sugiura operation (resection of the lower part of esophagus, devasculization of the stomach and splenecto

Pancreatic Pseudocyst-portal vein fistula with portal thrombosis and biliary obstruction: a rare complication of necrotising pancreatitis.

BACKGROUND: Pancreatic pseudocyst fistulation into the portal venous system is rare. This may cause portal-vein thrombosis, portal hypertension, and biliary obstruction, complicating diagnosis and treatment. CASE: A 45-year-old man with alcohol-induced necrotizing pancreatitis developed jaundice, pain, and abnormal liver tests. Imaging revealed a 6-cm pancreatic head pseudocyst connected to the portal confluence and thrombosed portal and splenic veins. CT showed cavernous transformation and bile duct compression. Pancreatic pseudocyst-portal vein fistula (PPVF) with portal hypertension-relate

Fibrous trabeculae in the liver of alligator (alligator mississippiensis).

In the mature alligator, fibrous trabeculae run from the portal areas and capsule through the hepatic parenchyma. The extent of these trabeculae becomes clear only after staining for collagen with, for example, Fast green or Picrosirius red. The trabeculae are less well developed in young caiman. The alligator's liver might use the trabeculae to withstand thrashing of the body.

Portal pyaemia secondary to colonic perforation.

Portal pyaemia or pylephlebitis is a form of septic (often suppurative) thrombophlebitis of the portal venous system. It may develop as a complication of intra-abdominal sepsis, such as diverticulitis or appendicitis. Patients typically present with a high fever that is sometimes accompanied by jaundice. We report a case of portal pyaemia associated with multiple liver abscesses and discuss the medical and surgical management of this condition.

Effect of sirolimus on liver cirrhosis and hepatic encephalopathy of common bile duct-ligated rats.

Cirrhosis is often associated with portal hypertension and portal-systemic collateral vessels formation attributed to angiogenesis, which leads to severe complications as hepatic encephalopathy. Sirolimus has anti-fibrosis and anti-angiogenesis effects, but whether it influences the severity of portal-systemic collaterals and hepatic encephalopathy is unknown. This study was thus designed to address this issue in rats with common bile duct ligation-induced liver cirrhosis. Sham-operated rats were surgical controls. Rats were intraperitoneally administered with 0.5 and 2 mg/kg/day sirolimus or

Construction and Evaluation of a Subcutaneous Splenic Injection Port for Serial Intraportal Vein Cell Delivery in Murine Disease Models.

The liver is the largest internal organ and the center of homeostatic metabolism. Liver-directed cell transplantation is, therefore, an attractive therapeutic option to treat various metabolic disorders as well as liver diseases. Although clinical liver-directed cell transplantation requires multiple cell injections into the portal venous system, a mouse model is lacking which allows us to perform repetitive cell injections into the portal venous system. Here, we propose a surgical model that utilizes the spleen as a subcutaneous injection port. Mouse spleens were translocated under the skin w

What's new in portal hypertension?

Portal hypertension is defined as increased pressure in the portal venous system. The most common cause of portal hypertension is cirrhosis. In this setting, there is an increase in intrahepatic resistance leading to an increase in portal pressure. By increasing portal blood flow, splanchnic vasodilation further aggravates portal hypertension. New pathogenic pathways are being established which might result in new therapeutic strategies. The presence of varices at endoscopy and/or other abdominal portosystemic collaterals confirms the diagnosis of portal hypertension. The role of non-invasive

Pathogenesis of pipe-stem fibrosis of the liver (experimental observation on murine schistosomiasis).

Mice infected with 30 cercariae of Schistosoma mansoni developed portal and septal fibrosis due to the massive and concentrated deposition o … s normal architecture. Vascular injection techniques with Indian ink and vinylite revealed that the portal system developed numerous dilated collateral venules coming from the large and medium-sized portal branches, about 10 weeks after schistosome infection. The lodging of schistosome eggs into these collaterals resulted in granulomatous inflammation and fibro …

Sonographic anatomy of the hepatic artery.

One hundred clinical abdominal sonographic examinations were assessed to determine the frequency with which various parts of the hepatic artery system were imaged. The common, proper, and right hepatic arteries were seen in a large percentage of cases (92%, 75%, and 73%, respectively). Sixty-one consecutive patients with dilated biliary ducts were evaluated to determine the relationship of hepatic artery branches to the biliary tree and portal venous system. Normal and variant hepatic artery anatomy is discussed. Knowledge of the hepatic artery system and its variations is useful in the sonogr

Direct percutaneous access to an omental vein for embolization of Roux-en-Y limb varices in a child.

An 11-year-old boy with a history of hepatoblastoma treated with chemotherapy, radiation therapy, and liver transplantation presented with bleeding from Roux-en-Y limb varices. The transhepatic approach for portal intervention posed a risk of liver graft injury. An omental vein that was dilated as a collateral vein due to portal hypertension was found and compressible under ultrasound. The omental vein was percutaneously punctured, and the varices were embolized through a jejunal vein. No complication occurred. Direct percutaneous access to the portal venous system is a useful technique for po

Neonatal vitelline vein aneurysm with thrombosis: prompt treatment should be needed.

Vitelline veins are a pair of embryonic structures. The veins develop the portal vein system. Serious problems occur if the vitelline vein does not regress and becomes an aneurysm. Thrombus formation in the vitelline vein aneurysm could lead to portal vein thrombosis and portal hypertension unless promptly and correctly treated. Though vitelline vein aneurysm is an extremely rare anomaly, it rapidly progresses to portal vein thrombosis that requires prompt diagnosis and treatment. We reported a case of neonatal vitelline vein aneurysm and thrombosis that was cured by prompt operation.

The Role of the Spleen in Portal Hypertension.

As liver disease progresses, intrahepatic vascular resistance increases (backward flow theory of portal hypertension) and collateral veins develop. Adequate portal hypertension is required to maintain portal flow into the liver through an increase in blood flow into the portal venous system (forward flow theory of portal hypertension). The splenic artery resistance index is significantly and selectively elevated in cirrhotic patients. In portal hypertension, a local hyperdynamic state occurs around the spleen. Splenomegaly is associated with a poor prognosis in cirrhosis and is caused by splee

Impact of Regional Hepatic Immunosuppression With Budesonide on Bacterial Elimination in Porcine Abdominal Sepsis.

INTRODUCTION: The liver is essential for bacterial elimination, preventing intestinal bacteria from entering the systemic circulation, and for producing inflammatory cytokines. Glucocorticoids have been reported to exert heterogeneous effects on bacterial clearance in sepsis. Using a porcine model of gram-negative abdominal sepsis, we investigated how portal venous administration of budesonide, a glucocorticoid with extensive hepatic first-pass metabolism, affects hepatic bacterial and endotoxin elimination as well as the systemic inflammatory response, in comparison with systemic administrati
