The purpose of this study was to assess the clinical and radiographic outcomes of total hip arthroplasty (THA) in patients who had osteonecrosis to see if prior hip preserving surgery affected outcomes. Implant survivorship, Harris hip scores, and radiographic outcomes were compared between 87 patients (92 hips) who had undergone prior hip preserving procedures and 105 patients (121 hips) who had only undergone THA. Corneal neurotization is a fast-evolving surgical procedure for sensory reinnervation in neurotrophic keratopathy. After neurotization, prior reports document return of corneal sensation on average 8 months after surgery with 38 mm of sensation gain measured via cochet bonnet esthesiometer testing. Here, the authors describe a dual nerve grafting approach via simultaneous parallel sural nerve grafts from both the supratrochelar and supraorbital nerves to the affected contralateral cornea with return of sensation by postoperative week 11.

Nasal bone stabilization, in the setting of comminuted nasal fracture or surgical osteotomy, represents a challenging surgical experience. Postoperative shifting of osseous fragments may result in compromised outcomes in an otherwise well-performed procedure. Although prior studies have reported nasal bone fixation with implementation of wires, plates, or halos, these techniques are often difficult to employ routinely. Herein, the authors describe a novel and facile technique for the maintenance of unstable nasal bones using customized intranasal bolsters.

Serous macular detachment occurs in nearly 50% of eyes with optic disc pit. These macular detachments have been historically difficult to treat. The authors report a case of macular retinal detachment due to an occult juxtapapillary retinal break in a patient with optic disc pit refractory to prior treatments. This case report describes the unusual finding of bilateral Marcus Gunn jaw-wink. Only a few bilateral jaw-wink patients have been published as case reports. The authors dispute this notion as portrayed in the literature that a bilateral wink is a rare entity. A thorough review of prior case series on congenital ptosis and jaw-wink demonstrates that bilateral involvement is more common than previously thought. A 6-month-old infant with congenital icthyosis was brought to the authors with bilateral cicatricial ectropion of the upper and lower eyelid with resultant exposure keratopathy. Skin autograft was not possible due to generalized skin involvement, leading to unavailability of a donor site. A maternal skin allograft was performed for both eyelids of each eye. Prior human leukocyte antigen typing was not done, and postoperative immunosuppressive agents were not given. At 10 months' follow-up, the graft was well accepted, and the ectropion was corrected.

Tarsal tunnel surgery complicated with ganglia or any other type of cystic mass can be a very challenging operation. Preoperative planning before any tarsal tunnel surgery involving a soft-tissue mass is imperative. Plans to reconstruct the posterior tibial nerve and/or artery should be in place. The authors will present a case study that involved tarsal tunnel syndrome with an associated ganglion in the tarsal canal. They will review what microsurgical techniques and equipment should be on hand prior to performing this complicated surgical procedure.

The viability of using buprenorphine to treat opiate dependence was well documented prior to federal approval in October 2002. What has been lacking in the literature is "hands-on" experience of providers from a clinical management and practice management perspective. This article adds to the knowledge base by providing information about buprenorphine treatment as well as anecdotes from patients treated by the authors, leading to a detailed list of factors worth considering for the treatment provider contemplating adding an opiate-addicted population to an existing treatment base.

The authors anesthetized a 48-year-old woman with endometrial cancer and a large ovarian cyst. She developed cardiac failure initially followed by the sick sinus syndrome and A-V block from hypertrophic cardiomyopathy, prior to neuromuscular symptoms.
