The acute and long term responses to corticosteroid treatment in 65 patients with pulmonary eosinophilia have been reviewed. Of the 247 episodes of pulmonary eosinophilia that were documented during a median follow up period of 14 years, 186 were treated with prednisolone. Complete clearing of chest radiographic infiltrates occurred in 65% of the 247 episodes, partial clearing in 25%, and no response in 9%. Over a two year interval, we have treated five children with neurofibromatosis who developed brainstem gliomas. They had an insidious onset of symptoms and other indicators of low grade tumor despite several features that have been ascribed to rapidly progressive brainstem gliomas. Following standard radiation therapy a more protracted improvement was observed with the persistence of preradiation findings for several months. This experience confirms the better prognosis in brainstem glioma associated with neurofibromatosis.

The functional outcome of cerebral hemodynamics in the chronic stage of juvenile hydrocephalus was determined using single photon emission computed tomography (SPECT). Lessons learned from a 20 year prospective study of 100 Rheumatoid Arthritis (RA) patients are considered. Forty six had died and the causes … air or bed (Steinbröcker grade IV) and 24% markedly restricted (grade III). These figures show that prognosis of RA patients referred to a rheumatologist is probably worse than generally suggested. Rheumatoid arthritis is a severely disabling and potentially lethal disease.

In a consecutive screening of 2,401 newborn, a foot deformity was noted in 100 of the infants (4.2 percent). At follow-up 5-6 years later, the children with a foot deformity were reexamined and compared with normal controls. Seventy-six infants had some adduction deformity of the foot at birth. This study reviews complications of treatment in 123 mallet fingers treated operatively and nonoperatively. In the 84 digits splinted there was a 45% rate of complications, mostly skin related; however, these complications were almost always transient. For the 45 surgically treated digits the complication rate was 53%, with 76% of these complications still present at a mean follow-up of 38 months. Forty-eight wheezy infants were followed up for 25 to 44 months. These infants were classified into three groups: those with asthma (developed asthma later), the wheezy group (had successive wheezing episodes), and the non-wheezy group (grew out of the wheezy episodes). Serum IgE levels at the first visit were not significantly different in the three groups, but the frequency of exposure to cigarette smoke was higher in the asthma and wheezy groups than in the nonwheezy group.

The medical histories of a group of 511 patients hospitalized for schizophrenia were examined for the occurrence of perinatal distress. These patients were part of a 40-year follow-up study, so that their prognostic outcome was known at the time of this study. In this way, 200 cases of chronic schizophrenia were compared with 311 cases of psychotics with good prognosis. It was found that a history of perinatal distress was much more common in the poor prognosis group than in the good prognosis group. This difference could not be accounted for by age or family history of psychiatric illness.

Utilising the population based data resources of the Rochester Epidemiology Project, we estimated survival and risk of subsequent colon cancer in the 182 residents of Rochester, Minnesota, initially diagnosed with chronic ulcerative colitis (CUC) between 1985 and 1979. Twenty five (13.7%) had a proctocolectomy during the course of follow up. Three patients developed colorectal adenocarcinoma after the initial diagnosis of CUC (relative risk = 1.9, 95% CI 0.4-5.4). Correlation between tinnitus and audiometric loss and modification of the former by both surgical and medical treatment provide sufficient data to suggest its origin and to give the patient a prognosis confirmed in practically every case. Audiometric localization of the tinnitus is essential, since this allows establishment of the etiology, treatment and prognosis of the symptom. When tinnitus is irreversible, the hope of a possible recovery must be given to the patient to avoid the inevitable anxiety produced.

During a 15-year period 67 renal transplantations were performed in 60 patients aged 3 to 19 years. Cadaver donors were preferred (90%). By giving conventional immunosuppressive drugs (corticosteroids, azathioprine), graft survival was increased dramatically, attaining 82% after 1/2-5 years. Growth retardation remained a critical complication. Full rehabilitation was noted in 88% of all patients followed up for more than one year.

Precise information was obtained by a computerized analysis of the treatment, prognosis and clinical outcome of 108 representative cases selected from a series of 160 patients with Horton's disease (HD) diagnosed between 1970 and 1981. At the end of the study, 18 patients had died (16,6 p. 100), the commonest cause of death being a cerebrovascular accident.
