Rehabilitation of an edentulous atrophic maxilla with four unsplinted narrow diameter titanium-zirconium implants supporting an overdenture.

The edentulous maxilla is often affected by bone resorption, sometimes making it difficult to place standard diameter implants. Narrow diameter implants made of titanium-zirconium (Ti-Zr) alloy, which has superior mechanical properties compared with titanium, have been proposed for these difficult situations. This retrospective clinical observation reports the outcome of the use of reduced diameter implants made of Ti-Zr alloy supporting maxillary overdentures retained with locator abutments. The charts of all patients who received maxillary overdentures supported by four unsplinted implants f

A utility analysis of dental implants.

By applying a utility (usefulness) scale termed the "Feeling Thermometer" to 111 edentulous patients, a measure of effectiveness (quality-adjusted prosthesis years) that can be compared across several treatment strategies was developed. The results suggest that the utility scale was a valid (known group, P < .05), reliable (intraclass correlation coefficient = .713) measure of patients' preferences for the three treatment strategies: (1) conventional complete dentures (controls, n = 77), (2) transosteal implant-supported prostheses (n = 19), and (3) endosseous implant-supported prostheses (n =

Extraction, immediate-load implants, impressions and final restorations in two patient visits.

BACKGROUND: Since the advent of immediately loaded implants, patients have experienced shorter treatment times, reduced amount of surgical therapy, preserved gingival esthetics and the comfort and security of fixed prostheses. Practitioners have experienced less expense in the form of chair time and greater patient satisfaction. CASE DESCRIPTION: The authors present two cases in which, at the first clinical visit, they extracted maxillary incisors, placed hydroxyapatite-coated implants, took impressions for final restorations and stabilized the implants immediately with a fixed interim prosth

Implant-surgical and prosthetic rehabilitation of patients with multiple dental aplasia: a clinical report.

The expanded experience with oral implants and supplementary augmentation techniques has opened new possibilities for treating patients with oligodontia or anodontia with fixed prostheses. A problem in treating such patients is the need to place implants in growing maxillae or mandibles, as many of these patients are children or adolescents. When implant treatment is postponed until the patient is full grown, dysfunctions become manifest, which necessitates extensive surgical measures to achieve a fixed prosthetic restoration. This report illustrates the problems associated with different conc

[Use of autotransplants and implants in ossiculoplasty].

The paper presents a comparative analysis of efficacy of ossiculoplasty (OP) using different transplants. A total of 202 operations were performed. Prostheses made of the fragments of the auditory ossicles, or a cortical layer of the temporal bone (n=81), of nail plate (n=56), titanium implants (n=65) were applied. A satisfactory result was achieved in 72-87% ossiculoplasties. Functional outcomes of OP did not vary significantly with type and material of the prosthesis. Main causes of poor OP results were fixation or displacement of the prosthesis with fibrous tissue; in case of titanium prost

Establishment of High Channel-Count Packaging in Active Implantable Medical Devices for Neuroprosthesis.

Neuroprostheses have contributed extensively to humankind's capacity to treat disease and injury. Hearing loss is routinely overcome; pain is controlled; and myriad neurological, sensory, motor, and psychological disorders are managed through interventions involving neuromodulation. Fabrication of active implantable medical devices (AIMDs) often relies on highly skilled operators, leading to high costs, reliability challenges, and limitations to further device miniaturization and manufacturing output. Extending AIMD benefits to other disorders may also require a substantial increase in the num

Anatomic and reverse shoulder prostheses in fracture sequelae of the humeral head.

OBJECTIVES: Arthroplasty for the proximal humerus problems secondary to fractures is troublesome, because of necrosis of the tubercles and the resulting insufficiency of the rotator cuff. The aim of this study was to investigate whether better results can be achieved with the differential use of anatomic and reverse shoulder prostheses, in comparison to the preoperative status. METHODS: Fifty-five patients with secondary fracture prostheses due to sequelae of fractures of the humeral head were followed. Anatomic prostheses were implanted in 36 cases (fracture sequelae types 1 and 2 according

Immunological responses to maxillary on-lay allogeneic bone grafts.

4 patients with severely resorbed maxillae underwent rehabilitation with on-lays of allogeneic femoral head bone grafts, titanium implants (Brånemark System) and fixed prostheses. Donors and recipients were matched according to blood groups (ABO/Rh). Potential immunological responses were monitored by determining autoantibodies in peripheral blood, and inflammatory reactions in the recipient site. Biopsies from the bone graft 6 months after grafting showed vital bone. After a mean observation period of 19.5 months, 22 of 23 implants were osseointegrated. None of the investigated autoantibodies

[Endoluminal vascular prostheses].

Endoluminal vascular prostheses that can be implanted by percutaneous routes represent the most recent development in vascular interventional radiology. Various commercially available types of prosthesis are presented and the construction principles and applications are described. At present secure indications for the implantation of endoluminal prostheses are limited to the elimination of aneurysms and arteriovenous fistulae of the large vessels near the trunk in sections that do not cross a joint. The wide use in peripheral occlusive diseases cannot yet be recommended because confirmed data

Management of peripheral giant cell granuloma around complete-arch fixed implant-supported prosthesis: A case series.

Abnormal peri-implant tissue response in the form of benign reactive lesions, such as peripheral giant cell granuloma and pyogenic granuloma, is a less frequent biologic complication associated with dental implant therapy. However, these lesions can cause gingival pain, swelling, and discomfort, as well as peri-implant bone loss and possible implant failure. Few reports in the dental literature have described these lesions around complete-arch fixed implant-supported prostheses. The purpose of this clinical report was to describe 3 distinct scenarios in patients with complete-arch fixed implan

How Is the Created Alveolar Space Finally Restored After Maxillary Anterior Segmental Distraction Osteogenesis?

OBJECTIVE: Maxillary anterior segmental distraction osteogenesis (MASDO) for cleft and palate patients is the alternative treatment option to correct malocclusion with maxillary arch deficiency and severe crowding. After MASDO, prosthetic considerations are crucial for patients with cleft lip and palate because it is related to facial esthetics and occlusal function. The objectives of this study were to investigate the final restoration type for created alveolar space by MASDO. METHODS: Thirteen patients with cleft lip and palate who underwent MASDO and orthodontic treatment from the years 20

Multivariate analysis of the influence of peri-implant clinical parameters and local factors on radiographic bone loss in the posterior maxilla: a retrospective study on 277 dental implants.

OBJECTIVES: The aim of the present study was to investigate whether peri-implant clinical parameters (modified plaque index (mPI), bleeding and/or suppuration on probing (B/SOP)) and local factors (type of prostheses, screw emergence, platform diameter, and abutment angulation) might contribute to the development of additional bone loss and peri-implantitis around dental implants. MATERIALS AND METHODS: Two hundred seventy-seven external hex connection implants placed in the posterior maxilla of 124 patients were retrospectively evaluated. They were divided into two groups: physiologic bone l

Surgical and minimally invasive treatment of ischaemic and non-ischaemic priapism: a systematic review by the EAU Sexual and Reproductive Health Guidelines panel.

Surgical treatments for ischemic priapism (IP) include shunts or penile implants. Non-ischemic priapism (NIP) is usually the result of penile/perineal trauma causing an arterial fistula and embolisation may be required. We conducted a systematic review on behalf of the EAU Sexual and Reproductive health Guidelines panel to analyse the available evidence on efficacy and safety of surgical modalities for IP and NIP. Outcomes were priapism resolution, sexual function and adverse events following surgery. Overall, 63 studies (n = 923) met inclusion criteria up to September 2021. For IP (n = 702),

Osteonecrosis secondary to internal fixation.

Allergies to metals as a cause of failure of joint prostheses has historically been a controversial issue. Loosening of a prosthesis may be due to trauma, infection of the surrounding area, or a faulty implantation of the device. However, cases of loosening of the device do occur without any history of the above criteria. This leads one to consider other explanations for this phenomenon, such as sensitivity of the tissues to one of the metals in the implant. The following presentation demonstrates that metal allergies are a surgical concern when considering joint prostheses, or internal fixati
