In vivo testing of an infection-resistant annuloplasty ring.

BACKGROUND: An infection-resistant surface incorporated into a prosthetic cardiac valve has great potential clinical applications. STUDY DESIGN: A sewing ring construct was created using ciprofloxacin-treated polyester. Then ciprofloxacin-treated and untreated constructs were implanted subcutaneously on the dorsum of rats and inoculated with Staphylococcus aureus. At 7, 14, and 30 days animals were sacrificed and the implants were retrieved. Each implant was assessed for frank purulence and gross tissue incorporation by a blinded observer. The implants were processed for conventional histolog

[Risk factors for prosthesis exposure in treatment of genital prolapse via the vaginal approach].

OBJECTIVES: Prosthetic reinforcement in the surgical repair of pelvic prolapse by the vaginal approach is currently on the increase. However, this technique is not without tolerance-related problems. The most frequently described complication is prosthesis exposure, including erosion and delayed healing. It is independent of a granuloma and a major infection as pelvic cellulitis. Its mechanism is associated with defective vaginal healing. The purpose of our study is to define the risk factors for exposure of the prosthetic material. PATIENTS AND METHODS: Two hundred and seventy-seven medical

MR imaging of infectious processes of the knee.

Infectious processes about the knee can result from the hematogenous spread of infection, spread from a contiguous source, direct implantation of pathogens, and prior surgery. Soft tissues, joints, and bones can be infected by bacteria, fungi, parasites, or viruses. MR imaging is useful in identifying cellulitis, abscess, septic arthritis, and osteomyelitis. The inherent tissue contrast provided by MR imaging allows for the delineation of soft-tissue infection and osteomyelitis. Therefore, MR imaging is a useful tool in evaluating the extent of infection, and in facilitating adequate debrideme

Long-term outcomes of bovine pericardium neo-aortic reconstructions in infected vascular fields.

INTRODUCTION: Infected aortic fields are among the most complex surgical challenges, often requiring urgent intervention and associated with a high risk of morbidity and mortality. Bovine pericardium offers a customisable, off-the-shelf option for in situ reconstruction using biological material. We evaluate long-term outcomes following emergency bovine pericardium neo-aorta reconstruction in infected aortic fields. METHODS: Prospectively collected data for all patients treated with bovine pericardium neo-aorta reconstructions from 2018 to date were analysed. The surgical approach included re

Dalbavancin for treating prosthetic joint infections caused by Gram-positive bacteria: A proposal for a low dose strategy. A retrospective cohort study.

OBJECTIVE: Gram-positive bacteria are the leading cause of prosthetic joint infection (PJI). Dalbavancin is a lipoglycopeptide with remarkable pharmacokinetic properties and high bactericidal activity against most Gram-positive bacteria. Although clear evidence regarding its effectiveness in bone and joint infections lacks, recent studies suggest a promising role of dalbavancin in PJI. METHODS: From June 1st 2016 to May 1st 2018, all patients diagnosed of PJI and treated with DAL alone or in combination with other drugs were retrospectively evaluated. Dalbavancin susceptibility of every isola

Enucleation in equids with and without the use of an orbital suture meshwork implant: a retrospective study of 37 cases.

This retrospective study on 37 horses was conducted to assess long-term complications, scarring, cosmetic appearance and owner satisfaction of horses that underwent transpalpebral enucleation with (n=26) or without (n=11) non-absorbable sutured meshwork prosthesis between 2006 and 2013. Postoperative complications related to the surgery were wound infection, discharge and dehiscence, and no significant difference between groups was found. In one horse, the meshwork had to be removed in order to treat a chronic fistula. Horses with infection or inflammation before surgery were more likely to en

Transesophageal echocardiographic diagnosis of a mycotic ascending aortic pseudoaneurysm as a source of embolism.

Pseudoaneurysm of the ascending aorta may develop after aortic injury from trauma or surgery. We report a case of dehiscence of an aortic graft leading to ascending aortic pseudoaneurysm that was diagnosed by transesophageal echocardiography. In addition, the transesophageal echocardiogram identified the pseudoaneurysm as the likely cause of a saddle embolus to the iliac bifurcation of the aorta. This case is also unique in that the patient's pseudoaneurysm was infected with fungus.

Infective endocarditis of a prosthetic mechanical valve caused by Trichosporon inkin in an immunocompetent patient: Case report.

BACKGROUND: Infective endocarditis caused by Trichosporon inkin is an exceptionally rare fungal infection, particularly in immunocompetent patients. This microorganism is part of the normal microbiota of the skin and gastrointestinal tract, but can cause invasive infections associated with prosthetic devices or complex clinical conditions. Reported cases in the literature are scarce, making it difficult to establish standardized diagnostic and therapeutic guidelines. CASE REPORT: We report the case of an immunocompetent female patient with rheumatic fever due to which she underwent a mitral v

Antimicrobial resistance and biofilm formation of penile prosthesis isolates: insights from in-vitro analysis.

BACKGROUND: Inflatable penile prostheses (IPPs) have been shown to harbor biofilms in the presence and absence of infection despite exposure to various antimicrobials. Microbes persisting on IPPs following antibiotic exposure have not been adequately studied to assess biofilm formation capacity and antibiotic resistance. AIM: In this study, we aimed to assess these properties of microbes obtained from explanted infected and non-infected IPPS using an in vitro model. METHODS: 35 bacterial isolates were grown and tested against various single-agent or multiple agent antibiotic regimens includi

Patient-Related Predictors of Treatment Failure After Two-Stage Total Hip Arthroplasty Revision for Periprosthetic Joint Infection: A Systematic Review and Meta-Analysis.

BACKGROUND: Periprosthetic joint infection (PJI) treatment has high failure rates even after 2-stage revision. Risk factors for treatment failure (TF) after staged revision for PJI are not well defined, nor is it well established how they correlate with the risks of developing an index PJI. Identifying modifiable risk factors may allow preoperative optimization, while identifying nonmodifiable risk factors can influence surgical options or advise against further surgery. We performed a systematic review and meta-analysis to better define predictors of TF in 2-stage revision for PJI. METHODS:
