Pulmonary involvement in Behçet's disease is an uncommon condition (12%). Thromboembolism of the superior vena cava and/or other mediastinal veins, aneurysms of the aorta and pulmonary arteries are the main vascular manifestations in addition to pulmonary infarct and intrathoracic hemorrhage. Despite their scarcity, respiratory symptoms may be life-threatening. The aim of this study was to assess the contribution of thoracic imaging for one of the most serious aspects of the disease: pulmonary artery aneurysm. We report five patients with pulmonary artery aneurysms (mean age: 39.5 years). Hemo

Pulmonary embolism (PE) is a disease with diverse clinical manifestations, which lacks specificity but may be life-threatening. We describe a 39-year-old male who presented with acute PE complicated by pulmonary infarction, confirmed by computed tomography pulmonary angiography 2 weeks following laparoscopic appendectomy. His initial symptom was right lower-back pain. This case underscores the diagnostic challenge posed by an atypical presentation of PE, in which isolated back pain was the sole initial manifestation. Furthermore, we delineate the pathophysiologic mechanisms that may account fo…

Described here is a rare cardiac complication attributed to mucormycosis in a 63-year-old woman who developed sudden cardiac arrest and pulmonary insufficiency in the course of being treated for acute monocytic leukemia (acute myelogenous leukemia, AML M5a). At autopsy, fresh thrombi were noted in the left pulmonary artery and the left atrium. Postmortem coronary angiography revealed complete occlusion of the circumflex branch of the left coronary artery, and histological examination showed a mucormycotic embolism in the corresponding portion. Multiple small mucormycotic thrombi were also note

Pulmonary dirofilariasis, caused by Dirofilaria immitis, rarely affects humans and is usually asymptomatic, but may present as chest pain, cough, hemoptysis, wheezing, low-grade fever, and malaise. The dead and dying worms obstruct branches of the pulmonary artery, causing infarction and a granulomatous reaction. Coin lesions are apparent on radiography, raising concern of malignancy. Complete surgical excision is the treatment of choice and an anthelmintic can be administered if residual lesions are present or the patient is from an endemic area. We present two cases of pulmonary dirofilarias

We report the case of a 60-year-old woman with a recent history of a cerebrovascular accident. Because of clinical suspicion of pulmonary embolism and negative Doppler ultrasound findings of the lower limbs, spiral computed tomography of the pulmonary artery was performed and demonstrated pulmonary emboli. We emphasize the role of computed tomography of the abdomen, performed 3 min after the thoracic acquisition, which showed an unsuspected thrombus within the abdominal aorta and the left renal artery with infarction of the left kidney. Paradoxical embolism was highly suspected on computed tom

In a retrospective study over the years 1978-1982, 729 cases of acute pulmonary embolism were analyzed in relation to history, clinical signs and laboratory findings and the results compared with the findings of the urokinase pulmonary embolism trial. As far as history and clinical symptoms were concerned, breathlessness, chest pain, tachypnea, tachycardia and cyanosis were the dominating features. Among laboratory tests, the radiological and electrocardiographic findings of pulmonary hypertension were of little value. In contrast, arterial hypoxemia and isotope scanning provided the most reli

BACKGROUND: Myocardial infarction is rare in children but frequently occurs unexpectedly with atypical presentation. It can cause a progressive lethal course unless prompt treatment is initiated.
METHODS: Paediatric cases of myocardial infarction diagnosed by the presence of ischaemic myocardial insults and symptomatic ventricular dysfunction were reviewed retrospectively.
RESULTS: Eighteen patients (5 days to 14 years of age; median 3 months) with myocardial infarction were studied. The aetiology was variable, including congenital coronary anomalies: anomalous left coronary artery from pulm

PURPOSE: To establish whether a relationship exists between computed tomography features of lung opacities in severely neutropenic patients … solidations. The presence of a perinodular ground-glass halo, the similarity of consolidations to a pulmonary infarction and the presence of cavitation (crescent-shaped or not) were recorded.
RESULTS: Invasive pulmonary aspergillosis was diagnosed in 68 patients; bacterial pneumonia in 56. Nodules (85) were more common than consolidations (39); their distribution amo …

Pulmonary veno-occlusive disease (PVOD) and pulmonary capillary hemangiomatosis (PCH) are rare causes of pulmonary hypertension, regarded by some as distinct entities. However, their presentations are similar and both are associated with poor prognoses. We therefore reviewed 38 specimens [autopsies (n=15), surgical biopsies (n=15), explants (n=7), and pneumonectomy (1 case)] from 35 patients diagnosed as either PVOD (n=30; av. age 34 y, range 4 to 68 y; 19M:11F) or PCH (n=5, av. age 42 y, ranging from 9 months to 60 years; 3M:2F) to assess their interrelationship. PCH was identified in 24 (73%

In a 70 years old worker in the bauxite industry in Surinam, who died of myocardial infarction, bauxite deposits were found in the lung in areas of mild pulmonary fibrosis. The identification and the localization within these areas of the bauxite deposits was determined by a histochemical mineralization technique and by scanning electron microscopy and electron microprobe analysis. The possible relationship between the presence of bauxite in the lung tissue and the pulmonary fibrosis is discussed.

Pulmonary atresia with intact ventricular septum and right ventricle-dependent coronary circulation confers a high risk for myocardial infarction and complications of ischaemia, including left ventricle aneurysms. We describe an infant with pulmonary atresia with intact ventricular septum and right ventricle-dependent coronary circulation and a left ventricle apical aneurysm who successfully underwent the Dor procedure and a subsequent Fontan operation. Surgery for postinfarction left ventricle aneurysm can be considered in infants.

The radiological features of invasive pulmonary aspergillosis, observed in 20 patients with hematologic malignancies, are reported. From the analyses of the initial signs and their modifications in the course of the disease it was possible to identify some aspects strongly suggestive of the mycotic nature of the pulmonary lesions. Rounded pneumonia and hemorrhagic infarction often complicated by cavitation with or without intracavitary nodule are the most suggestive aspects, and the latter carry good prognostic significance.

Five patients developed pleural empyema due to Clostridium perfringens in the absence of penetration of the thorax; two of the patients presented with pyopneumothorax. Thirteen additional cases from the literature are reviewed. Predisposing factors to the development of pleural empyema appear to include aspiration pneumonia, pulmonary embolization and infarction, and bacteremia from other foci. Pleural disease and pulmonary tuberculosis may also predispose patients to pleural empyema. Treatment consists of drainage and antimicrobial chemotherapy.

Pulmonary edema is a frequent and common cause of death in patients in critical care settings. It is seen as a complication of myocardial infarcts, hypertension, pneumonia, smoke inhalation, and high-altitude pulmonary edema. Pulmonary edema occurs when there are alterations in Starling forces and capillary permeability, opposition to lymphatic flow in the lungs, decreased plasma oncotic pressure, central nervous system lesions, and following some types of strenuous exercise. Pulmonary edema presents initially with crackles, wheezing, and dry cough and progresses to tachypnea, dyspnea, orthopn

Pulmonary embolism and massive hemoptysis caused by intravascular foreign bodies have rarely been reported. We report a case of an end-stage … treatment, the patient gradually recovered. After six months of follow-up, no pulmonary embolism or pulmonary infarction occurred. Our case report presents an alternative approach to extracting a foreign object from the pulmonary artery by locating the foreign object within the vascular terminations, without resorting to forceful removal. This method mitigates the pot …

Human pulmonary dirofilariasis has been documented from many parts of the world, but not in Korea so far. We experienced a patient of pulmonary dirofilariasis who had visited a local clinic because of chest pain for 1 month. On chest radiograph, a coin lesion of 2 cm diameter and enlargement of the mediastinal lymph node were shown. An exploratory lung resection was done. Pathologically the lesion was a pulmonary dirofilariasis complicated with necrotic pneumonia, fibrosis, and infarction. At the center of the lesion, degenerated nematode sections with multilayered cuticle, thick musculature,

Autopsy findings in 10 cases of high-altitude pulmonary edema have been collected from published articles and personal observations. All cases were males with a mean age of 37 years (22-62). The altitude of occurrence was from 8400 to 17 500 feet. The mean combined lung weight in nine cases was 1682 g (1200-3000 g). Cerebral edema was present in five of eight cases. The most frequency pulmonary findings in addition to diffuse edema consisted of leukocyte infiltrates, alveolar hemorrhages, thrombi in small pulmonary arteries, and alveolar hyaline membranes. Pulmonary infarction was present in o…

Patients with HES and pulmonary infiltrates may pose certain diagnostic problems as the infiltrates may be attributed to infection, infarction, congestive heart failure, or HES itself. We report an 87-year-old woman with idiopathic HES presenting with bibasal alveolar infiltrates. Differential cell count in BAL fluid yielded a very high percentage (73 percent) of eosinophils. Other authors previously mentioned the absence of eosinophils in the lavage fluid despite an important peripheral eosinophilia in a patient with the idiopathic HES but without HES-related pulmonary involvement. Thus, BAL
