An observational study of pre-operative transcranial Doppler examinations to predict cerebral hyperperfusion following carotid endarterectomies.

The objective of this observational study was to determine whether pre-operative transcranial Doppler measurements could identify patients at risk for early cerebral hyperperfusion following carotid endarterectomies. Fifty-five patients (mean age 64.4 years) with symptomatic internal carotid artery stenosis were included. Pre-operative transcranial Doppler measurements included middle cerebral artery blood flow velocities at normocapnia, pulsatility indices and estimation of the cerebro-vascular reserve capacity ipsilateral to the internal carotid artery stenosis. The percentage change in bloo

Pulsatile Release of Human Gonadotropin Releasing Hormone Using a Photoactivated Depot Containing an Unusual Light Cleaved Functionality.

In this work, we demonstrate that the photoactivated depot (PAD) approach can be used to produce pulsatile release of human gonadotropin releasing hormone (GnRH) using green light. Pulsatile release is critical for a range of hormones and produces distinct physiological responses in comparison with continuous release. In particular, this has been observed with the use of GnRH for the treatment of cognitive impairment in Down syndrome. We have synthesized a GnRH PAD material utilizing a green light activated nonpolar tag based on the thiocoumarin photocleavable group. This material is essential

Evaluating pulsatile flushing by pushing method and catheter size for educating nurses on peripheral vascular patency: a computational fluid dynamics simulation analysis.

BACKGROUND: Maintaining the patency of peripheral intravenous catheters is essential for preventing occlusion and catheter-related complications. Clinical guidelines and field education emphasize the effectiveness of pulsatile flushing for this purpose. However, due to the absence of standardized methods, its implementation among clinical nurses remains low. This study aimed to compare the effects of pulsatile flushing according to different infusion methods and catheter sizes to support the education of clinical nurses on intravenous infusion therapy. The findings aim to provide foundational

Modeling the hepatic arterial flow in living liver donor after left hepatectomy and postoperative boundary condition exploration.

Preoperative and postoperative hepatic perfusion is modeled with one-dimensional (1-D) Navier-Stokes equations. Flow rates obtained from ultrasound (US) data and impedance resulted from structured trees are the inflow and outflow boundary condition (BC), respectively. Structured trees terminate at the size of the arterioles, which can enlarge their size after hepatectomy. In clinical studies, the resistance to pulsatile arterial flow caused by the microvascular bed can be reflected by the resistive index (RI), a frequently used index in assessing arterial resistance. This study uses the RI in

Fluid flow in distensible vessels.

1. Flow in single vascular conduits is reviewed, divided into distended and deflated vessels. 2. In distended vessels with pulsatile flow, wave propagation and reflection dominate the spatial and temporal distribution of pressure, determining the shape, size and relative timing of measured pressure waveforms, as well as the instantaneous pressure gradient everywhere. Considerable research has been devoted to accessing the information on pathological vascular malformations contained in reflected waves. Slow waves of contraction of vessel wall muscle, responsible for transport of oesophageal, ur…

Decreased LH pulsatility during initiation of gonadotropin superovulation treatment in the cow: evidence for negative feedback other than estradiol and progesterone.

LH pulse secretion is suppressed during superovulation of cattle. The objective of this study was to determine how soon after initiation of superovulation treatments this suppressive effect occurs, and to test the hypothesis that decreased LH pulsatility is not related to changes in circulating estradiol or progesterone. Heifers (n = 7/group) were injected with eCG (FOLLIGON: a single injection of 2,500 IU) or twice daily injections of decreasing doses of FOLLTROPIN-V (total equivalent of 280 mg of NIH-FSH-P1) or F.S.H.-P (total equivalent of 28 mg of Armour standard) or saline (time controls)

Effect of pattern of administration on the response to exogenous pituitary-derived chicken growth hormone by broiler-strain pullets.

The effect of pulsatile versus continuous intravenous administration of exogenous, pituitary-derived chicken growth hormone (cGH) on growth performance and endocrine/metabolite status of broiler-strain pullets was determined. In a first study, 8-week-old pullets, surgically prepared with intravenous catheters and maintained via a fluid swivel/spring tether/harness system, were administered cGH or vehicle (control) over a 10-min period every 90 min (i.e., 90-min pulse pattern) for 21 consecutive days. Feed intake, body weight gain, and carcass yield and composition were determined in conjunctio

The value of two-dimensional echocardiography and pulsed Doppler technique in facilitating percutaneous catheterization of the subclavian vein.

We have found combined two-dimensional echocardiographic imaging and range-gated pulsed Doppler flow mapping to be very useful and accurate in identifying the location and course of the subclavian vein. It can be efficiently utilized to minimize the incidence of iatrogenic complications during percutaneous central venous catheterization. This is particularly relevant in patients who are predisposed to a high risk of complications from the procedure.

First evaluation of a novel pulsatile LVAD: Feasibility and haemodynamic impact in acute heart failure.

AIMS: Non-pharmacological therapies for acute decompensated heart failure (HF) and cardiogenic shock have evolved considerably in recent decades. Short-term mechanical circulatory support (MCS) devices can be used as circulatory backup. While nearly all available devices use continuous flow, evidence indicates that pulsatile flow can be more effective. This study presents the first experimental use of a novel counter-pulsatile left ventricular (LV) assist device (LVAD) with a primary focus on assessing its feasibility and effectiveness. METHODS: The pulsatile ventricular assist platform (pVAP…

Left ventricular assist device in patients with body mass index greater than 30 as bridge to weight loss and heart transplant candidacy.

INTRODUCTION: In obese patients with heart failure, weight reduction may be difficult due to physical restrictions, but may be necessary to achieve heart transplant candidacy. We report the outcomes of obese patients who underwent implantation of a left ventricular assist device (LVAD) using a pulsatile (HeartMate XVE [XVE]) or continuous flow (HeartMate II [HMII]) design and the effect on body mass index (BMI). METHODS: Of 37 patients with BMI >30 kg/m(2) who underwent LVAD implantation, 29 survived at least 30 days and were followed for weight change. In the 30-day survivors, end points of

Measures to Prevent Neurologic Deficits in Urgent CABG Surgery With Bilateral Carotid Occlusion.

Patients with bilateral high-degree carotid stenosis or occlusion impose high risk for neurologic complications during coronary artery bypass graft surgery (CABG). Former articles have described successful CABG in patients with bilateral carotid artery occlusion with uneventful recovery, with perioperative cerebral blood flow monitoring consisting of electroencephalography or near-infrared spectroscopy. In this case report, we describe the use of pulsatile flow on cardiopulmonary bypass and transcranial Doppler monitoring during successful CABG in a patient with bilateral carotid occlusion, le…

Successful Hemodialysis Arteriovenous Fistula Creation in a Patient With Continuous-Flow Left Ventricular Assist Device Support.

Heart failure necessitating left ventricular assist device (LVAD) support can lead to kidney failure requiring dialysis. Some of these patients may require long-term hemodialysis (HD). Optimal vascular access for a patient on long-term HD therapy with an LVAD remains a complex issue. The majority of LVADs are of the continuous-flow type, and it has been theorized that native arteriovenous fistula maturation may be impaired in a setting of decreased pulsatile arterial flow. We describe a case of successful creation and use of an arteriovenous fistula in an HD-dependent patient with a continuous

Clinical utility of imaging for pulsatile tinnitus.

INTRODUCTION: Pulsatile tinnitus affects millions of patients annually, yet the prevalence of time-sensitive imaging findings remains unclear. There is limited consensus regarding initial imaging workup between radiological and otological guidelines; diagnostic algorithms have been proposed but are based on strict patient criteria. We aimed to elucidate the prevalence of time-sensitive imaging findings in the workup of pulsatile tinnitus and assess imaging associations with outcomes. METHODS: This retrospective study included a cohort of 100 randomly sampled patients from academic medical cen

Hypoplastic left heart syndrome: report of an operation utilizing the ductus arteriosis in outflow reconstruction.

An operation utilizing the ductus arteriosus to construct an outflow tract from the right ventricle is described. This operation uses the right ventricle for pulsatile flow to both systemic and pulmonary arteries, provides complete reconstruction of the ascending aorta and arch, and delivers predictable pulmonary blood flow. Although unsuccessful in this case report, which involves a hypoplastic left heart syndrome patient, the technique may provide successful palliation with the use of ductal tissue. This method has not been previously described.
