Lack of Racial Diversity in Surgery and Pathology Textbooks Depicting Diseases of the Breast.

INTRODUCTION: The lack of racial diversity depicted in medical education texts may contribute to an implicit racial bias among clinicians. This bias influences outcomes, as familiarity with the various cutaneous manifestations of disease is essential to making an accurate diagnosis. To better understand the racial disparities in breast surgery, we sought to determine the extent of skin tone representation depicted in images of breast surgery and pathology textbooks. METHODS: Textbooks were screened for color images of conditions with sufficient skin tissue present to assign the Fitzpatrick sk

Experiencing racial discrimination increases vulnerability to PTSD after trauma via peritraumatic dissociation.

Background: Racial discrimination is a traumatic stressor that increases the risk for posttraumatic stress disorder (PTSD), but mechanisms to explain this relationship remain unclear. Peritraumatic dissociation, the complex process of disorientation, depersonalization, and derealization during a trauma, has been a consistent predictor of PTSD. Experiences of frequent racial discrimination may increase the propensity for peritraumatic dissociation in the context of new traumatic experiences and contribute to PTSD symptoms. However, the role of peritraumatic dissociation in the relationship betw

The Index of Concentration at the Extremes (ICE) and Pregnancy-Associated Mortality in Louisiana, 2016-2017.

OBJECTIVES: Social and contextual factors underlying the continually disproportionate and burdensome risk of adverse health outcomes experie … her factors. CONCLUSIONS FOR PRACTICE: In addition to continuing to consider the deeply entrenched racism and economic inequality that shape the experience of pregnancy-associated death, we must also consider their synergistic effect on access to resources, maternal population health, and health inequities.

Dual-process theory of racial isolation, legal cynicism, and reported crime.

Why is neighborhood racial composition linked so strongly to police-reported crime? Common explanations include over-policing and negative interactions with police, but police reports of crime are heavily dependent on resident 911 calls. Using Sampson's concept of legal cynicism and Vaisey's dual-process theory, we theorize that racial concentration and isolation consciously and nonconsciously influence neighborhood variation in 911 calls for protection and prevention. The data we analyze are consistent with this thesis. Independent of police reports of crime, we find that neighborhood racial

COVID in New York City, the epicenter: A New York University perspective and COVID in Duluth, the Bold North: A University of Minnesota perspective.

In the first part of this dual commentary, perspectives are gathered from students at New York University. COVID in New York City is compared with the trauma of the World Trade Center. Triggers, racism, concerns, acts of kindness, and the future are considered for both. The second part of the commentary is written in juxtaposition to the New York City perspective by sharing the Upper Midwest experiences of this COVID-19 pandemic period, particularly in the small city of Duluth. Perspectives are gathered over time from students at the University of Minnesota, Duluth. The Minnesota stories are p…

Invisible Bruises: Theoretical and Practical Considerations for Black/Afro-Latina Survivors of Childhood Sexual Abuse.

This article focuses on the intersectionality of race, gender, and violence in the lives of Black/Afro-Latinas who have experienced childhood sexual abuse. First, we discuss the trauma of slavery, racism, and colonization in Latin America and the Caribbean, which objectifies Black bodies and renders Blackness invisible. Second, we identify institutional and interpersonal barriers to seeking trauma support. Third, we present preliminary findings from a Listening Circle among survivors who participated in the Black Latinidad: Building Siblinghood to End Child Sexual Abuse Project. Finally, we pr…

From Thomas Mann to John Green, how authors and books shape our understanding of TB.

Over the centuries, from Thomas Mann to John Green, many authors have attempted to tell the story of TB, or stories of people with TB, via books, poems, art, and films. Books, in particular, have helped us better understand the disease, its history, and its impact on people. This Editorial is a celebration of a selection of books and authors who have made important contributions, broadly grouped under four categories: historical; fiction; personal narratives; contemporary. Collectively, these books underscore the fact that TB has always thrived on racism, prejudice, poverty, and injustice, and…

Racial, Ethnic, and Socioeconomic Disparities in Cardio-Obstetrics.

Racial, ethnic, and socioeconomic disparities persist in American medicine. Black pregnant people die from pregnancy-related causes at 2 to 3 times the rate of their White counterparts, with cardiovascular disease as the leading contributor. These disparities in cardio-obstetrics arise from intersecting structural forces-economic inequity, residential segregation, limited health care access, implicit bias, and the cumulative physiological toll of structural racism. Effective solutions require a multifaceted action: community health worker programs, group prenatal care, hospital quality improve…

The association between state-level negative racial sentiment and maternal hypertension in the US from 2016 to 2021: An observational study using Twitter data.

BACKGROUND: Racial disparities in maternal hypertension (i.e., prepregnancy and gestational) due to experiences of racism may contribute to ongoing racial disparities in US birth outcomes, including low birth weight and preterm birth. Despite evidence linking racism and adverse birth outcomes, no studies have examined the plausible association between area-level negative racial sentiment and disparities in maternal hypertension. To address this gap, we used 2016-2021 US birth certificate data to examine the associations between state-level Twitter-derived negative sentiments toward racial/ethn…

Family and community influences on the social and sexual lives of Latino gay men.

The purpose of this study was to explore the effect of family and community on the social and sexual lives of a group of Latino gay men living in a metropolitan area. A secondary analysis of four focus groups with 28 Latino gay men was conducted. Families had a difficult time acknowledging and supporting participants' homosexuality. Participants experienced racism, discrimination, and physical and verbal abuse as a result of their ethnicity and homosexuality. These negative effects contributed to their marginalization and made them vulnerable to depression and suicide. Health care professional…

Sexual risk as an outcome of social oppression: data from a probability sample of Latino gay men in three U.S. cities.

Based on data from a probability sample of 912 Latino gay men in 3 U.S. cities, a multivariate model of sexual risk was tested, including experiences of homophobia, racism, and poverty as predictors. Participants reported multiple instances of verbal and physical abuse, rude mistreatment, and discrimination on account of their sexual orientation and their race or ethnicity. Many reported experiences of poverty, such as inability to pay for basic necessities of food or shelter. Men who reported more instances of social discrimination and financial hardship were more psychologically distressed a…

Clinically Useful Family Medicine Research.

What is next for family medicine? After the long, rough road since the beginning of the COVID-19 pandemic, other topics are again receiving renewed attention. Family medicine researchers continue to consider issues important to our patients and practices. There is a collection of clinical research on children's health care. One article outlines practical actions to move medical academia past racism. The need for physician trust in patients is also often overlooked. Other articles address how to improve the practice of family medicine and a framework for thinking about legal and ethical issues …
