Radionuclide evaluation of the interventricular septum following coronary artery bypass surgery.

The cases of 25 consecutive patients who had undergone radionuclide ventriculography both before and after coronary artery bypass surgery and who had had normal septal motion before surgery were reviewed. Abnormal septal motion was present in 22 patients (88%) postoperatively. In contrast, a new motion abnormality appeared following surgery in the apical segments of only 5 of 21 patients (24%) and in the lateral segments of 1 of 22 (5%) patients. All patients improved clinically following surgery, and only two had evidence of intraoperative myocardial infarction. Postoperatively, thallium imag

Technetium-99m MAG3 renal scintigraphy with demonstration of aortic dissection.

Demonstration of type B aortic dissection is rare during renal scintigraphy. The discordant radionuclide pattern of asymmetric renal flow with equivalent renal function and excretion has been previously reported in aortic dissection. However, delayed scintigraphic features of the false lumen have not been described. The author presents such a case with persistent technetium-99m MAG3 accumulation in the posterior mediastinum on postvoid planar imaging.

Inadvertent ligation of the left pulmonary artery instead of patent ductus arteriosus. Noninvasive diagnosis by pulmonary perfusion imaging.

Pulmonary perfusion scintigraphy was used to noninvasively evaluate a neonate four days after attempted surgical closure of a large patent ductus arteriosus (PDA). No perfusion to the left lung was seen, suggesting inadvertent ligation of the left pulmonary artery (PA). This was confirmed by cardiac catheterization and reoperation. Radionuclide pulmonary perfusion imaging is an ideal noninvasive technique for evaluating pulmonary perfusion in neonates after complicated thoracic surgery.

PET in neurotherapeutic discovery and development.

Positron emission tomography (PET) is a highly sensitive, quantitative imaging technique that can track sub-nanomolar quantities of positron-emitting radionuclides throughout the body. By incorporating such radionuclides into molecules of interest, we can directly assess their pharmacokinetic and pharmacodynamic (PK/PD) characteristics in vivo without changing their physicochemical characteristics or eliciting a pharmacological response. As such, PET imaging has long been used as a tool to aid drug discovery programs from preclinical biomarker validation all the way through to clinical trials.

Scintigraphic quantitation of gastrointestinal motor activity and transport: oesophagus and stomach.

For the recognition and characterisation of oesophageal motor disorders, manometry represents the most reliable tool but yields no information on bolus transport. The transport can be quantitated by radionuclide techniques. The patient is positioned supine beneath a gamma-camera and instructed to swallow a radiolabelled bolus in a single gulp. Using a marker over the cricoid and the activity in the stomach as landmarks, regions of interest are drawn representing the upper, middle and lower third of the oesophagus and the gastric fundus. Activity-time curves enable one to recognise the clearanc

[Imaging features of osteomyelitis of the jaws with different diagnostic methods. A review].

Osteomyelitis of the jaws (OM) is an infectious pathology involving the bone marrow. It is due to various etiologies, the main one being odontogenic. The diagnosis of OM is a challenge for dentists, who must rely on clinical and imaging examinations. Panoramic radiography is the most frequently used imaging test, but it has some limitations that have improved with the incorporation of new imaging methods such as computed tomography, cone beam computed tomography, magnetic resonance imaging, and bone scintigraphy with radionuclides. The purpose of this review was to describe the imaging charact

Nuclear medicine in diagnosis and therapy of bone and joint diseases.

Concerning bone and joint diseases therapy of rheumatic synovitis (= radiosynoviorthesis) was introduced in 1952 before clinically relevant diagnostic procedures were developed. Radionuclides of Sr and later on 99mTc phosphonates then started the wide use of bone scintigraphy since > 30 years. The diagnostic methods have an excellent sensitivity for detection of local abnormalities of bone metabolism, the specificity of such studies, however, is low. Modifications of the technique (3-phase-bone-scintigraphy, pinhole collimators, ROI-technique), increasing knowledge of pathological scan pattern

Recent trends in utilization rates of abdominal imaging: the relative roles of radiologists and nonradiologist physicians.

PURPOSE: To study recent trends in the utilization of abdominal imaging in the Medicare population. MATERIALS AND METHODS: The Medicare Part B databases for 1996 through 2005 were reviewed, and all Current Procedural Terminology, 4th ed, codes pertaining to noninvasive imaging of the abdomen and pelvis were identified. The codes were grouped into 6 categories: (1) computed tomography (CT) and CT angiography, (2) magnetic resonance (MR) and MR angiography, (3) ultrasound, (4) radionuclide imaging, (5) plain radiography, and (6) gastrointestinal fluoroscopy. Global and professional-component cl…

Positron Emission Tomography in Atherosclerosis Research.

Positron emission tomography (PET) is a quantitative imaging technique that uses molecules labeled with positron-emitting radionuclides to visualize and measure biochemical processes in the tissues of living subjects. In recent years, different PET tracers have been evaluated for their ability to characterize the atherosclerotic process in order to study the activity of the disease. Here, we describe detailed PET methods for preclinical studies of atherosclerosis and summarize the key methodological aspects of PET imaging in clinical studies of atherosclerosis.

[Current radiotherapy for extrapulmonary tuberculosis].

Radiation studies are prominent in the diagnosis of extrapulmonary tuberculosis. X-ray study preserves its basic value. Its-based concepts of the pathology of osteoarticular, urogenital, lymphabdominal tuberculosis largely allow for analyzing the images obtained by new medical visualization techniques, such as X-ray tomography, computed tomography, magnetic resonance imaging, ultrasonography, and radionuclide studies. Each method has its merits and demerits and is used by a certain algorithm at different sites of extrapulmonary tuberculosis.

Bone scintigraphy in slipped capital femoral epiphysis.

Tc-99m diphosphonate bone scans were performed on 11 children with slipped capital femoral epiphysis. On pinhole hip images, seven hips in seven patients had increased radionuclide uptake in the physis and adjacent proximal femoral metaphysis where the slip had occurred. Three hips in three patients had decreased radionuclide uptake in the femoral head on the side of the slipped epiphysis, indicating compromise of the femoral head blood supply. Three or more months following internal fixation, three children had scintigraphy that showed loss of the usual focal uptake in the physis and adjacent

Radiolabelled somatostatin analogs for diagnosis and radio-guided surgery of neuroendocrine breast cancer undetectable with conventional imaging procedures.

Some neoplasms are classified as primary neuroendocrine tumours (NETs) because of their positivity for neuroendocrine markers [chromogranins A and B (CgA, CgB) and neuron-specific enolase (NSE)]. Neuroendocrine differentiation has been reported, for example, in both "in situ" and infiltrating breast cancer. Diagnosis of NET is bio-humoral (CgA, NSE, synaptophysin) and instrumental. Even if the final diagnosis is made by open biopsy, radionuclide imaging using radiolabelled somatostatin analogs, such as In-111 pentetreotide, may detect neuroendocrine primary tumours and metastases before they b…

Unusual false-positive radioiodine whole-body scans in patients with differentiated thyroid carcinoma.

Radioiodine whole-body imaging is the most accurate method in the diagnosis of metastases from differentiated thyroid cancer. However, false-positive images rarely occur. The authors report unusual cases of thymic hyperplasia and post-traumatic skull changes mimicking mediastinal, skull, or cerebral metastases. Nonthyroidal causes were diagnosed by other radionuclide studies (bone and brain scintigraphy) and CT scans. Follow-up and undetectable thyroglobulin levels helped confirm the benign cause.

Patient dosimetry in radionuclide therapy: the whys and the wherefores.

The importance and methodology of contemporary patient dosimetry in well-established radionuclide therapies are reviewed. The different protocols used for radioiodine treatment of thyrotoxicosis are discussed. Special attention is paid to patient dosimetry in the largest safe dose approach for curative radioiodine therapy of thyroid remnants and metastases in the post-surgical treatment of differentiated thyroid cancer. Nowadays, meta-[131I]iodobenzylguanidine (131I-MIBG) therapy for neuroblastoma relies on bone marrow dose levels. Issues related to whole-body and tumour dosimetry in this type

Right ventricular ejection fraction from equilibrium krypton-81m blood pool scans: a noninvasive predictor of pulmonary arterial hypertension.

Based on the relation between the impedance to right ventricular ejection and ejection fraction of the right ventricle (RVEF) we tested the hypothesis that radionuclide RVEF correlates inversely with pulmonary artery pressure and allows the noninvasive prediction of pulmonary hypertension. A recently developed improved equilibrium right ventricular blood pool imaging technique using ultrashort-lived krypton-81m and simultaneous right heart catheterization were combined for assessment of right ventricular systolic performance in relation to right ventricular loading in a heterogeneous group of

The Use of Glycine-Containing Peptide-Based Chelators for Labeling with 99mTc Improves the Imaging Properties of EpCAM-Targeting Designed Ankyrin Repeat Ec1.

Noninvasive radionuclide imaging of epithelial cell adhesion molecule (EpCAM) expression in lung, ovarian, breast, kidney, and other cancers can stratify patients for EpCAM-targeted therapy. The constructed scaffold proteins, designed ankyrin repeat proteins (DARPins), are highly specific high-affinity probes for radionuclide imaging. A clinical study demonstrated that the anti-EpCAM DARPin [99mTc]Tc-(HE)3-Ec1 showed precise EpCAM imaging at 2, 4, and 6 h after injection in patients with nonsmall cell lung cancer. However, a noticeable accumulation in healthy organs has prompted the developmen…

Bone scan appearance of osteogenesis imperfecta in an adult.

An adult patient with undiagnosed osteogenesis imperfecta presented with a history of multiple fractures. Radiographs could not definitively diagnose the underlying abnormality. The radionuclide bone image suggested the presence of a skeletal dysplasia, and prompted the histopathologic review of fracture fragments. The scan appearance of osteogenesis imperfecta in the adult and possible underlying mechanisms for the radiopharmaceutical distribution are discussed.

Theoretical aspects on the use of single-time-point dosimetry for radionuclide therapy.

Objective. This study considers the error distributions for time-integrated activity (TIA) of single-time-point (STP) methods for patient-specific dosimetry in radionuclide therapy.Approach. The general case with the same pharmaceutical labelled with different radionuclides for imaging and therapy are considered for a mono-exponential time-activity curve. Two methods for STP dosimetry, both based on the combination of one activity estimate with the population-mean effective decay constant, are investigated. The cumulative distribution functions (CDFs) and the probability density functions for

Effect of nitroglycerin on splanchnic and pulmonary blood volume.

BACKGROUND: Sublingual nitroglycerin (SL NTG) is useful for treating acute decompensated heart failure, possibly by increasing splanchnic capacitance and reducing left ventricular (LV) preload. We evaluated a radionuclide method to study these effects, initially in subjects without heart failure. METHODS AND RESULTS: Red blood cells were labelled by an in vitro method. Abdominal and chest images were obtained at rest, showing relative regional blood volumes. The abdomen was then re-imaged during progressive escalation of intrathoracic pressure using continuous positive airway pressure to asse

Sialography: report of 3 cases.

Salivary gland examination is an important part of oral examination, especially because of it's involvement in most of the systemic diseases. Patients most commonly seek medical attention when the major salivary glands like parotid and submandibular gland become enlarged or painful. The various imaging modalities practiced to check the salivary gland disorders include conventional radiography, sialography, ultrasonography, computerized tomography, radionuclide imaging and magnetic resonance imaging. Sialography is one of the oldest imaging procedures and still most commonly practiced, as it is…
