Epidemiology of distal radius fractures and factors predicting risk and prognosis.

STUDY DESIGN: Literature Review. INTRODUCTION: For optimal Distal Radius Fracture (DRF) rehabilitation and fracture prevention, it is important to understand the epidemiology and factors predictive of injury, chronic pain, chronic disability, and subsequent fracture. PURPOSE: To summarize the literature reporting on DRF epidemiology, risk factors, and prognostic factors. METHODS: Literature synthesis. RESULTS: Although incidence varies globally, DRFs are common across the lifespan and appear to be on the rise. Risk of DRF is determined by personal factors (age, sex/gender, lifestyle, healt

[Descriptive, radiographic, topographic and functional anatomy applied to distal radius fractures].

Extensive anatomical knowledge is needed in order to correctly treat distal radius fractures: normal and pathological osteology of the distal radius, distal radio-ulnar joint and radiocarpal joint-both on the descriptive and functional level-the neurovascular and muscular environments of the distal radius-which are essential to the surgical approaches-and the angles that need to be restored during the reduction. All of these concepts are broached in this article.

[New minimally invasive approach for palmar plating in distal radius fractures].

Distal radius fractures are among the most common fractures. Types A3 and C2 make up almost half of these. Volar fixed-angle plate fixation is the gold standard of treatment today. We wanted to minimise the approach to the distal radius through a transverse 2-3 cm incision in the wrist flexor fold, tunnelling beneath the pronator muscle. The operative technique is demonstrated here. 11 patients, averaging 70 years of age, underwent surgery for 7 A3 and 4 C2 type fractures. Average operating time was 50 minutes. Length of admission averaged 2.9 days. Using a visual analogue scale, pain decrease…

Hardware-related complications after dorsal plating for displaced distal radius fractures.

There has been a trend away from dorsal fixation of distal radius fractures secondary to a historically higher complication rate. However, the literature on low-profile dorsal plates and titanium implants for the treatment of these fractures is limited. The goal of the current study was to evaluate hardware-related complications and removal rates after open reduction and internal fixation of unstable, displaced distal radius fractures using a dorsal approach with a low-profile titanium plate. A single surgeon treated 125 patients with isolated, unstable, dorsally displaced distal radius fractu…

Secondary displacement of distal radius fractures treated by bridging external fixation.

Loss of reduction remains an important problem after treatment of distal radius fractures, whatever the type of bone fixation. We assessed retrospectively the rate of secondary displacement after external fixation of distal radius fractures in order to identify possible risk factors for instability. We reviewed the pre-operative and serial post-operative radiographs of a retrospective series of 35 distal radius fractures treated by bridging external fixation. When classified according to the Société Française d'Orthopédie et Traumatologie (SOFCOT) criteria, the rate of secondary displacement w…

Percutaneous cannulated screw fixation in the treatment of distal radius fractures.

PURPOSE: The aim of this study was to determine the results of closed reduction and percutaneous fixation of distal radial fractures with standard 4.0-mm cannulated screw. METHODS: We collected prospective outcomes data for 20 patients between 18 and 60 years of age (mean 39) with acute, displaced, extraarticular and unstable fractures of the distal radius treated with closed reduction, percutaneous cannulated screw fixation and early mobilization. The visits at months 2, 6 and 12 were specified as index follow-up visits and assessments of motion, grip strength and standard radiographs were p

Intra-articular fractures of the distal radius evaluated by computed tomography.

PURPOSE: To describe the fracture lines of intra-articular distal radius fractures as evaluated by computed tomography scans. METHODS: With computed tomography, we examined 95 intra-articular fractures of the distal radius from 91 patients. Multiplanar computed tomography images or 3-dimensional images, or both, were assessed. We divided the periphery of the distal articular surface of the distal radius into 6 segments (sigmoid notch, dorsoulnar, dorsoradial, volar radial, volar ulnar, radial styloid) and examined which segment had fracture lines. For fractures involving the sigmoid notch, we…

Infection rate of percutaneous Kirschner wire fixation for distal radius fractures.

PURPOSE: To review the infection rate in 43 patients who underwent percutaneous Kirschner wire fixation for distal radius fractures. METHODS: Records of 13 men and 30 women aged 25 to 86 (mean, 49) years who underwent closed reduction and percutaneous Kirschner wire fixation for unstable distal radius fractures were reviewed. Each fracture was fixed with 2 to 3 wires of 1.6-mm diameter. Kirschner wires were left protruding through the skin for easy removal, with their ends bent outside the skin to prevent migration. Wounds were cleaned and dressed with gauze and a plaster-of-Paris cast was ap…

Epidemiology and seasonal variation of distal radius fractures in Oulu, Finland.

UNLABELLED: The purpose of this study was to assess changes in epidemiology and reasons for wintertime excess of distal radius fractures in Oulu, Finland. Our results showed that age-specific incidence of distal radius fractures in elderly women in Finland has increased compared with a previous study. Slippery pavement surfaces assessed by a new meteorological model partly explained wintertime excess of fractures, but factors beyond weather are contributing to seasonality. INTRODUCTION: In this report, we describe the epidemiology and seasonal variation of distal radius fractures in Oulu, Fin…

"PAF" analysis of acute distal radius fractures in adults. Preliminary results.

There is not enough evidence in the literature to support the use of any treatment in distal radius fractures, mainly because of the heterogeneous aspects of most series. There is a need for more standardized analyses of distal radius fractures that should allow the identification of more homogeneous groups of patients. The authors propose a novel synthetic method to analyse acute distal radius fractures in adults. A one-page chart includes criteria related to the patient (P), the energy of the accident (A), and the characteristics of the fracture (F) along with associated ulnar and carpal les…

Evaluating bone union of distal radius fractures by measuring impedance values.

The measurement of impedance is a noninvasive and simple quantitative technique for evaluation of fracture healing. Fractures of the distal radius are the most common long bone fractures, accounting for approximately 20% to 25% of all acute fractures. External fixators have been applied clinically to treat unstable distal radius fractures. The evaluation of radiological findings and clinical signs are current standard methods and are effective in assessing fracture union of the distal radius, but these methods are dependent on subjective elements. For the objective methods, previous studies ha…

Distal metaphyseal radius fractures in children: reduction with or without pinning.

We retrospectively reviewed 39 distal metaphyseal radius fractures in children. This study compares the results of closed reduction with or without percutaneous pinning. Twenty-four fractures were treated by closed reduction and above-elbow cast immobilisation. Re-displacement was noted after one week in three patients, for which they were revised with reduction and pinning. Fifteen patients were initially treated by closed reduction and pinning. In these patients we saw no redisplacement after six weeks immobilisation in a forearm cast. We recommend closed reduction and pinning as a predictab…

Ulnocarpal stabilization in the management of comminuted fractures distal end radius.

BACKGROUND: Malunion due to progressive radial collapse during healing is a common complication following comminuted fracture distal radius treated by conventional methods. Many treatment modalities have been described with their own merits and demerits. Stabilization of ulnocarpal articulation is an effective method to prevent radial collapse during healing, and hence this study. MATERIALS AND METHODS: A prospective study of 200 patients of comminuted intraarticular fracture lower end radius between 20-75 years of age irrespective of sex, treated by closed reduction and percutaneous stabiliz

What counts: outcome assessment after distal radius fractures in aged patients.

OBJECTIVES: Outcome of surgical interventions at the distal radius does not only depend on the type of intervention used, it also depends on the way the outcome is measured. Substantial differences in outcome assessment between different measurement tools and poor correlation among them result in the question about the best instrument for the evaluation of treatment after distal radius fractures. The aim of the review is to discuss pros and cons of the parameters that are available to assess the outcome after distal radius fractures. The review should help to choose the appropriate instruments…

Dorsal plating for displaced intra-articular fractures of the distal radius.

Thirty patients with a mean age of 42 years and a dorsally displaced intra-articular fracture of the distal radius were treated by dorsal plating (Forte, PI and AO T plates). Twenty-eight required autogenous iliac crest bone graft. Mobilisation was started at 1-2 weeks and patients were left free by 6-8 weeks. At a minimum of 6 months (mean 11 months) functional outcomes were excellent or good in 93% according to the Gartland and Werley score and 60% according to Green and O'Briens modified score. The mean time taken to return to work was 10 weeks. At the final follow-up, three patients had in
