Interictal evoked central nervous system responses are characterized in migraineurs by a deficit of habituation, at both cortical and subcortical levels. The click-evoked vestibulo-collic reflex (VCR) allows the assessment of otolith function and an oligosynaptic pathway linking receptors in the saccular macula to motoneurons of neck muscles. Three blocks of 75 averaged responses to monaural 95-dB normal hearing level 3-Hz clicks were recorded over the contracted ipsilateral sternocleidomastoid muscle in 25 migraineurs between attacks and 20 healthy subjects, without vestibular symptoms. Ampli…

The expiration reflex (ER) is a forced expiratory effort against a closed glottis that subsequently opens to eject laryngeal debris and prevent aspiration of material. It is distinct from the cough reflex. Its source is usually assumed to be restricted to the larynx and vocal folds, and its possible origin from the tracheobronchial (TB) tree has been suggested but never studied. The current authors re-analysed previous records with mechanical or chemical stimulation of the TB tree to see if an ER can consistently be elicited, and to see whether it has properties similar to that from the larynx…

The authors examined the glabellar reflex and the palmomental reflex in 100 subjects, including patients with Parkinson disease (n = 41), patients with progressive supranuclear palsy (n = 12), patients with multiple system atrophy (n = 7), and healthy, age-matched, controls (n = 40). The study provides evidence that these reflexes, particularly glabellar reflex, are relatively sensitive signs of parkinsonian disorders, but they lack specificity as they do not differentiate among the three most common parkinsonian disorders.

Changes in posture demand rapid cardiovascular adjustments to maintain blood pressure and volume distribution. We demonstrated a vestibulo-cardiac reflex in supine individuals by measuring electrocardiogram and arterial blood pressure after small backwards drops of the head triggered at varying intervals after the R-spike. In normal volunteers heart rate was accelerated by drops occurring within 500-600 ms of a beat, but no rapid effect was noted in patients with a vestibular defect. The speed at which the vestibular signal of head drop accelerated heart rate implies a direct reflex. Impairmen…

INTRODUCTION: Dizziness or syncope may occur during treatment of nasal disease. These symptoms are considered imputable to the vagovagal reflex, which is partly involved in neurogenic syncope. In response to trigeminal nerve stimulation, the vagus nerve causes a sudden fall in heart rate and blood pressure. Elevation in blood adrenaline level during pain or tension was noted, and its effects on vagovagal reflexes were studied.
MATERIALS AND METHODS: Ten cats were used in the experiment. The vagus nerve was exposed on the right side of the neck by making an incision in the trachea, and a plati…

OBJECTIVES: We have investigated the deterioration of cardiovascular reflexes in Parkinson's disease.
PATIENTS AND METHODS: In a group of 20 patients with Parkinson's disease (PD) and in a group of 10 age-matched control subjects a battery of cardiovascular reflex (CVR) tests (Valsalva manoeuvre, deep breathing test, handgrip test, orthostatic test) was repeated after 3 years.
RESULTS: In PD patients at deep breathing test, at Valsalva manoeuvre and at orthostatic test the heart rate response decreased significantly (P<0.05). The impairment of systolic and diastolic blood pressure response a…

The peristaltic reflex represents the basis of peristalsis and has two components, ascending contraction above and descending relaxation below the site of distension. Studies of the two components of the reflex in the human oesophagus performed by concurrent monitoring of oesophageal body and lower oesophageal sphincter (LOS) motility are lacking. We investigated the peristaltic reflex in eight healthy volunteers (aged 19-25 years; five male, three female) by two series of eight graded (3-10 mL) balloon distensions performed 11 cm above the LOS, monitoring motor activity in the oesophageal bod…

Spasm of the near reflex has been characterized as the variable appearance of pseudomyopia, convergent strabismus and miosis. These characteristics may appear together or separately. In addition, abnormalities of accommodation may appear not only as pseudomyopia, but may also be manifest in cases with significant hypermetropia in which the patient is unable to relax accommodation even when plus lenses are used. The intent of this review is to identify the various clinical presentations of anomalies of the entire near reflex as well as the component parts. The relationship to functional and org…

In 4 patients with familial olivopontocerebellar atrophy (OPCA) we have recently described an abnormal movement of facial muscles characterized by rhythmic muscle twitching during voluntary activation (facial action myoclonus). In the present article, we present the results of a neurophysiological study of brainstem reflexes in those 4 patients, in 4 other patients with OPCA but without facial action myoclonus, in 3 patients with pure cerebellar cortical atrophy, and in 6 normal volunteers used as control subjects. All patients had similar clinical features, but only the patients with facial a…

OBJECTIVE: To review the clinical utility of blinking and the release reflexes, palmomental, glabellar, grasp, and snout, as an important part of the bedside neurological examination.
DESIGN: Articles published from 1966 to 1993 that addressed blinking or the release reflexes were identified by searching the MEDLINE database. Thirty-seven references provided clinically useful and applicable information.
RESULTS: There is controversy regarding the optimal method of clinical elicitation of these reflexes. The release reflexes are frequently seen as an incidental abnormality in the elderly. A p…

We describe a method for measurement of the sensitivity of upper airway reflexes. The technique is based upon delivery of an irritant chemical stimulus (dilute concentrations of ammonia vapour) to the upper airway. The technique is non-invasive and uses equipment which is portable, allowing measurements to be made in the clinical environment.

The present study was aimed to analyse the influence of bronchoconstriction (Carbachol, 10 micrograms/kg, i.v.) on the cough response to mechanical stimulation of the tracheal mucosa. Experiments were performed in anaesthetised and spontaneously breathing cats, using the isolated glottis technique. Airflow, pleural pressure, subglottic pressure, blood pressure and total lung resistance were recorded. During bronchoconstriction, the cough response was inhibited significantly as shown by the decrease in the number of cough efforts (p < 0.001), in maximum flow during inspiratory (p < 0.001) and e

Arteries to the cervical trachea were perfused at constant flow in anesthetized sheep. Perfusion pressures (PP), blood pressure (BP), and changes in tracheal smooth muscle tone (Ptr) were measured. Stimulation of pulmonary C-fiber receptors decreased PP (-6.5%) and BP (-16.8%) and increased Ptr (+61.5%), changes prevented by vagotomy and therefore reflex. Stimulation of cardiac receptors and slowly adapting pulmonary stretch receptors decreased PP (-7.9%) and BP (-21.0) and increased Ptr (+19.0%), changes reversed by vagotomy and therefore reflex. Stimulation and inhibition of slowly adapting …

In Friedreich's ataxia, the muscle stretch reflexes are absent or hypoactive in the upper and lower extremities because of pathologic involvement of the dorsal root ganglion cells. Paradoxically, the masseter reflex remains unaffected and may even be hyperactive in some cases. The masseter reflex is hypothesized as unique among stretch reflexes in that its afferent nerve cell body (the mesencephalic nucleus) is located intra-axially within the brainstem rather than in the craniospinal ganglia, where the afferent nerve cell bodies subserving the other stretch reflexes are located. This study su…

The startle response to unexpected auditory and somaesthetic stimulation was studied in 8 patients with hereditary or symptomatic hyperekplexia. It was abnormal in its resistance to habituation and in its exaggerated motor response. Both noise and taps to the face and head elicited a normal early blink response, separate from the subsequent true startle reflex. The earliest reflex EMG activity recorded after the blink was in sternocleidomastoid. EMG activity in masseter, and trunk and limb muscles followed later. This pattern of muscle recruitment suggests a brainstem origin for the abnormal s…

Noninvasive determination of anal and genitoanal reflexes was evaluated in clinically normal cats. Thirty adult mixed-breed cats (15 sexually intact or castrated males, 15 sexually intact or spayed females) were sedated by IV administration of ketamine, acetylpromazine, and atropine. Anal reflexes were recorded from the anal sphincter muscle after ipsilateral and contralateral electrical stimulation of the perineal skin. Genitoanal reflexes were recorded from the anal sphincter muscle after electrical stimulation of the penis or clitoris. An anal sphincter response to tibial nerve stimulation …

We studied the reflex arginine vasopressin (AVP) response to hypotensive, isosmotic fluid subtraction (by isolated UF) in 14 uraemic patients on renal dialysis treatment: five with normal autonomic function and nine with autonomic involvement of various degrees. Fluid subtraction caused a comparable mean arterial pressure (MAP) decrease in the two groups. The reduction in right atrial pressure was inversely related with the severity of autonomic neuropathy (rs = -0.72, P = 0.004), being distinctly attenuated in the second group (P = 0.006). Plasma arginine vasopressin increased similarly in pa…

Based on the examination of 89 children (71 patients with various bladder diseases and 18 healthy subjects), the authors developed a new procedure for recording the vesicosphincteral reflex by the use of urethral profilometry. The procedure involved comparison of background and stimulation examination findings of urethral profilograms. The optimum parameters were defined for detection of vesicosphincteral reflex and the conditions were developed for its performance. The procedure may be useful in the diagnosis of neurogenic bladder, sphincter-detrusor dyssynergia, stress-induced enuresis, etc.
