A biomechanical analysis of controllable intraoperative variables affecting the strength of rotator cuff repairs at the suture-tendon interface.

BACKGROUND: The tissue-suture interface remains the weakest aspect of a rotator cuff repair, highlighting the importance of identifying techniques to improve stitch strength. Choice of suture-passing devices, size of the tissue bite, and stitch configuration are variables that may influence stitch strength and therefore repair stability. PURPOSE: To evaluate the effect that size of the tissue penetrator device and tissue bite size have upon the holding strength of commonly used stitches. STUDY DESIGN: Controlled laboratory study. METHODS: Three different-sized tissue-penetrating devices, sm

Internal impingement syndromes.

Internal causes of impingement are secondary to rotator cuff and capsular dysfunction and are categorized by the location of the impingement and the underlying pathophysiological or mechanical cause of the impingement. These include posterosuperior impingement, anterosuperior impingement, anterior impingement, and entrapment of the long head of the biceps tendon. The objective of this article is to review magnetic resonance imaging findings of each of the 4 types of internal impingement syndromes and discuss the pathophysiology behind the impingement.

[Reconstruction of massive rotator cuff rupture using a deltoid muscle flap].

In extensive rotator cuff tears with rupture of the supra- and infraspinatus muscles, we have obtained good and very good results persisting for 3 years or longer only in 40% of cases treated by resection and debridement, and in 25% of our cases the head of the humerus was found to be no longer centrally located in the joint socket on X-ray examination 10 years after treatment. It was therefore proposed that an active muscle flap taken from the anterior part of the deltoid muscle (part III according to Fick) should be used to span the trophic defect in the rotator cuff, being sutured into heal

Bicipital groove dysplasia and medial dislocation of the biceps brachii tendon.

The purpose of this study was to investigate the plain film finding of dysplasia of the lesser tubercle of the humerus and its relationship to medial dislocation of the tendon of the long head of the biceps brachii muscle as diagnosed by shoulder arthrography. Of 55 patients referred for arthrography of the shoulder because of undiagnosed shoulder pain, 12 demonstrated flattening of the medial wall of the bicipital tendon groove. Of these, 58% had medial dislocation of the biceps tendon, and 43% of patients with dislocation of the biceps tendon were also shown to have a tear of the rotator cuf

Changes in shoulder muscle activities and glenohumeral motion after rotator cuff repair: an assessment using ultrasound real-time tissue elastography.

BACKGROUND: Although rotator cuff repair is performed to restore the function of the rotator cuff muscles and glenohumeral (GH) joint motion, little has been known regarding the recovery process. The purpose of this study was (1) to investigate changes over time in activities of the supraspinatus and deltoid muscles assessed by ultrasound real-time tissue elastography (RTE) after rotator cuff repair and (2) to determine contributions of the activities of these muscles to the GH joint motion. METHODS: Twenty patients after rotator cuff repair and 13 control participants were enrolled in this s

Modified Long Head of Biceps Tendon Rerouting and Fixation as Partial Capsular Reconstruction for Massive Irreparable Rotator Cuff Tears.

Massive irreparable rotator cuff tears (MIRCTs) present a particularly complex and difficult challenge for the orthopedic surgeon. Fatty infiltration of muscles, chronic excessive retraction of tendons, and overall tissue degeneration predispose these cases to high failure rates following surgical treatment. Several approaches have been introduced for transferring the long head of the biceps tendon (LHBT) to help stabilize the humeral head and reestablish shoulder muscle strength. This article proposes a technical modification of capsular reconstruction using arthroscopic LHBT rerouting and fi

Factors associated with persistent shoulder pain following arthroscopic rotator cuff repair: a scoping review.

BACKGROUND: Rotator cuff (RC) tears are among the most common and disabling musculoskeletal conditions of the shoulder. When conservative management fails, arthroscopic rotator cuff repair (ARCR) is performed. Some patients, however, report persistent shoulder pain following the surgical procedure. To improve patient outcomes, it is essential to identify factors associated with persistent pain following ARCR, which is the aim of this scoping review. METHODS: The scoping review followed the Joanna Briggs Institute (JBI) methodology and Preferred Reporting Items for Systematic Reviews and Meta-

Arthroscopic Management of Massive Irreparable Rotator Cuff Tears: Whole Rotator Cable Reconstruction Using Proximal Biceps Tendon Autograft.

Massive irreparable rotator cuff tears (MIRCTs) are not uncommon in clinical practice, significantly impacting shoulder function and daily activities. Extensive tear size, tendon contracture, and fat infiltration within the rotator cuff pose significant challenges for both patients and clinicians. This type of tear is a key area of interest and a challenge in research and treatments. Current treatment options include conservative management, debridement, partial repair, superior capsule reconstruction (SCR), tendon transfers, and reverse total shoulder arthroplasty (RTSA). However, clinical ou

Optimized design of an enthesis-mimicking suture anchor-tendon hybrid graft for mechanically robust bone-tendon repair.

Repair of functionally graded biological interfaces requires joining dissimilar materials such as hard bone to soft tendon/ligament, with re-injuries/re-tears expected to be minimized by incorporating biomimicking, stress-reducing features within grafts. At bone-tendon interfaces (entheses), stress can be reduced via angled insertion, geometric flaring, mechanical gradation, and interdigitation of tissues. Here, we incorporated enthesis attributes into 3D in silico and physical models of a unique suture anchor-tendon hybrid graft (SATHG) and investigated their effects on stress reduction via f

Optimizing Tissue Engineering for Clinical Relevance in Rotator Cuff Repair.

Rotator cuff tear (RCT) is the most common cause of disability in the upper extremity. It results in 4.5 million physician visits in the United States every year and is the most common etiology of shoulder conditions evaluated by orthopedic surgeons. Over 460,000 RCT repair surgeries are performed in the United States annually. Rotator cuff (RC) retear and failure to heal remain significant postoperative complications. Literature suggests that the retear rates can range from 29.5% to as high as 94%. Weakened and irregular enthesis regeneration is a crucial factor in postsurgical failure. Altho

Automatic MRI-based rotator cuff muscle segmentation using U-Nets.

BACKGROUND: The rotator cuff (RC) is a crucial anatomical element within the shoulder joint, facilitating an extensive array of motions while maintaining joint stability. Comprised of the subscapularis, infraspinatus, supraspinatus, and teres minor muscles, the RC plays an integral role in shoulder functionality. RC injuries represent prevalent, incapacitating conditions that impose a substantial impact on approximately 8% of the adult population in the USA. Segmentation of these muscles provides valuable anatomical information for evaluating muscle quality and allows for better treatment plan

Microfragmented adipose tissue in the treatment of a full-thickness supraspinatus tear: a case report.

A 70-year-old female presented with an 8-month history of right anterior shoulder pain and weakness, unresolved with conservative management. Among other shoulder pathology, the patient was diagnosed with a full-thickness supraspinatus tear and elected to proceed with the microfragmented adipose tissue procedure to treat the injured tendon and nearby relevant structures. Improvements in pain and function were documented along with progressive healing of the supraspinatus on ultrasound and MRI following the procedure. This case demonstrates the efficacy of microfragmented adipose tissue as a re

Editorial Commentary: Shoulder Superior Capsular Reconstruction Remains to Be Proven Superior to Partial Repair of the Rotator Cuff for Active Patients With Irreparable Tears and Without Osteoarthritis.

The management of irreparable rotator cuff tears in active patients without severe osteoarthritis is challenging. Retracted tears of the superior cuff result in devastating glenohumeral kinematics and decreased shoulder function. Surgical solutions such as superior capsular reconstruction (SCR) or tendon transfers may improve shoulder function. Regarding SCR, the superior capsule has been described as a static stabilizer allowing for the centering of the humeral head. However, some bases for SCR are based on biomechanical studies that should be regarded as time zero, absent healing, and genera

Clinical Approach to Inconclusive Subscapularis Tear Diagnosis: a Meta-analysis.

To identify factors associated with subscapularis (SSC) tears and provide a theoretical basis for clinical diagnosis, we included studies related to subscapularis tears published before February 1, 2023. We screened for six predictors across previous studies for the meta-analysis. The predictors included age, sex, coracoid overlap (CO), coracohumeral distance (CHD), impairment of the long head of the biceps tendon (LHB), and dominant arm. The Newcastle-Ottawa Scale (NOS) was used to evaluate the quality of the studies. The risk ratios (RRs) and the weighted mean differences (WMDs) were used to
