Subacute puerperal inversion of the uterus is preferably managed by surgery. Conservative management by Aveling's repositor or vaginal packing was cumbersome and took long. OBJECTIVE: There is a void in the literature describing reliable surgical landmarks that aid in the dissection of the facial recess in the absence of skeletonizing the mastoid segment of the facial nerve. The posterior ligament of the incus is a readily distinguishable "white dot" along the incus buttress that has been used to guide dissection in a safe and efficient manner. The goal of our study is to describe a surgical approach that utilizes this surgical landmark to drill the facial recess and to take anatomical measurements demonstrating the safety and reliability of this approach.

BACKGROUND: Increased safety and diminished blood loss are achieved through laparoscopic-assisted vaginal hysterectomy by selective coagulation and transsection of the uterine vessels at their origin.
METHODS: Three laparoscopic steps are performed: coagulation and transsection of the round ligament, of the uterine artery at its origin, and of the fallopian tube and ovarian ligament or (in BSO) the infundibulopelvic ligament. The uterine vessels are identified from the pararectal space and, following the internal liliac artery, and the ureter. Hysterectomy is completed transvaginally.

Pelvic endometriosis is a well-known clinical risk factor for pelvic inflammation and adhesions. We present a complex case of a woman undergoing a cesarean section where the traditional incision on the anterior lower uterine segment was not possible due to a congested pelvis. There are multiple reports of gastric neuroendocrine carcinoma in bearded dragons (Pogona spp), but other types of alimentary neoplasia are rarely reported. A case of Morgagni's hiatal hernia is presented. Because of clinical symptoms (dizziness and chest pain) the patient was referred to the Cardiology Department and the diagnosis was a casual finding. The hernia sac contained transverse colon, great omentum and round ligament. We comment on the mechanism, etiologic factors, diagnosis, and surgical treatment.
