Capturing concussion related changes in dynamic balance using the Quantified Y Balance Test - a case series of six elite rugby union players.

Concussion is one of the most common injuries reported across a myriad of sports. Recent evidence suggests that individuals may possess sensorimotor deficits beyond clinical recovery, predisposing them to further injury. This preliminary prospective case series aimed to determine if an inertial sensor instrumented Y balance test can capture changes in dynamic balance, regardless of apparent `clinical recovery', in six concussed elite rugby union players. The findings from this case series demonstrate that the inertial sensor-based measures can detect clinically meaningful changes in dynamic ba…

Evaluation of GFAP, UCH-L1 and S100B Blood Biomarkers for the Management of Sports-Related Concussion in Professional Rugby Players: A Prospective Observational Study.

BACKGROUND: The incidence of sports-related concussion (SRC) in rugby has been rising in recent years. The Head Injury Assessment (HIA) protocol, used to manage SRC, is limited by its reliance on subjective symptom reporting. Blood biomarkers such as glial fibrillary acidic protein (GFAP), ubiquitin C-terminal hydrolase L1 (UCH-L1) and S100B could serve as complementary objective tools for SRC management. RESULTS: A prospective cohort study was conducted in a French elite professional rugby team (Association Montferrandaise Clermont Auvergne Sports, Association Sportive Montferrandaise Clermo…

Tackling sport-related concussion: effectiveness of lowering the maximum legal height of the tackle in amateur male rugby - a cross-sectional analytical study.

OBJECTIVE: Rugby union ('rugby') is popular globally, with most of its participants being amateur. Concern regarding sport-related concussion (SRC) sustained during rugby is increasing. SRC occurs most frequently in the tackle, yet few interventions have aimed to mitigate this risk factor. This study investigated the influence of a lowered legal tackle height on SRC incidence in amateur rugby. DESIGN: Cross-sectional analytical study: 2018 (control-standard tackle height) and 2019 (intervention-lowered legal tackle height) seasons. SETTING: South African collegiate student rugby competition.…

A blinded study of inter-rater agreement of video signs for potential concussion in the National Rugby League comparing novice versus expert reviewers.

Inter-rater agreement between novice and expert clinicians for the six international consensus video signs of concussion was investigated in the National Rugby League. Consecutive head injury events from the 2017 and 2018 seasons were included (n = 636). Video footage was examined by two novice and two experienced raters. Agreement for any video sign and the exact combination of video signs was fair (Gwet's AC1 of 0.31, 0.37). Agreement for individual video signs ranged from fair (motor incoordination, Gwet's AC1 0.28) to almost perfect (impact seizures, Gwet's AC1 0.99). Agreement was signifi…

Mahi tahi (working together) and the lessons learned from implementating a concussion management pathway for community rugby in New Zealand: Haerenga (our journey).

Concussion guidelines are widely available, but implementing them consistently in community sport remains difficult. Common barriers include limited access to medical care, reliance on volunteers, fragmented communication, and sporting cultures that may discourage reporting. In response, New Zealand Rugby developed and refined over multiple seasons, a community Concussion Management Pathway (CMP), to operationalise World Rugby's principles of 'Recognise and Remove' in real-world community settings. This editorial reflects on lessons learned from the design, implementation, and iterative refine…
