Assessing the effect of chemotherapy dose reductions and time delay on event-free survival in Ewing Sarcoma: A retrospective analysis of the Euro Ewing 2012 trial.

BACKGROUND: Treatment adjustments are commonly implemented during chemotherapy to mitigate toxicity, but their impact on event-free survival (EFS) remains poorly understood. This study aims to evaluate the causal effect of dose reductions and time delay during the VDC/IE induction regimen (vincristine, doxorubicin, and cyclophosphamide, alternating with ifosfamide and etoposide) on EFS in patients with Ewing Sarcoma. METHODS: Chemotherapy administration data from the Euro Ewing 2012 (EE2012) trial were used to emulate a target trial, where the target exposures were a binary indicator for tota

AVN944 Elicits Apoptotic Responses and Impedes Tumorigenic Potential in Ewing's Sarcoma Cells.

Inosine monophosphate dehydrogenase 2 (IMPDH2) is implicated in survival and proliferation of cancer cells because of its role in guanine nucleotide biosynthesis. This study evaluates the efficacy of AVN944, an IMPDH2 inhibitor, as a treatment for Ewing's sarcoma, a challenging malignancy in pediatric and young adult patients. Gene expression data, as well as clinical outcomes for sarcoma patients, from The Cancer Genome Atlas (TCGA) were analyzed to determine the association between IMPDH2 expression and survival. Human Ewing's sarcoma cell lines and xenograft models were used to evaluate the

Solitary intracranial dural-based Ewing's sarcoma in a pregnant female: A case report.

Ewing's sarcoma (ES) is a rare and highly malignant tumor characterized by small round blue cells, typically originating from bones and soft tissues in children and young adults, although it is rarely found as a primary intracranial lesion. We presented a case of a 26-year-old primigravida presenting with headache, vomiting, blurred vision, papilledema, and abducens nerve palsy. Magnetic resonance imaging (MRI) scan revealed a large extra-axial cystic-solid mass located in the occipitoparietal region, which resembled an atypical meningioma. Surgical excision was carried out, and histopathologi

Laryngeal Preservation in Extraosseous Ewing's Sarcoma of the Larynx: A Case Report and Review of the Literature.

Extraosseous Ewing's sarcoma (EES) of the larynx is an exceptionally rare malignancy, with very few cases reported in the literature. Its clinical behavior and optimal management remain poorly defined. We report the case of a 14-year-old patient who presented with hoarseness of voice. Imaging and histopathological evaluation confirmed a diagnosis of laryngeal EES. The patient was treated with systemic chemotherapy followed by radiation therapy, resulting in a favorable clinical and radiological response. EES can occur across a broad age range and may demonstrate aggressive local growth and ear

Ewing's Sarcoma in Adults: A Predictive Nomogram and Survival Analysis of a Cohort of 937 Patients.

BACKGROUND: Ewing sarcoma is a rare malignancy of bone and soft tissue that most commonly affects children, adolescents, and young adults. Although multimodal treatment has improved survival in localised disease, outcomes remain poorer in adults, particularly in those with metastatic disease, larger tumours, or anatomically complex primary sites. Current staging systems are useful for treatment allocation but do not fully capture individual prognostic variation. This study aimed to develop and internally validate a predictive nomogram for disease-specific survival in adult patients with Ewing

Unusual pelvic mass in a young adult: prostatic Ewing's sarcoma with distant metastases.

Ewing's sarcoma is an aggressive small round cell tumour that usually arises from bone or soft tissues in children and young adults. Primary origin within the prostate is exceptionally rare, with only isolated cases documented. We describe a man in his 20s who presented with urinary obstruction, constipation and leg pain. Imaging revealed a bulky pelvic mass arising from the prostate and bilateral hydroureteronephrosis. Histology demonstrated a round cell neoplasm with strong CD99 and FLI1 positivity, confirming Ewing's sarcoma. Staging PET-CT identified widespread metastases involving lung, l

A systematic review on the impact of delayed local therapy in patients with Ewing sarcoma of the pelvis.

BACKGROUND: Local treatment of pelvic Ewing Sarcoma (EWS) is czhallenging due to complex anatomy and potential complications. Local therapy may be deferred to maintain chemotherapy dose-intensity, but the impact of this delay on outcomes remains unclear. METHODS: A systematic review using the PICO model evaluated whether delayed local therapy affects long-term outcomes in localized pelvic EWS. Ovid Medline, Ovid Embase, and Cochrane Central databases were searched through June 2024. Studies were screened by two independent reviewers for quantitative and qualitative data extraction. RESULTS:

Primary intracranial extraosseous Ewing's sarcoma with intraspinal metastasis in children: illustrative case.

BACKGROUND: The diagnosis of intracranial extraosseous Ewing's sarcoma (EES) poses challenges due to the absence of specific clinical and imaging features prior to surgery. It is crucial to differentiate the tumor from other small round cell malignancies postoperatively. OBSERVATIONS: A 7-year-old patient was admitted to the authors' hospital due to the in situ recurrence of a posterior fossa tumor more than 1 month after the initial surgery for headache. Subsequently, a second surgery was performed at the authors' hospital, and pathological analysis indicated medulloblastoma. Genetic testing

Pediatric Ewing's Sarcoma and Permanent Chemotherapy-Induced Alopecia: A Case-Control Study.

Permanent chemotherapy-induced alopecia (PCIA), defined as the persistence of hair loss for > 6 months following chemotherapy cessation, is a rare pediatric diagnosis. A retrospective chart review was conducted to characterize cases of PCIA not previously reported in pediatric Ewing's sarcoma (ES) patients after completion of therapeutic chemotherapy, comparing demographic factors, cumulative chemotherapy dosing, family history of hair loss, and treatment methodologies with a control group. Of 12 patients with an history of pediatric ES, three were diagnosed with PCIA. Our findings suggest tha

Extraskeletal Ewing Sarcoma of the Extremities and Trunk: A Retrospective Analysis of a Mono-Institutional Series.

INTRODUCTION: Extraskeletal Ewing sarcoma (EEwS) is a rare malignant tumor, and current international recommendations indicate systemic and local treatment like bone Ewing sarcoma (BEwS); to the best of our knowledge, very few studies tried to explore the clinical and genetic characteristics of this tumor, and the most appropriate treatment strategy remains uncertain. METHODS: We reviewed 35 EEwS cases enrolled at Rizzoli Orthopedic Institute in Bologna, Italy, between 1988-2022. We performed RNA sequencing in 18 Ewing sarcoma cases, including 12 BEwSs and 6 EEwSs. We analyzed overall surviva

Abdominal distention, immunopositivity, and aggressive nature: A rare Ewing's sarcoma of the adrenal gland.

This case report describes a 32-year-old Pakistani male patient with an Ewing sarcoma (ES) of the adrenal gland. Presenting complaints were abdominal distention, pain, low-grade fever, and weight loss. Initial studies, including imaging and tumor markers, ruled out any other possible origins of the mass. A percutaneous biopsy verified the tumor's neuroendocrine origin. Extensive involvement of nearby anatomical structures was discovered through exploratory laparotomy, rendering total resection difficult. Based on the presence of malignant, round, blue cells that were positive for specific immu

3D-Printed Talus-Calcaneus Prosthesis in Treating Ewing's Sarcoma: A Case Report.

BACKGROUND: Malignant tumors originating in the talus are rare and present significant challenges in reconstruction. Traditional treatments, such as below-knee amputation or tbiocalcaneal fusion, often result in significant loss of ankle function. After tumor resection, reconstruction of the talus and calcaneus is necessary to preserve ankle function. However, the intricate anatomical structure and unique location of the talus and calcaneus present significant challenges for prosthetic reconstruction. CASE PRESENTATION: Here, we present the case of an 11-year-old adolescent patient diagnosed

A Systematic Review of the Gonadotoxicity of Osteosarcoma and Ewing's Sarcoma Chemotherapies in Postpubertal Females and Males.

Data on gonadotoxicity of chemotherapies are essential to better counsel young females and males about the risk of infertility and to better indicate fertility preservation measures before cancer therapies. However, such data have not recently been reviewed for bone cancer. Therefore, a systematic literature search was conducted considering papers published since 2000. This study is part of the FertiTOX® project, which aims to improve the lack of data regarding gonadotoxicity of cancer therapies to enable more accurate counseling regarding fertility preservation. Only relapse-free women and me

Outcomes of ilium and iliosacral Ewing's sarcoma resection reconstructed with tibial strut allograft.

AIMS: Ilium is the most common site of pelvic Ewing's sarcoma (ES). Resection of the ilium and iliosacral joint causes pelvic disruption. However, the outcomes of resection and reconstruction are not well described. In this study, we report patients' outcomes after resection of the ilium and iliosacral ES and reconstruction with a tibial strut allograft. METHODS: Medical files of 43 patients with ilium and iliosacral ES who underwent surgical resection and reconstruction with a tibial strut allograft between January 2010 and October 2021 were reviewed. The lesions were classified into four re

Large retroperitoneal extraskeletal Ewing's sarcoma with renal pedicle invasion: a case report.

BACKGROUND: Extraskeletal Ewing's sarcoma (EES) is a rare malignant tumor primarily found in children and young adults. Localized disease can present with nonspecific symptoms such as local mass, regional pain, and increased skin temperature. More severe cases may present with systemic symptoms such as malaise, weakness, fever, anemia, and weight loss. Among these lesions, retroperitoneal sarcomas are relatively uncommon and difficult to diagnose. Since they are usually asymptomatic until large enough to compress or invade the surrounding tissues, most are already advanced at first detection.
