Serum magnesium is linked with sperm concentration, motile sperm count and serum anti-Müllerian hormone in infertile men.

RESEARCH QUESTION: What is the relationship between serum magnesium, semen quality and reproductive hormones in infertile men? DESIGN: This study was a secondary analysis of data from a randomized controlled trial involving 330 infertile men treated with cholecalciferol + calcium or placebo for 150 days. Each participant underwent a physical examination and had blood and semen parameters assessed. Thirty-one men were excluded due to missing serum magnesium, leaving 299 men stratified by serum magnesium tertile. RESULTS: Sperm concentration and total sperm count were higher in men in the high

Predicting Adolescent Response to School-Based Mindfulness: A Secondary Analysis of the MYRIAD Trial.

IMPORTANCE: Depression most commonly first emerges during adolescence, making early prevention critical. While school-based mindfulness training (SBMT) offers a scalable prevention approach with broad reach, evidence of its effectiveness is mixed, and there is a compelling case for a more personalized approach to prevention. OBJECTIVE: To develop a data-driven algorithm from baseline characteristics to predict which adolescents are most likely to benefit from SBMT. DESIGN, SETTING, AND PARTICIPANTS: The My Resilience in Adolescence (MYRIAD) cluster randomized clinical trial was conducted fro

Ethnic Differences in Characteristics of Women Diagnosed With Early Gestational Diabetes: Findings From the TOBOGM Study.

OBJECTIVE: To compare the prevalence and clinical characteristics of early gestational diabetes (eGDM) and associated birth outcomes among women of different ethnic groups. METHODS: This is a secondary analysis of an international, multicenter randomized controlled trial of treating eGDM among pregnant women with GDM risk factors enrolled at < 20 weeks' gestation. GDM diagnosis was made using WHO-2013 criteria. Ethnicity was classified by self-identification. While Europids required at least one risk factor for recruitment, for others, ethnicity itself was a risk factor. RESULTS: Among women

Acute Cannabis Administration Transiently Reduces Mitochondrial DNA in Young Adults: Findings from a Secondary Analysis of a Double-Blind, Placebo-Controlled, Randomized Clinical Trial.

Background: Cannabis is one of the world's most commonly used substances; however, many questions remain unanswered as to how cannabis impacts the body. Recently, there has been a resurgence of research into the effects of plant-derived cannabinoids on mitochondrial health. In particular, a number of studies implicate mitochondrial-Δ9-tetrahydrocannabinol (Δ9-THC) interactions with altered memory, metabolism, and catalepsy in mice. Although the research in this field is expanding rapidly, there is little known about the effects of cannabis on mitochondria health in human subjects either in acu

Real-World Practice in Secondary Stroke Prevention Following Embolic Stroke of Undetermined Source: Secondary Analysis of the CASPR Registry.

BACKGROUND AND OBJECTIVES: Multiple clinical trials have failed to show benefit of anticoagulation over antiplatelets in preventing recurrent stroke among patients with embolic stroke of undetermined source (ESUS). However, there are limited data on antithrombotic prescribing patterns and clinical outcomes in a real-world setting. METHODS: This is a subgroup analysis of a multicenter retrospective observational cohort of consecutive adults with ESUS (admitted between 2015 and 2023). Patients were categorized according to antithrombotic initiation by day 7 after stroke or discharge, whichever

Bayesian re-analysis suggests participation rather than referral underlies the benefits of social prescribing for chronic diseases.

BACKGROUND: Social prescribing (SP) is increasingly implemented to address the social determinants of health in people with chronic diseases, yet uncertainty remains regarding its effectiveness and the mechanisms through which benefit is achieved. We performed a Bayesian re-analysis of randomized trials to distinguish overall treatment effects from implementation-related mechanisms. METHODS: We systematically identified randomized controlled trials of SP following PRISMA 2020 guidelines. Bayesian hierarchical models were used to estimate intention-to-treat (ITT) effects on physical activity,

Prognostic significance of heart rate trajectories on organ failure in predicted severe acute pancreatitis: secondary analysis of a randomized trial.

Elevated heart rate is associated with poor clinical outcomes across various patient populations, including acute pancreatitis. This study aimed to assess whether trajectories of daily highest heart rate during the acute phase were associated with prolonged organ failure in patients with predicted severe acute pancreatitis (pSAP). This was a secondary analysis of data from a multicenter, randomized, controlled trial that assessed the effects of different crystalloids in patients with pSAP. Based on the daily highest heart rate trajectories over the first five days, patients were categorized in

Early-event pathways to 6-month death or dependency after ischemic stroke: Secondary analysis of the international stroke trial.

BACKGROUND: Broad 6-month death-or-dependency outcomes after acute ischemic stroke can conceal opposing early benefit and harm pathways after antithrombotic treatment. We examined these pathways in the corrected public-use International Stroke Trial dataset. METHODS: We analyzed 19,285 randomized participants with assignable 6-month outcome classification; 150 with missing classification were excluded. The mediator was the first recorded selected event within 14 days or earlier death/discharge: early death, intracranial hemorrhage or hemorrhagic stroke, major extracranial bleeding, pulmonary

Trajectory and predictors of maternal relationship satisfaction from pregnancy to five years postpartum.

PURPOSE: Relationship satisfaction is a key factor in women's mental health and wellbeing outcomes, however, relationship satisfaction may change in times of stress and uncertainty, such as the perinatal period. While positive romantic relationships during the perinatal period may offer benefits, changes in relationship satisfaction and what may contribute to them longitudinally is not currently well understood. METHOD: One hundred and fifty-seven women in late pregnancy (~ 30 weeks gestation) took part in a longitudinal analysis of relationship satisfaction from pregnancy to five years postp
