Clinical significance of gastrin radioimmunoassay.

Serum gastrin radioimmunoassay (RIA) is a sensitive and specific method suitable for measurement of circulating concentrations of this peptide hormone, which is a major regulator of gastric acid secretion. When performed under optimal conditions this RIA permits measurement of low and normal serum gastrin levels and changes that occur after physiologic stimulation. Hypergastrinemia may be secondary to atrophy of the acid-secreting gastric mucosa. This form of pypergastrinemia is appropriate and leads to no seriousequences. Hypergastrinemia associated with gastric acid hypersecretion is inappro…

[Biological diagnosis of Zollinger-Ellison syndrome in the context of a polyendocrine adenomatosis].

The diagnostic features of Zollinger-Ellison syndrome (ZES) in patients with type I poly-endocrine adenomatosis with a gastrinoma are analysed on the basis of 11 cases. Both from a clinical (frequency of ulcer disease and of diarrhoea) and biological standpoint (hypersecretion of gastric acid, hypergastrinaemia, or results of secretin stimulation test), the characteristics of ZES are not different whether a gastrinoma is present alone or in association with involvement of other endocrine glands. However the signs may be either masked by changes related to another endocrine disorder or appear l…

Lymph node gastrinoma in multiple endocrine neoplasia type 1 - a diagnostic challenge.

BACKGROUND: Gastrinomas are the most frequent hormonally-active neuroendocrine tu-mours in patients with multiple endocrine neoplasia type 1 (MEN1). CASE REPORT: We report on the diagnostic difficulties in a 62-year-old female patient with MEN1 and lymph node gastrinoma. At six and twelve months after resection of a lymph node gastrinoma, no signs of recurrence were observed. Basal and peak gastrin levels during secretin stimulation test were normalized. Extensive explorations, including gastrointesinal endoscopy, endoscopic ultrasonography, and Ga-68-DOTATOC-PET/CT, did not reveal a primary …

[Exocrine pancreatic function and gastric mucosa and duodenal mucosa in patients with chronic kidney failure].

Uraemic pancreatopathy is frequently observed in patients with chronic renal failure. The aim of the study was to assess some parameters of exocrine pancreatic function in uraemic patients maintained on intermittent haemodialyses. Elevated serum amylase activity was found in these patients. The most significant finding was the low bicarbonate output in duodenal content after secretin-cerulein stimuli, comparable with that obtained in patients with chronic pancreatitis. It indicates pancreatic exocrine defect in uraemic patients. A fall in protein output and lower amylase activity in duodenal c…

Effects of gastrointestinal peptides on formation of gallstone in guinea pigs.

In light of the effects of gastrointestinal (GI) peptides on bile secretion and biliary tract mobility, we studied the effects of GI peptides on gallstone formation in guinea pigs fed on low protein lithogenic diet. The peptides under study included cholecystokinin octapeptide (CCK-8), vasoactive intestinal peptide (VIP), somatostatin (SRIF), secretin (SEC), and neurotensin (NT). Hepatic bile flow, electrolytes, and other bile components were also measured. It was found that CCK-8 and VIP suppressed the formation of gallstones and increased hepatic bile flow and Na+, K+, Cl- output significant…

Measurement of gut hormonal peptides in biopsies from human stomach and proximal small intestine.

Using sensitive and specific radioimmunoassays, concentrations of hormonal peptides have been measured in small biopsies taken from the human stomach, duodenum, and proximal jejunum. Comparison is made of these hormone concentrations and the number of respective endocrine cells present determined by quantitative immunocytochemistry. Immunoreactive somatostatin, VIP, motilin, and gastrin were detected in all regions examined, whereas secretin and GIP were undetectable in antral extracts. Enteroglucagon-like immunoreactivity was present only at and beyond the ligament of Treitz, although a few e…

Types of pancreatic islet cells and their immunocytochemical identification.

In the Lausanne classification of islet cells which is based mainly on the ultrastructural characteristics of secretion granules, a total of nine different cell types have been described. By immunocytochemistry at least 12 different hormones or peptides have been either detected or postulated as being within cell types in the pancreatic islets. The cells responsible for the secretion of insulin, glucagon, GIP, pancreatic polypeptide and somatostatin have been firmly established. The identification of cells containing VIP, secretin, gastrin, biogenic amines and other peptides still remain tenta…

Islet cell tumors and the ulcerogenic syndrome.

It has now been well established that the ulcerogenic syndrome associated with non-beta islet cell tumors of the pancreas is due to excess g … ve tests with measurement of serum gastrin in response to intravenous calcium infusion, intravenous secretin injection and feeding of a standard meal. Gastrinomas are usually located within the pancreas, are often multifocal and metastatic, but may be primary in non-pancreatic sites. There is substantial heterogeneity in the molecular forms of circulating …

The coordination of gastrointestinal hormones and the autonomic nerves.

We have conducted investigations on the relationship between gastrointestinal hormones and the autonomic nervous system. In this paper are described main themes of our investigations where a close association between gastrointestinal hormones and the autonomic nervous system was shown from the functional and morphological points of view. The "nervism" proposed by Pavlov, where the regulation of various organs in the body was thought to be accomplished wholly by the nervous system, was based on the findings on the physiology of the digestive glands. In the meantime, the discovery of secretin fr…

Gastric glucagon release during gastric and intestinal phase of a meal in dogs.

The intragastric instillation of a liver meal at pH 7 elicited a prompt and significant rise in gastric vein glucagon levels in anesthetized … tide (GIP) (1 microgram . kg-1 . h-1), but neither CCK-octapeptide (0.5 microgram . kg-1 . h-1) nor secretin (1 CU . kg-1 . h-1), elicited a significant rise in gastric vein glucagon levels. It is concluded that 1) gastric glucagon release is stimulated during the gastric and intestinal phase of a meal, 2) gastric glucagon release is stimulated by GIP, and …

Evidence for pyloropancreatic reflux for pancreatic exocrine secretion.

In seven dogs provided with innervated pouch of the pyloric antrum, gastric fistula, and chronic pancreatic fistula, the effect of graded antral distenion on pancreatic exocrine secretion was studied. The effect of antral distension was tested both when gastrin release was promoted (alkaline distention) and when gastrin release was blocked (acid distension). These studies were performed both before and after transthoracic truncal vagotomy. Graded alkaline antral distension caused graded rise in protein-rich pancreatic secretion and graded increase in the concentration of serum immunoreactive g

[The diagnosis of pancreatic endocrine tumors].

The pancreatic endocrine tumors are uncommon neoplasms and are classified into non-functional and functional tumors. According to whether the secreted hormones are originated from pancreatic islet cells or not, the tumors are also classified into normotopic and ectopic tumors. Except for insulinoma, more than 60% of them reveal malignant behavior. The presence of endocrine tumor is diagnosed when patients develop a hormone-specific symptom, and the location of tumors are usually diagnosed by a combination of ultrasonography, computed tomography, magnetic resonance imaging and selective angiogr

Increased pancreatic acinar content and secretion of cationic trypsinogen following 30-day continuous ethanol intoxication in rats.

The effects of sustained, high blood alcohol levels (216 +/- 120 mg/100 ml, S.D.) for 30 days on cholecystokinin (CCK)-mediated pancreatic exocrine function were studied in a rat model that achieves both maximally controlled, optimal nutrition and high alcohol intake (approximately 40.5% of total calories). In alcohol-fed rats, basal plasma levels of immunoreactive cationic trypsinogen (ICT) were reduced by 50% (P less than 0.05), but intravenous doses (0-30 IDU/kg/hr; 1 IDU = approximately 62.5 ng CCK-8) of cholecystokinin octapeptide (CCK-8) resulted in a 3-fold greater maximal concentration
